Risdiplam Impact in Treatment Naïve and Non-Naïve Pediatric and Adult Patients With Spinal Muscular Atrophy.
Risdiplam, an oral disease-modifying therapy, has demonstrated safety and efficacy in clinical trials, but real-world data from Asia are limited. This prospective study followed 34 symptomatic SMA patients (24 adults, 10 children) over 36 months of risdiplam treatment. The cohort included 5 patients with SMA type 1, 25 with SMA type 2, and 4 with SMA type 3, with ages ranging from 6.9 to 50.9 years. Most (91%) were wheelchair users, and 74% had scoliosis managed by bracing or spinal surgery. Twenty-five patients (22 adults, 3 children) were treatment naïve, and 9 patients (2 adults, 7 children) switched from nusinersen. Motor function was assessed at baseline, 6, 12, 24, and 36 months using MFM-32, RULM, ATEND, and HFMSE. In pediatric patients, HRQOL was evaluated using PedsQL Neuromuscular Module and PedsQL Generic Core scale. HRQOL of their parents was assessed with PedsQL Family Impact Module. After 36 months, MFM-32 score was significantly improved in those switching therapies [p = 0.042], while both MFM-32 and RULM scores were significantly improved in treatment-naïve group [p = 0.005 and p = 0.043]. Parents experienced HRQOL gains in their Physical domain (2 years: PedsQL Family Impact Module, p = 0.038). Pediatric patients showed improvement in the Communication domain (3 years: PedsQL Neuromuscular Module, p = 0.042). Risdiplam was generally well tolerated; however, two adults discontinued due to adverse effects. Moreover, two adult patients died during treatment. This study fills Asian real-world data gaps, stresses the importance in monitoring in severe SMA, and underscores the need for larger, long-term safety and efficacy studies.
Quantitative Whole-Body Muscle MRI in Adults With Spinal Muscular Atrophy-A Sensitive Tool for Long-Time Evaluation of Disease Progression.
Spinal muscular atrophy (SMA) is a neuromuscular disorder characterized by progressive muscle weakness due to SMN protein deficiency. While effective therapies exist, their impact on slowly progressive adult SMA patients remains unclear. Reliable biomarkers for monitoring disease progression and treatment response are urgently needed. This pilot study evaluated the utility of longitudinal quantitative muscle MRI (qMRI) to monitor disease progression in adult SMA patients treated with nusinersen over an extended period. Nine adult patients with genetically confirmed 5q-SMA underwent whole-body muscle MRI and clinical assessment, including the Hammersmith Functional Motor Scale-Expanded (HFMS-EXP), Revised Upper Limb Module (RULM), and 6 min walk test (6MWT). Muscular fat fraction (mFF) was quantified in 20 muscles over a median follow-up of 54 months. Baseline mFF correlated strongly with clinical measures (HFMS-EXP: r = -0.90, p = 0.001; 6MWT: r = -0.96, p < 0.001), but not with age at onset or age at MRI. Over the observation period, a significant increase in mFF was detected (averaged annual increase of all studied muscles: 0.47%, p = 0.011), accentuated in the lower leg muscles. In contrast, clinical measures showed no consistent change. Consequently, no significant correlations were found between changes in mFF and clinical scores. This study provides the longest reported longitudinal qMRI assessment in adult SMA patients treated with nusinersen, demonstrating that mFF progressively increases despite stable clinical scores. The results suggest that qMRI may be a sensitive and objective biomarker for detecting subtle disease progression in adult SMA, potentially surpassing clinical measures.
Workshop report: Findings from the 2025 Italian SMAkers Educational Initiative on SMA management in Italy.
Newborn screening programs have changed the landscape of spinal muscular atrophy (SMA). The impact on presymptomatic patients is profound, but several challenges remain, such as how to communicate diagnostic results to families, when treatment should be started in screen-positive but presymptomatic patients, and how to manage adults who now survive longer with the disease. A workshop held in Florence in May 2025, as part of the SMAkers Study Group activities, offered clinicians across the Italian SMA Network the chance to discuss these open questions. Over two days, plenary lectures were alternated with working group sessions on communication strategies, presymptomatic management, adult care, and fatigability. Prior to the meeting, a survey collected data on current practice in Italy. Centers broadly agreed on a multidisciplinary, family-centered approach that integrates genetic results, neurophysiology, and clinical findings into treatment planning for newborns, incorporating CMAP in addition to SMN2 copy number and neurofilament light chain, measured by some sites. There was consensus on care of adult patients, including assessments of motor and respiratory functions, bulbar status, fatigability, patient-reported outcomes and psychological health. Despite known survey limitations, these data reflect a coherent Italian model reinforcing the value of shared multidisciplinary care within a structured network.
Electrophysiological assessment of motor unit loss in adult spinal muscular atrophy types III and IV: a multicenter national study comparing MUNIX, CMAP, and MUSIX.
Spinal muscular atrophy (SMA) types III and IV are the most common late-onset forms, and they progress slowly, making the identification of sensitive biomarkers critical. The Motor Unit Number Index (MUNIX) estimates motor unit loss and may complement traditional electrophysiological measurements such as Compound Muscle Action Potential amplitudes (CMAP). However, their respective performances have never been directly compared in adult SMA. In a French multicenter study (NCT04690998), 71 adult patients with SMA and 24 healthy controls underwent clinical and electrophysiological evaluation. MUNIX, CMAP, and Motor Unit Size Index (MUSIX) were recorded in four muscles, and sum scores (SumMUNIX, SumCMAP, SumMUSIX) were calculated. Reliability was assessed using intraclass correlation coefficients (ICCs), and associations with functional outcomes were explored. MUNIX and CMAP effectively distinguished SMA patients from controls, showing strong test - retest reliability. MUNIX showed the highest discriminative performance (AUC = 0.92), while CMAP demonstrated the strongest and most consistent associations with clinical severity. In multivariate analyses, only CMAP remained independently associated with all functional and strength measures, whereas MUNIX and MUSIX lost significance. MUNIX demonstrated the highest discriminative performance among biomarkers for differentiating SMA from controls, indicating early motor unit loss even when CMAP values were within normal limits. However, disease burden and functional impairment were better reflected by CMAP, probably due to it integrating both motor unit loss and reinnervation. The complementary nature of these profiles supports their combined use (concurrent application), and longitudinal studies are warranted to assess their responsiveness in adult SMA as well as their appropriateness for clinical trial settings.
Administration practice and adherence of nusinersen in adults with 5q-spinal muscular atrophy in China: an ambispective multicenter study.
Due to nusinersen’s route of administration and dosing regimen, the spinal muscular atrophy (SMA) population and/or their caregivers need to travel to hospitals and work around physician’s schedules to receive treatment. Real-world studies with large sample sizes on administration practice and adherence rate of nusinersen are lacking, especially in non-United States settings. This study aimed to investigate the administration practice and adherence of nusinersen in Chinese adults with 5q-SMA. An ambispective, multicenter registry of adults with 5q-SMA in China provided the longitudinal data for this analysis. Nusinersen was scheduled on Day 0, 14, 28, 63, and every 4 months thereafter. Adherence rate was calculated at dose level. A dose was deemed adherent if the interval between the current and preceding dose aligned with the standard dosing regimen, allowing a grace period of ± 7 days for doses 2 through 4 and ± 28 days for all subsequent doses. A total of 177 participants receiving nusinersen with 1,329 doses were included in the study. All injections were given in inpatient setting with no ventilatory support or sedation needed. Only one injection (0.1%) was performed under general anesthesia. Almost all (n = 1,322, 99.5%) injections were given through interspinous lumbar puncture. Injections in participants with scoliosis require more imaging techniques for guiding administration compared with those in participants without scoliosis (68.3% vs. 29.1%). Ultrasound was the most commonly used guide before or during administration (674/708). The adherence rate was 92.8% (1,062/1,144). The median inter-dose intervals aligned well with dosing schedule, with 14 days for Dose 2 and 3, 35 days for Dose 4, and 120–124 days for Dose 5 to 10 as maintenance doses. Findings characterize the patterns of nusinersen administration practice, demonstrating high real-world adherence to nusinersen administration in Chinese adults with 5q-SMA. The registry was registered at clinicaltrials.gov (NCT05618379) on November 16th, 2022. The online version contains supplementary material available at 10.1186/s12883-026-04730-x.
