Common and Distinct Features in Serum Proteomic Profiles in Keratoconus, Post-Laser Vision Correction Ectasia, and Pellucid Marginal Degeneration.
Both intrinsic and extrinsic factors are implicated in keratoconus (KTCN), pellucid marginal degeneration (PMD), and post-laser vision correction (PLVC) ectasia etiology. Serum proteomic profiling was performed to molecularly differentiate the ectasia and to assess systemic components in their pathophysiology. Serum samples from 93 patients with KTCN, 10 patients with PLVC, 4 patients with PMD, and 44 non-ectatic controls were profiled using matrix-assisted laser desorption ionization-time of flight tandem mass spectrometry (MALDI-TOF/TOF MS/MS). Clinical and environmental variables (e.g. allergy/atopy/asthma and eye rubbing intensity), together with ophthalmologic parameters (K1, K2, Kmax, TCT, and anterior/posterior elevation), were assessed in the principal component analysis (PCA), Weighted Gene Co-expression Network Analysis (WGCNA), linear modeling, and other assessments. Serum m/z features were correlated with RNA-seq data from corneal epithelium (CE) samples of 31 individuals. As findings of the PCA, associations were observed for atopy and/or asthma (estimate = -5.52, P = 0.03), PMD diagnosis (estimate = 8.3, P = 0.04), and age (estimate = -0.14, P = 0.01), with observable trends for KTCN status and intense eye rubbing. Correlations between clinical variables and individual m/z features indicated that posterior elevation, reflecting KTCN severity, was associated with an ATP5B protein-assigned peak (m/z 1921.9655, ρ = -0.31, adjusted P = 0.016). In the integration analyses, correlations between serum peaks and corresponding CE gene expression, including APOH- (ρ = 0.43, P = 0.017) and ALB-assigned peaks (ρ = 0.35-0.42, P < 0.05) were identified. Pairwise eigengene comparisons showed negative correlation between KTCN and non-ectatic controls (Pearson r = -0.89, P = 0.007). The local ocular pathology is reflected in serum proteomic profiles, together with accompanying systemic inflammation and intense eye-rubbing behavior. Common proteomic features of different types of corneal ectasia may reflect overlapping pathophysiological mechanisms.
Acute Corneal Hydrops: A Comprehensive Review of Diagnosis, Treatment, and Outcomes.
This review provides an overview of acute corneal hydrops (ACHs), focusing on their risk factors, epidemiology, pathogenesis, imaging techniques, and treatment options. This narrative review was based on a literature search conducted in PubMed and Google Scholar, covering publications from 1980 to 2025. No restrictions were applied regarding language or study type. The identified studies were assessed for their relevance and clinical applicability to the diagnosis, management, and outcomes of ACHs. ACHs are linked with corneal conditions like keratoconus (2.6%-7.5%), pellucid marginal degeneration (6%-11.5%), and keratoglobus (11%). It arises from structural changes in Bowman layer and the anterior stroma, triggered by factors such as trauma and increased intraocular pressure, leading to rupture of Descemet membrane. Risk factors include chronic allergic eye diseases (eg, vernal and atopic keratoconjunctivitis), eye rubbing, developmental conditions (eg, Down syndrome), early age, steeper keratometry, and reduced visual acuity at presentation. Imaging techniques such as ultrasound biomicroscopy, anterior segment optical coherence tomography, and in vivo confocal microscopy aid in characterizing CHs, monitoring recovery, and detecting complications. Treatments aim to reduce discomfort, promote healing, prevent complications, and support future visual rehabilitation. ACH develops in several corneal disorders, with risk factors overlapping those of keratoconus. Advances in imaging have improved the understanding of corneal changes, aiding in treatment and follow-up strategies. Despite various treatment options, no consensus exists on the best approach, underscoring the need for further studies to refine management and improve patient outcomes.
Update on pellucid marginal degeneration.
To review recent developments (2014-2024) in the diagnosis and treatment of pellucid marginal degeneration (PMD), focusing on advances in corneal imaging technologies, biomechanical assessments, and the effectiveness of current contact lens treatments and surgical techniques. Additionally, to discuss the role of PMD in cataract surgery and the use of toric intraocular lenses (IOLs). Structured search of PubMed, Scopus, and Embase (Nov 2024; Jan 2014-present) using PMD-related keywords/MeSH. After dual independent screening of 129 citations using predefined inclusion/exclusion criteria, 50 articles met eligibility and were included. Recent technologies enable better assessment of corneal curvature and pachymetry relationships, aiding differentiation of PMD from other ectatic diseases. Advances in biomechanical assessments have improved early detection and diagnostic precision. Most patients can be managed conservatively with new types of rigid contact lenses. Modified corneal cross-linking (CXL) protocols show promise in stabilizing the disease. Lamellar transplants offer visual outcomes comparable to penetrating keratoplasty with fewer complications. Wedge resection shows promising outcomes, but larger studies with long-term follow-up are still required. In cataract surgery, toric IOLs may provide effective astigmatism correction in selected PMD patients. The diagnosis and treatment of PMD have significantly evolved in the last decade. Improved imaging and biomechanical assessments aid in accurate diagnosis. Contact lenses and lamellar surgery are mainstay treatments. Future clinical studies should focus on the CXL protocols.
Effect of Customized Corneal Crosslinking on Patients With Pellucid Marginal Degeneration.
To evaluate the effect of customized corneal crosslinking on pellucid marginal degeneration (PMD). Twenty-eight eyes with PMD were included. Fifteen eyes were treated with customized corneal crosslinking at Helsinki University Eye Hospital. Three treatment zones with different ultraviolet energy levels were planned according to corneal tomography, focusing on the inferior part of the cornea. The control group consisted of 13 conventionally crosslinked eyes. Visual acuity, refraction, and corneal tomography were obtained preoperatively and at 1 month, 6 months, and 1 year after treatment. Corneal tomography showed stability and visual acuity improvement after customized crosslinking in all but 1 patient with a 1-year follow-up. Mean change in logMAR visual acuity was -0.15 ( P = 0.02). However, the improvement in Kmax was not significant (-0.11D, P = 0.8). In the control group, the mean visual acuity did not change (-0.04, P = 0.44), although there were significant changes in keratometry (Kmax -0.81 D, P = 0.02 and Kavg -0.57 D, P = 0.007). Customized corneal crosslinking can be safely used in patients with PMD. No clear advantages over standard crosslinking were observed in this study.
Coincident Pellucid Marginal Degeneration and Fuchsiridocyclitis: a case report.
We report a rare coexistence of Pellucid Marginal Degeneration (PMD) and Fuchs' heterochromic iridocyclitis (FHI) in a 40-year-old female. The overlap of corneal ectasia and chronic uveitis created significant diagnostic and surgical challenges, highlighting the importance of individualized planning in complex anterior segment diseases. The patient presented with painless, progressive visual decline in the right eye. Clinical examination revealed inferior corneal thinning, mature cataract, pigmented keratic precipitates, and peripheral anterior synechiae (PAS). Corneal tomography confirmed PMD with a characteristic "crab-claw" pattern. The patient underwent phacoemulsification with posterior chamber intraocular lens implantation. Postoperatively, visual acuity improved, although optic disc cupping and persistent PAS underscored the need for long-term glaucoma surveillance. The coexistence of PMD and FHI is exceptionally rare. PMD complicates surgical planning by affecting keratometric reliability and corneal biomechanics, while FHI contributes to cataract formation, intraocular inflammation, and risk of secondary glaucoma. Multimodal imaging, tailored surgical strategy, and vigilant postoperative follow-up are key to optimizing outcomes. Simultaneous PMD and FHI represent a unique clinical entity requiring comprehensive assessment and individualized management. Tailored surgical planning and vigilant surveillance are essential to preserve vision and mitigate long-term risks.
