Prevalence of germline pathogenic variants in cancer predisposition genes in the Japanese population.
Germline pathogenic variants (GPVs) in cancer predisposition genes increase cancer risk, but their population-level prevalence in Japan remains unclear. To estimate the prevalence of GPV carriers of hereditary cancer predisposition syndromes (HCPSs) in the Japanese population. A cross-sectional analysis of the Tohoku Medical Megabank Organization datasets was conducted to assess allele frequencies of rare GPVs, estimate carrier prevalence, and describe variant distribution. The cumulative allele frequency of rare GPVs of cancer predisposing genes was 1.47% for autosomal dominant and 3.20% for autosomal recessive diseases. The estimated prevalence of GPV carriers was 2.90% (1 in 35 individuals) and 0.10% (1 in 977 individuals) for autosomal dominant HCPSs and autosomal recessive HCPSs, respectively; ~6.19% of the population (1 in 16 individuals) were carriers. Variant types differed across genes, and hotspot variants had a significant impact on allele frequencies. Moreover, 47 structural variants (0.28%) having the potential to cause structural disruption were identified. Allele frequency analysis of rare GPVs of cancer predisposition genes offered valuable insight into the prevalence of GPV carriers of HCPSs in the Japanese population. Hotspot variants may represent founder mutations within specific ethnic groups, and their presence or absence could affect gene-specific allele frequencies.
Successful Management of Suspected Epstein-Barr Virus-Associated Hemophagocytic Lymphohistiocytosis in BRAF-Mutant Cholangiocarcinoma Following Treatment With Immune Checkpoint Inhibitors and BRAF/MEK Inhibitors: A Case Report.
The BRAF V600E mutation is a rare genetic alteration in cholangiocarcinoma for which sequential therapy with immune checkpoint inhibitors (ICIs) and BRAF/MEK inhibitors may be effective. However, the full spectrum of adverse events associated with the sequential use of these agents remains unclear. Hemophagocytic lymphohistiocytosis (HLH) is a fatal cytokine release syndrome that can be triggered by a cytokine storm. A 49-year-old man underwent left lateral hepatectomy and adjuvant chemotherapy for cholangiocarcinoma and subsequently developed lymph node recurrence. The patient initially received systemic chemotherapy with gemcitabine, cisplatin, and S-1; however, due to disease progression, the regimen was switched to gemcitabine, cisplatin, and durvalumab. After these treatments, a BRAF V600E mutation was identified through comprehensive gene panel testing, leading to the initiation of BRAF and MEK inhibitors. However, 3 months later, the patient presented to the emergency department of Steel Memorial Muroran Hospital with fever and fatigue in June 2024. He was initially diagnosed with septic shock but was unresponsive to broad-spectrum antibiotics. Laboratory tests revealed elevated ferritin levels, elevated soluble interleukin-2 receptor levels, and Epstein-Barr Virus (EBV)-DNA. HLH was diagnosed, and multidisciplinary treatment, including steroid pulse therapy, was initiated. The patient's condition improved dramatically, and he survived. We report a rare case of cholangiocarcinoma with suspected EBV-associated HLH that developed after sequential therapy with an ICI and BRAF/MEK inhibitors. Clinicians should consider HLH as a differential diagnosis in patients with a history of ICI therapy who present with severe unexplained inflammation or shock. Prompt diagnosis and multidisciplinary management are crucial to prevent death.
Contribution of germline and somatic mutations to risk of neuromyelitis optica spectrum disorder.
Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune disease characterized by optic neuritis and transverse myelitis, with an unclear genetic background. A genome-wide meta-analysis of NMOSD in Japanese individuals (240 patients and 50,578 controls) identified significant associations with the major histocompatibility complex region and a common variant close to CCR6 (rs12193698; p = 1.8 × 10-8, odds ratio [OR] = 1.73). In single-cell RNA sequencing (scRNA-seq) analysis (25 patients and 101 controls), the CCR6 risk variant showed disease-specific expression quantitative trait loci effects in CD4+ T (CD4T) cell subsets. Furthermore, we detected somatic mosaic chromosomal alterations (mCAs) in various autoimmune diseases and found that mCAs increase the risk of NMOSD (OR = 3.37 for copy number alteration). In scRNA-seq data, CD4T cells with 21q loss, a recurrently observed somatic event in NMOSD, showed dysregulation of type I interferon-related genes. Our integrated study identified novel germline and somatic mutations associated with NMOSD pathogenesis.
Recent advances in the treatment of FGFR-altered cholangiocarcinoma.
Cholangiocarcinoma (CCA) is a rare, highly lethal biliary malignancy comprising intrahepatic (iCCA), perihilar (pCCA), and distal (dCCA) subtypes, with a rising global incidence. Surgery is the only curative option, yet most patients present with advanced disease, making systemic therapy the mainstay. Molecular profiling has revealed marked heterogeneity, and fibroblast growth factor receptor 2 (FGFR2) fusions in iCCA represent one of the most actionable targets. An extensive literature search was performed in PubMed/MEDLINE, Embase, and ClinicalTrials.gov to identify preclinical studies, clinical research, and clinical trials evaluating FGFR inhibitor therapy in CCA. This review summarizes FGFR signaling biology, the prevalence and diagnostic challenges of FGFR2 fusions, and clinical evidence for FGFR inhibitors. Resistance mechanisms, diagnostic strategies, toxicity profiles, and ongoing trials are also reviewed. FGFR inhibitors are transitioning from experimental to established targeted options, underscoring the need for routine molecular profiling and optimized fusion detection. Key challenges include overcoming resistance, refining patient selection, and defining the role of FGFR inhibition in combination strategies and earlier-line or perioperative settings. Further evidence is needed to clarify long-term clinical benefit in FGFR2-altered CCA.
Novel organoid-based exploration reveals the role of LMTK3/FADS2 signaling in metastatic breast cancer progression in felines and humans.
Cancer research in veterinary medicine is still under development compared to human medicine. Feline breast cancer (FBC) is highly malignant, intractable, and has the potential to become a valuable model for human metastatic BC (HBC). We developed primary FBC organoids and analyzed their morphology, gene expression patterns, and response to anti-cancer drugs. FBC organoids recapitulated the structure and behavior of tumor cells, exhibiting diverse hormone receptor expressions and tumorigenic potential. Moreover, varying sensitivities to chemotherapies and targeted drugs were evident across FBC organoid lines. Furthermore, LMTK3 was significantly upregulated in FBC organoids compared to feline normal mammary organoids (FNM). Further, LMTK3/FADS2 pathway was revealed to be implicated in progression by influencing cell proliferation, invasion, and apoptosis of FBC organoids. Treatment with C28 (an LMTK3 inhibitor) also prevented cell viability of human BC organoids. The survival time of human BC patients with high co-expression of LMTK3 and FADS2 was shorter than that with low co-expression. These findings highlight the importance of LMTK3/FADS2 pathway in BC progression and indicate that FBC organoids might help to do comparative research and identify conserved mechanisms between HBC and FBC.
