Tratamento e manejo
O objetivo é controlar as crises epilépticas e prevenir danos neurológicos progressivos.[1]
Prognóstico e qualidade de vida
O prognóstico varia amplamente conforme a doença metabólica específica, a idade de início, a gravidade do comprometimento neurológico e a precocidade do tratamento.

Mitochondrial Dysfunction in Monogenic Developmental and Epileptic Encephalopathies.
Before diagnostic whole exome sequencing, monogenic/chromosomal developmental and epileptic encephalopathies (DEEs) were frequently misdiagnosed as mitochondrial disorders (MDs) with epilepsy, due to overlapping clinical and biochemical features. Assessing muscle functional assays in patients with a genetic diagnosis and epilepsy offers a unique opportunity to explore mitochondrial dysfunction in monogenic/chromosomal DEEs, in comparison to the mitochondrial dysfunction observed in genetically confirmed MDs. In this retrospective cohort study, clinical and biochemical data were extracted from patients suspected of MD with epilepsy who underwent muscle/fibroblast biopsy (2005-2015). Patients were classified into four groups based on the final diagnosis. Mitochondrial Disease Criteria scores were assigned. Statistical analyses were conducted using Fisher's exact, analysis of variance, and Kruskal-Wallis tests. Of 27 included participants, eleven (40.7%) had DEEs, four (14.8%) had genetically confirmed MDs, eight (29.6%) were suspected MD cases without genetic confirmation, and four (14.8%) had nonmitochondrial metabolic diseases. Mitochondrial dysfunction was common across all groups; 85% of participants met probable/definite Mitochondrial Disease Criteria, over 70% had elevated plasma lactate (>2.5 mmol/L), and 92% exhibited impaired adenosine triphosphate production. Surprisingly, moderate to severe complex dysfunction was observed in all groups except genetically confirmed MDs. Our findings indicate that mitochondrial dysfunction is prevalent in nonmitochondrial DEEs. Patients previously diagnosed with an MD based only on muscle/fibroblast biopsy may benefit from whole exome sequencing to identify genetic variants, for which targeted therapy may be available. Future research should explore whether treatment or prognosis of nonmitochondrial DEEs should be tailored to improve mitochondrial function.
Molecular mechanisms underlying the ferroptosis-induced epileptiform activity in mouse cortical slices.
Ferroptosis, a newly defined iron-dependent programmed cell death, characterized by excessive accumulation of lipid peroxides and reactive oxygen species, is involved in epilepsy, particularly in those forms resistant to drugs. In a previous study, we have demonstrated that exposure of mouse cortical slices to the ferroptosis inducer RSL-3, induces interictal epileptiform discharges. To investigate the mechanisms underlying ferroptosis-induced epileptic activity in RSL-3-treated cortical slices, we analysed the expression of the main contributors to ferroptosis susceptibility in cells. The expression of proteins involved in RSL-3 induced ferroptosis pathways were analysed on mouse cortical slices by Western blot and qRT-PCR. The epileptogenic response was investigated by electrophysiological patch clamp recordings, in current clamp mode, from layer 2/3 mouse cortical slices. In cortical neurons, the ferroptosis induction by RSL-3 was associated with a reduced expression of the GPX4/GSH redox pathway, responsible for the clearance of lipid peroxides, and an upregulation of 15-LOX, which promotes the formation of lipid peroxides. Furthermore, the cysteine/glutamate antiporter Xc-, a modulator of excitotoxicity in the brain, was up-regulated either after RSL-3-treatment or by incubating neurons with 4-HNE, the bioactive product of lipid peroxidation. Interestingly, 4-HNE was able to generate spontaneous interictal bursts. Our findings establish a direct link between lipid peroxidation and Xc- activity. These results suggest that the xCT antiporter may represent a promising therapeutic target for treating ferroptosis-mediated drug-resistant epilepsy.
Stimulating amyloid-β clearance in cerebral amyloid angiopathy with low-sodium oxybate and/or non-invasive vagus nerve stimulation (Clear-Brain): study protocol for a randomised pre-post trial.
Cerebral amyloid angiopathy (CAA) is caused by the accumulation of amyloid-beta (Aβ) in the cerebrovasculature. The glymphatic system is thought to be involved in the clearance of cerebral waste products, including Aβ. Stimulation of the glymphatic system through enhancing deep sleep with low-sodium oxybate (LXB) or inhibition of cortical spreading depolarisations via non-invasive vagus nerve stimulation (nVNS) could potentially increase clearance of Aβ and hence improve disease course. We will perform a pre-post trial to assess whether treatment with LXB, nVNS or a combination of both interventions can enhance the clearance of Aβ in patients with CAA. A total of 60 participants, 30 with sporadic CAA and 30 with Dutch-type CAA, will be randomly assigned to receive either LXB, nVNS or both interventions, resulting in three groups (20 in each group: LXB, nVNS and both). The study spans 6 months, comprising a 3-month observational phase and a 3-month intervention phase. The primary outcome measure will be the morning levels of Aβ40 and Aβ42 in cerebrospinal fluid (CSF) before and after the intervention. We will assess possible disease progression with (non-)haemorrhagic imaging markers on 7-Tesla MRI at baseline, before and after intervention, as a secondary outcome. Additionally, the activity of the glymphatic system by means of fluid dynamics will be assessed with CSF-Selective T2-weighted Readout with Acceleration and Mobility encoding (CSF-STREAM) on 7-Tesla MRI. The study was reviewed and approved by the Medical Research Ethics Committee Leiden The Hague Delft (P23.100) on 8 April 2024. The first participant was enrolled on 27 March 2025. Study results will be published in peer-reviewed journals and presented at scientific conferences. Additionally, study updates and results will be shared with participants via our newsletter twice a year. Molecular connectivity studies of cerebral glucose metabolism and blood flow: A scoping review.
The brain's functional organization relies on neural, metabolic, and vascular interactions. Molecular neuroimaging offers powerful tools for assessing macroscale brain connectivity by capturing relationships between regional perfusion and glucose metabolism. This review summarizes molecular connectivity studies of cerebral blood flow (CBF) and metabolism, focusing on methodological approaches and key findings. A systematic search across MEDLINE, EMBASE, and Scopus identified studies employing radiotracers to examine brain perfusion or glucose metabolic connectivity. Data extraction focused on tracer type, connectivity methodology, population, and clinical relevance. Overall, 384 studies were included, covering healthy condition, dementia, movement disorders, psychiatric diseases, epilepsy, and disorders of consciousness. Both resting-state and task-based paradigms were identified, with perfusion studies being popular for detecting fast task-induced molecular connectivity changes. Metabolic connectivity, assessed via [18F]FDG-PET at rest, emerged as robust marker of functional integrity and disease progression, especially in neurodegenerative conditions. Multimodal PET/MRI studies revealed partial overlap between metabolic and hemodynamic connectivity. Noteworthy findings include the identification of default mode network through the study of CBF and disease-related covariance patterns in neurodegenerative disorders through the study of glucose metabolism. Integrating macroscale molecular brain organization studies with neurophysiological techniques will deepen the understanding of brain connectivity in health and disease. Additionally, total-body PET/MRI data may in the future elucidate brain-body interactions fostering a more comprehensive connectome framework.
Glymphatic System Dysfunction in Central Nervous System Diseases.
The glymphatic system is a perivascular cerebrospinal fluid (CSF)-interstitial fluid (ISF) exchange pathway that supports brain homeostasis by clearing metabolic waste and neurotoxic proteins. Across central nervous system diseases, converging evidence indicates that glymphatic dysfunction represents a shared pathophysiological axis linking vascular, astroglial, inflammatory, and sleep-related disturbances to impaired solute clearance. In this review, we synthesize mechanistic and clinical evidence for glymphatic impairment in acute brain injury (ischemic and hemorrhagic stroke, traumatic brain injury) and chronic neurological disorders (Alzheimer's disease, Parkinson's disease, cerebral small vessel disease, multiple sclerosis, idiopathic normal pressure hydrocephalus, idiopathic intracranial hypertension, epilepsy, and headache disorders). Major mechanisms include (i) aquaporin-4 (AQP4) depolarization/mislocalization at astrocytic endfeet, reducing perivascular water transport; (ii) perivascular space compression or obstruction from cytotoxic/vasogenic edema, blood-derived products, protein aggregates, or altered extracellular matrix; (iii) loss of arterial pulsatility and vascular stiffening, weakening the driving forces for convective exchange; (iv) blood-brain barrier disruption and neuroinflammation, which remodel perivascular architecture and amplify clearance failure; and (v) sleep and autonomic dysregulation, including altered noradrenergic tone, which suppresses glymphatic activity during periods when clearance is normally maximal. Clinically, glymphatic dysfunction can be probed using diffusion tensor imaging-analysis along the perivascular space (DTI-ALPS), contrast-enhanced MRI approaches, and structural surrogates such as enlarged perivascular spaces, with emerging associations to cognition, mood, and disease severity. Finally, we discuss translational strategies aimed at restoring clearance, including sleep/circadian optimization, vascular risk control, anti-inflammatory approaches, AQP4- and TRPV4-oriented targets, and neuromodulation. Mechanism-guided, standardized imaging and longitudinal interventional studies are needed to establish glymphatic biomarkers as actionable therapeutic and prognostic tools.
