A Murine Model of HSV-1-Induced Corneal Endotheliitis: Primary Infection, Latency, and Recurrence.
HSV-induced corneal endotheliitis (HSV endotheliitis) is a sight-threatening ocular disease, with poor underlying its pathogenesis due to the lack of suitable animal models, we therefore aimed to develop a murine HSV endotheliitis model. Herpes simplex virus type 1 (HSV-1) was injected into the anterior chamber (AC) of C57BL/6 mice to establish primary and latent infection. Recurrence was induced at least 5 weeks post-infection via ultraviolet B (UVB) corneal exposure. Clinical manifestations were dynamically monitored. The histopathology and molecular changes were assessed using transmission electron microscopy, immunofluorescence (IF) staining, hematoxylin and eosin (H&E) staining and multiple cytokines and chemokines analysis. HSV-1 load in corneas and trigeminal ganglia (TG) was detected via plaque assay, quantitative real-time PCR and IF staining. Compared with controls, the primary HSV-1-infected mice exhibited characteristic manifestations of viral endotheliitis, including corneal and iris edema, AC inflammation, keratic precipitates (KPs), elevated intraocular pressure, and corneal endothelial cell damage and loss. Early after infection, viral loads were elevated both in corneas and TGs. By day 28, however, a latency-associated transcript (LAT) in TGs increased markedly, while HSV-1 titers became undetectable, indicating viral latency. Following UVB corneal exposure, the recurrent mice showed significant stromal edema, increased KPs, extensive endothelial cell loss, and elevated viral loads, confirming the successful induction of recurrence. We successfully established a murine model of primary infection, latency and recurrence of HSV endotheliitis, providing a reliable platform for investigating pathological mechanisms and potential treatments.
Clinical Characterization of Corneal Endotheliitis With Distinctive Features in Shiba Inu Dogs.
To characterize the clinical features of corneal endotheliitis in Shiba Inu dogs that differ from typical "blue eye," and to compare this condition with primary corneal endothelial degeneration. Sixteen Shiba Inu dogs (30 eyes) diagnosed with corneal endotheliitis. Medical records were retrospectively reviewed. Clinical findings, vaccination history, treatment, and outcomes were evaluated. Ophthalmic examinations included slit-lamp biomicroscopy, specular microscopy, and optical coherence tomography. The mean age of affected dogs was 5.9 ± 3.7 years, which was significantly younger than that of dogs with primary corneal endothelial degeneration. No clear temporal relationship was observed between vaccination and disease onset. The affected eyes exhibited conjunctival hyperemia, characteristic geographic corneal endothelial opacities, and localized corneal edema. Corneal endothelial cell density was significantly lower than that in clinically normal eyes but significantly higher than that in Shiba Inu dogs with primary corneal endothelial degeneration. Optical coherence tomography revealed hyper-reflective areas on the endothelial surface of the cornea. Topical steroidal anti-inflammatory therapy achieved rapid resolution of inflammatory signs and improvement of corneal edema in all cases; however, recurrence occurred in some eyes during tapering. Endothelial opacities persisted after resolution of inflammation, and gradual endothelial cell loss was observed in some eyes during long-term follow-up. Corneal endotheliitis in Shiba Inu dogs is characterized by geographic endothelial opacity, localized corneal edema, and favorable response to anti-inflammatory therapy. The clinical features, younger age of onset, and milder corneal endothelial damage suggest that this condition may differ from primary corneal endothelial degeneration.
Epstein-Barr Virus Corneal Endotheliitis and Uveitis in a Patient With Extranodal Natural Killer/T-Cell Lymphoma.
To report a case of Epstein-Barr virus (EBV) corneal endotheliitis and uveitis in a patient diagnosed with extranodal natural killer/T-cell lymphoma (ENKTL). This is a case report performed at a tertiary care center in the Republic of Korea. A 64-year-old man presented with decreased vision in his right eye that had persisted for 4 days. Best-corrected visual acuity was finger counting at 50 cm in the right eye and 0.8 in the left eye. Intraocular pressure was within the normal range in both eyes. Slit-lamp examination revealed scattered keratic precipitations and diffuse corneal edema in the right eye. Because of the severe edema, anterior chamber (AC) details were obscured. In the left eye, +2 inflammatory cells were observed in the AC and +1 cells in the vitreous. A multiplex polymerase chain reaction test using aqueous humor samples from each eye yielded positive results for EBV but negative results for other herpes viruses. One week after initiating treatment with a topical acyclovir ointment, topical steroids, and oral acyclovir, corneal edema and AC inflammation improved. Subsequently, the patient was diagnosed with nasal ENKTL and underwent 3 chemotherapy cycles in the hematology-oncology department. Five months after commencing topical and oral antiviral therapies, best-corrected visual acuities improved to 0.5 in the right eye and 1.0 in the left eye. EBV infection can cause severe corneal edema and uveitis. In patients with ENKTL, EBV should be considered a possible etiology of ocular involvement, and timely ophthalmic management may contribute to better visual outcomes.
Triple Immune Rings with Cytomegalovirus Corneal Endotheliitis.
Roles played by IL-8 in altering dynamics of trabecular meshwork cells after human cytomegalovirus infection.
Open-angle glaucoma (OAG) is the leading cause of blindness worldwide. Human cytomegalovirus (HCMV) is known to infect the trabecular meshwork cells (HTMCs) and corneal endothelial cells leading to chronic and recurrent elevations of the intraocular pressure (IOP) as secondary glaucoma. To investigate how HCMV affects the function of HTMCs, we analyzed the effects of HCMV infection on cultured HTMCs infected with the endothelial-adapted strain, TB40/E, of HCMV. We studied the induced molecular mechanisms focusing on the OAG-associated chemokines, IL-8 and CCL2. The HTMCs were analyzed for transcriptome changes using RNAseq analysis. Our results showed that HCMV infection activated interferon signaling and significantly increased the expression of IL-8 and CCL2. The IL-8-responsive transcriptional pathway was analyzed by using a CXCR2 antagonist which is associated with cellular movement and development of the hematological system. In contrast, the CCL2-sensitive pathway, assessed using a CCR2 antagonist, was linked to olfactory receptor signaling and keratinization. HCMV infection activated cell motility with the formation of lamellipodia and filopodia. The infection-induced activation of cell motility was dependent on both CXCR2 and CCR2, and IL-8 stimulated filopodia-mediated cell motility. HCMV infection also induced cell contraction that was dependent on CXCR2, but not on CCR2, and it involved the activation of Rac1/Cdc42. These results suggest that HCMV infection altered the cytoskeletal dynamics and contraction of the HTMCs in a CCR2- and CXCR2-dependent manner. These changes have the potential of causing an increase in the resistance to aqueous humor outflow in HCMV-associated anterior uveitis and corneal endotheliitis.
