Lupuslike Manifestations in Myelodysplastic Syndromes and Chronic Myelomonocytic Leukemia.
Immune-mediated inflammatory diseases are rare but increasingly reported among patients with myelodysplastic syndromes (MDS) or chronic myelomonocytic leukemia (CMML). Systemic lupus erythematosus (LE) and cutaneous LE associated with MDS/CMML have been rarely described, with atypical features and refractory disease. To provide a comprehensive description of the phenotype and therapeutic responses of LE associated with MDS/CMML and to compare them with idiopathic LE. This retrospective case-control study included nationwide, multicenter data from January 1975 to January 2023. Patients with MDS/CMML who either fulfilled classification criteria for systemic LE or had skin lesions diagnosed as cutaneous LE were included. For MDS/CMML systemic LE, a 2:1 case-control study was conducted with idiopathic systemic LE. Clinical features, centralized skin histopathology, and targeted next-generation sequencing were analyzed. Data were analyzed from May 2022 to June 2025. The clinical, pathological, and molecular features of LE occurring in the setting of MDS or CMML compared with idiopathic LE. Of 24 included patients, 9 (38%) were female, 15 (63%) were male, and the median (range) age at diagnosis was 65 (32-85) years. A total of 19 were diagnosed with systemic LE and 5 with cutaneous LE. The median (range) follow-up was 4.5 (1-31) years. Cutaneous involvement was the most common manifestation of LE (17 [71%]). Chilblain lupus was the predominant subtype (6 [35%]). Compared with idiopathic systemic LE, patients with MDS/CMML-associated LE were older (median [range] age, 65 [32-85] years vs 23 [11-55] years; P < .001), more frequently male (10 [53%] vs 3 [8%]; P = .008), had less kidney involvement (2 [10%] vs 27 [71%]; P < .001), had less articular involvement (7 [36%] vs 37 [97%]; P < .001), and had reduced anti-double-stranded DNA positivity (6 [32%] vs 29 [76%]; P = .001). The underlying hematologic diseases included MDS (16 [66%]) and CMML (8 [34%]), with 22 (92%) classified as lower risk (Revised International Prognostic Scoring System score of 3.5 or less). Centralized histopathological review reclassified 6 skin biopsies (50%) as MDS/CMML cutis. Identical myeloid variants were detected in blood and skin in 6 of 8 patients, supporting a clonal inflammatory process. Standard LE therapies were often poorly effective, while clone-directed therapies (azacitidine or allogeneic hematopoietic stem cell transplant) led to parallel hematologic and LE responses in 5 of 7 patients. In this study, MDS/CMML-associated lupuslike manifestations were a distinct entity mimicking systemic LE or cutaneous LE and characterized by clonal inflammation rather than classic autoimmunity in most cases. Early recognition is important, as treatment may require clone-targeting therapies rather than conventional LE therapy.
Cytomegalovirus Infection Mimics Manifestations of Underlying Diseases in Patients With Autoimmune Disorders: A Case Report and Literature Review.
Patients with autoimmune disorders are highly susceptible to infections including cytomegalovirus (CMV) leading to serious complications ranging from asymptomatic to severe systemic diseases. The first case was a 44-year-old woman with systemic lupus erythematosus (SLE) referred to the Rheumatology ward due to a necrotizing ulcer on the hand's finger and multiple ulcerative lesions on her lips and tongue. She had pancytopenia and tested positive for both herpes simplex virus (HSV) and CMV polymerase chain reaction (CMV-PCR). She was treated with ganciclovir for 14 days and subsequently recovered. The second case was a 55-year-old man, a known sarcoidosis case, admitted to the hospital due to lower extremities weakness and intraoral ulcers. The lab findings revealed leukopenia and elevated levels of ESR and CRP. Viral markers were all negative except for the CMV-PCR test. Electromyography and nerve conduction velocity (EMG-NCV) showed subacute axonal motor polyneuropathy in the lower limbs. Due to the high titers of CMV (500,000 copies/mL), he was treated with ganciclovir for 2 weeks and the symptoms improved dramatically. CMV infection in patients with inflammatory rheumatic diseases may lead to rare manifestations that can be misdiagnosed as a flare of the underlying disease.
Bullous Systemic Lupus Erythematosus in a Southeast Asian Cohort: High Systemic Association Driving Systemic Therapy.
Bullous systemic lupus erythematosus (BSLE) is a rare autoimmune blistering disorder occurring in association with systemic lupus erythematosus (SLE). Owing to its rarity, current knowledge remains limited, particularly in Southeast Asian populations. This study retrospectively evaluated the clinical, histopathologic, immunofluorescence, and serologic features of BSLE, together with a review of the existing literature. This review included patients diagnosed with BSLE at Siriraj Hospital, Mahidol University, between 2003 and 2024. Among 9,055 patients with cutaneous lupus erythematosus, 12 were identified as having BSLE (incidence 0.13%). The median age was 39.5 years with equal sex distribution. Most patients (75%) presented with BSLE at the time of SLE diagnosis, while others developed it later. Lesions appeared on sun-exposed and non-sun-exposed areas, mainly the extremities, and mucosal involvement occurred in 33.3%. Direct immunofluorescence most frequently demonstrated immunoglobulin (Ig) G deposition (91.7%), followed by complement 3 (83.3%), IgM (75%), and IgA (50%), typically with linear and/or granular deposition of IgG and C3 along the basement membrane zone. Antinuclear antibodies were present in all patients. Systemic involvement was common (75%), most frequently affecting the kidneys (66.7%), followed by hematologic abnormalities (58.3%). Owing to disease severity, most patients required treatment with systemic corticosteroids in combination with immunosuppressive agents rather than dapsone alone to achieve disease control. This study and literature review highlight that, although BSLE is an uncommon cutaneous manifestation of SLE, it is strongly associated with systemic involvement, particularly renal and hematologic disease, which often necessitates treatment with immunosuppressive agents.
Skin-Limited Neonatal Lupus in an Infant Conceived by In Vitro Fertilization and Surrogate Gestation.
Neonatal lupus erythematosus (NLE) is a rare autoimmune condition caused by the transplacental passage of maternal anti-Ro/SSA and anti-La/SSB antibodies, typically presenting with annular erythema or congenital heart block. We report a 3-month-old infant conceived by in vitro fertilization and carried by a surrogate mother who developed annular erythematous plaques in typical and atypical locations at 1 month of age. Skin biopsy and serologic testing confirmed NLE despite the absence of a known maternal autoimmune history; maternal anti-Ro/SSA and anti-La/SSB positivity was identified retrospectively through the surrogate's obstetrician. The cutaneous findings improved significantly with photoprotection and resolved without systemic involvement.
Erythroderma-Like Manifestations and High Fever in Palisaded Neutrophilic and Granulomatous Dermatitis Without Any Underlying Systemic Immunological Disease.
Palisaded neutrophilic and granulomatous dermatitis (PNGD) is a reactive granulomatous dermatitis; the umbrella term includes interstitial granulomatous dermatitis and interstitial granulomatous drug reaction, in addition to PNGD. The histological findings of PNGD include leukocytoclastic vasculitis, neutrophilic infiltrates, and collagen degeneration with palisades of histiocytes and small granulomas. Clinically, the patients show symmetrical, smooth papules that are skin-colored to red, some with central umbilication or crusting, located mainly on the elbows and extremities. PNGD usually occurs with some systemic diseases, such as systemic lupus erythematosus, suggesting a cutaneous reaction pattern due to underlying internal inflammation. Although the present case was histologically diagnosed as PNGD, while excluding other granulomatous diseases, multiple indurated erythematous papules with a tendency to coalesce widely were seen, mimicking erythroderma without any underlying immunological diseases. In addition, the eruption coincided with fluctuating high fever. This was a rare case of PNGD with no underlying immunological disease that presented with severe manifestations such as fever and erythroderma.
