NON-INFECTIOUS POSTERIOR UVEITIDES - Atypicals, Variants, and Masquerades: the jungle of differential diagnosis.
Non-infectious posterior and panuveitides (NIPUs) comprise a heterogeneous group of inflammatory disorders of the outer retina and choroid, historically referred to as "white dot syndromes." Recent consensus efforts by the Multimodal Imaging in Uveitis (MUV) Task Force have established standardized diagnostic criteria for the major NIPUs, including multiple evanescent white dot syndrome (MEWDS), multifocal choroiditis and panuveitis/punctate inner choroiditis (MFCPU/PIC), acute posterior multifocal placoid pigment epitheliopathy (APMPPE), serpiginous choroiditis, and birdshot chorioretinopathy (BSCR). Nevertheless, a substantial proportion of cases deviate from classical presentations and fall into diagnostic "grey zones", blurring boundaries between diseases entities and complicating both differential diagnosis and management. This review aims to describe the broad spectrum of atypical, variant, and secondary forms of NIPUs as well as masquerade syndromes. Atypical MEWDS includes bilateral presentations or complicated courses, while MFCPU/PIC with outer retinal atrophy emerges as a notable entity with unclear therapeutic implications. Inflammatory reactions resembling both MEWDS and MFCPU/PIC may also occur as secondary phenomena, triggered by other chorioretinal disorders, most notably inherited retinal diseases (IRDs). Placoid chorioretinopathies, including APMPPE, persistent placoid maculopathy, serpiginous choroiditis, and relentless placoid chorioretinitis, are often distinguished only a posteriori based on disease course, but likely represent a continuum of disorders unified by choroidal ischemia. Atypical presentations of BSCR may feature extensive outer retinal damage, mimicking IRDs. Equally important is the consideration of masquerade syndromes in all suspected cases of NIPUs, as they can present with similar features yet require entirely different treatments. Infectious masquerades include tuberculosis-associated serpiginous-like choroiditis, acute syphilitic posterior placoid chorioretinopathy, and West Nile virus chorioretinitis, whereas vitreoretinal lymphoma is the most frequent neoplastic masquerade. In conclusion, integrating clinical context with high-quality multimodal imaging remains essential to navigate the jungle of differential diagnosis in NIPUs, while future studies should aim to link imaging phenotypes with immune and molecular biomarkers to refine classification and guide targeted therapies.
Multiple Evanescent White Dot Syndrome After mRNA COVID-19 Vaccination.
A 46-year-old woman noticed blurred vision in her right eye two weeks after receiving the fourth dose of a COVID-19 vaccine and visited a local eye clinic. Then, the patient was referred to our hospital to investigate the cause of decreased vision in the right eye. At the initial visit to our hospital, the best-corrected visual acuity (BCVA) in the right eye was 0.7 in decimal units and 1.5 in the left eye, respectively. Intraocular pressure was 17 mmHg in both eyes. Abnormal findings were observed only in the right eye; the left eye was completely normal. Therefore, all abnormal findings described below are related to the right eye. Anterior segment examination revealed inflammatory cells in the anterior chamber. The lenses in both eyes were almost clear, and the right eye showed vitreous cells and vitreous opacity. Fundus examination revealed multiple small white exudative spots. Optical coherence tomography (OCT) demonstrated disruption of the ellipsoid zone (EZ) and hyper-reflective lesions beneath the retina. Fluorescein angiography (FA) showed hyperfluorescence consistent with staining at the sites of exudative lesions, while indocyanine green angiography (ICGA) revealed corresponding hypofluorescent spots due to blocked fluorescence. Full-field electroretinography (ERG) showed reduced amplitudes of both a- and b-waves. Goldman perimetry identified a central scotoma in the right eye. Based on these findings, the patient was diagnosed with multiple evanescent white dot syndrome (MEWDS) in the right eye. Topical betamethasone sodium phosphate and fradiomycin sulfate were initiated six times a day for the right eye at the first visit. The day after the initial visit, the BCVA in the right eye rapidly declined to 0.01 in decimal units, prompting the initiation of oral prednisolone at 30 mg/day. Eight days later, a sub-Tenon's capsule injection of triamcinolone acetonide (20 mg/0.5 ml) was administered. The symptoms gradually improved thereafter, and the hyper-reflective subretinal lesions observed on OCT disappeared. Oral steroid therapy was tapered, and the visual acuity (VA) improved to 1.0 in decimal units four months later. A case of MEWDS occurring after COVID-19 vaccination showed favorable recovery with oral steroid therapy and sub-Tenon's injection.
Choroidal neovascularization secondary to punctate inner choroidopathy after resolution of multiple evanescent white dot syndrome: A case report.
White dot syndromes include multiple evanescent white dot syndrome (MEWDS) and punctate inner choroidopathy (PIC), which are rarely reported to coexist or occur sequentially in a single eye. The co-occurrence increases diagnostic complexity, and there are no previous reports of choroidal neovascularization (CNV) secondary to PIC following the resolution of MEWDS. This case report aims to supplement clinical evidence for the pathogenesis and treatment of such sequential ocular diseases. A 42-year-old healthy female presented with blurred vision in her right eye for 3 days initially; 2 months later, her right eye vision worsened further, and 2.5 months after that, she developed blurred vision again with decreased visual acuity. At the first visit, the patient was diagnosed with MEWDS in her right eye based on fundus examination, fundus autofluorescence, fundus fluorescein angiography, visual field test, and optical coherence tomography (OCT) findings. Two months later, she was diagnosed with PIC in the same eye due to new peripapillary yellow-white lesions and corresponding OCT changes. Another 2.5 months later, she was diagnosed with CNV secondary to PIC based on subretinal hemorrhage, abnormal vascular flow on OCT angiography, and fundus fluorescein angiography confirmation. No treatment was given at the initial diagnosis of MEWDS. After the diagnosis of PIC, the patient received oral prednisone (1 mg/kg per day for 5 days, followed by gradual reduction over 6-8 weeks). For CNV secondary to PIC, intravitreal anti-vascular endothelial growth factor (VEGF) therapy (Ranibizumab, 0.5 mg/0.1 mL, once a month, twice in total) was administered. After 2 intravitreal anti-VEGF injections, the best-corrected visual acuity of the patient's right eye improved to 20/20, fundic hemorrhage resolved, and OCT demonstrated complete resolution of CNV. During 2 years of follow-up, no recurrence of CNV or PIC was observed, and best-corrected visual acuity remained stable at 20/20. MEWDS and PIC may share common etiological and pathogenetic mechanisms. PIC may develop sequentially after MEWDS in the same eye, and CNV may be a complication of PIC. A combination of oral corticosteroid therapy for PIC and timely intravitreal anti-VEGF injections for secondary CNV can achieve favorable long-term clinical outcomes. Regular follow-up is necessary for patients with MEWDS to monitor for potential progression to PIC and CNV.
Relentless placoid chorioretinitis associated with Crohn's disease and secondary MEWDS: a case report.
To describe a rare case of relentless placoid chorioretinitis (RPC) associated with Crohn's disease, complicated by secondary multiple evanescent white dot syndrome (MEWDS). A 21-year-old man presented with acute, painless vision loss in the right eye. Multimodal retinal imaging showed numerous active and atrophic placoid lesions involving both the posterior pole and the retinal periphery, consistent with RPC. Systemic evaluation revealed Crohn's disease. Despite intravenous and oral corticosteroids, new extramacular lesions developed and fundus autofluorescence showed stippling compatible with secondary MEWDS. Introduction of anti-tumor necrosis factor (anti-TNF) therapy for Crohn's disease stabilized ocular inflammation. Visual prognosis remained poor in the affected eye and preserved in the fellow eye. This case highlights an association between RPC and Crohn's disease and supports an autoimmune mechanism. Early systemic evaluation and timely initiation of corticosteroid-sparing immunomodulatory therapy may help prevent recurrences and vision-threatening complications.
A case of MEWDS like retinopathy associated with peripheral exudative haemorrhagic chorioretinopathy: Un caso de síndrome de múltiples puntos blancos evanescentes (SMPBE) secundario a coriorretinopatía hemorrágica exudativa periférica.
We describe a case of the recently described MEWDS like retinopathy in a patient with active PEHCR and history of bilateral non-arteritic ischaemic optic neuropathy (NAION). This patient was an 83-year-old man with a known history of PEHCR not requiring treatment and sequential NAION. He presented to the emergency eye services complaining of left eye (LE) reduced vision. Cystoid macular oedema (EMC) was noted. There were multiple hyper-autofluorescent dots within the macula which corresponded to subretinal hyper-reflective deposits on optical coherence tomography (OCT). The patient was observed with partial resolution of deposits and complete resolution of EMC noted 4 months following his initial presentation.
