Cardiac Myxoma in the right ventricle.
Right ventricular myxomas is rare cardiac tumor. We described a 35-year-old man who had a right ventricular myxomas. The mass was completely removed under cardiopulmonary bypass. This case illustrates the importance of multimodality imaging when establishing the primary diagnosis and determining the course of treatment.
Cardiac Fibroblastoma Presenting as NSTEMI.
Myocardial infarction with nonobstructive coronary arteries (MINOCA) represents ∼8% of acute myocardial infarctions, with causes including myocarditis, coronary spasm, and embolization from cardiac tumors. A 59-year-old woman presented with exertional chest pain and elevated troponins, consistent with non-ST-segment elevation myocardial infarction. Coronary angiography showed normal coronaries. Cardiac magnetic resonance imaging revealed a small ischemic infarct, prompting review of prior computed tomography, which identified a small aortic valve mass. Transesophageal echocardiography confirmed a mobile lesion. Surgical excision revealed a benign fibroblastoma, an extremely rare cardiac tumor. This case highlights the importance of thorough imaging review in MINOCA and expands the spectrum of cardiac tumors implicated in coronary embolization. This is to our knowledge one of the first reports of a cardiac fibroblastoma.
Cardiac Angiosarcoma as an Unexpected Cause of Palpitations: Case Report, Differential Diagnosis, and Current Therapeutic Approach to Cardiac Tumors.
We present a case of an 83-year-old woman who presented with palpitations, asthenia, and exertional dyspnea. The electrocardiogram showed atrial flutter, and echocardiography (UCG) revealed a large mass occupying the right atrial chamber. Contrast-enhanced computed tomography (CECT) and cardiac magnetic resonance (CMR) confirmed its origin and extension into the superior vena cava (SVC). A multidisciplinary medical committee considered palliative surgery as the best therapeutic option for the patient due to the impossibility of a complete surgical resection of the tumor. Primary malignant neoplasms of the heart are extremely rare. Their clinical behavior is aggressive, with high morbidity and mortality and frequent surgical limitations due to location and extent at diagnosis. Imaging techniques are essential for diagnosis such as UCG for initial detection, CMR for tissue characterization, and CT or positron emission tomography-computed tomography (PET-CT) for staging and therapeutic planning. Definitive diagnosis requires histologic study, although it is not free of significant complications. This is a case report of a rare cardiac tumor followed by review of literature on its diagnosis and therapeutic approach.
Left Atrial Epithelioid Angiosarcoma Presenting As Recurrent Stroke: A Case Report.
A 74-year-old man presented with recurrent cerebellar infarction and progressive anemia. The workup revealed a small jejunal tumor and a mobile mass in the left atrium. Urgent minimally invasive cardiac surgery was performed, and histopathology confirmed epithelioid angiosarcoma. Subsequent small bowel resection revealed a similar tumor. Despite chemotherapy and radiotherapy, bone metastases progressed, and the patient succumbed to the disease eight months postoperatively. The cardiac tumor was smaller than typically reported, suggesting early detection prompted by embolic events. Nevertheless, the clinical course was aggressive, highlighting the high malignancy of epithelioid angiosarcoma and the limited effectiveness of current therapies. Accumulation of further clinical experience is essential to improve outcomes in this rare cardiac tumor.
Papillary Fibroelastoma as a Cause of Cardioembolic Stroke in Young Adults: A Case Report.
Although ischaemic strokes are common worldwide, strokes in young adults are rare. Cardioembolism is a frequent cause of ischaemic strokes, often arising from atrial fibrillation or, less commonly, underlying structural heart defects. Primary cardiac tumours are commonly associated with embolic strokes. Although papillary fibroelastoma (PFE) is a rare cardiac tumor, it is one of the most common primary cardiac tumours that causes embolic strokes. We report the case of a 24-year-old woman who presented with sudden-onset right arm weakness. Subsequent investigations, including echocardiography, revealed a papillary mass consistent with PFE.
