An exploratory study on the placental efficiency, histomorphometric characteristics, hormonal shifts, and oxidative stress in dromedary camels with dystocia.
The study investigated placental efficiency, histomorphometry, and hormonal concentrations (progesterone, estradiol 17β, and cortisol) alongside oxidative stress indicators (malondialdehyde (MDA) and total antioxidant capacity (TAC)) and performed correlation analyses among these parameters in eutocia, dystocia, and uterine inertia deliveries in dromedary camels. A total of 24 pregnant dromedary camels, aged 8-11 years, with an average body weight of 520 ± 75 kg, were categorized into three groups based on parturition outcome: the eutocia group (EG), n = 12; the dystocia group (DG), n = 5; and the uterine inertia group (UIG). n = 7. The results revealed significantly lower (p < 0.05) syncytiotrophoblast layer thickness, chorionic villi length and diameter, and syncytiotrophoblast layer integrity in dystocia deliveries. Additionally, the inflammatory cell infiltration was higher (p < 0.01) in DG than in UIG and EG. The placentas from EG had long, slender, and highly vascularized chorionic villi, while in DG placentas, the villi were shorter and atrophied. Progesterone concentrations were significantly higher (p < 0.01) in DG compared to UIG and EG. While DG had the lowest mean estradiol-17β concentration, EG had the highest. Moreover, MDA and TAC concentrations were lowest in the EG group, followed by the UIG group and then the DG group. Additionally, a positive correlation was found between chorionic villi diameter and placental efficiency, and between placental efficiency and vascular density. There were negative correlations among hormonal, antioxidant, placental efficiency, and histomorphometry parameters, as well as among these parameters and inflammatory cell infiltration. In conclusion, dystocia in camels was associated with hormonal dysregulation, presumably oxidative stress, and placental structural damage. However, for a definitive and powerful outline, further studies on large and more balanced populations are necessary in the future.
Relationship Between VSELs, MUSE Cells and MSCs in Health and Disease.
A comprehensive understanding of how diverse adult stem cell populations function in harmony is crucial for maintaining homeostasis and ensuring the normal functioning of body tissues. Two types of stem cells in adult tissues have attracted attention, including very small embryonic-like stem cells (VSELs) and multi-lineage differentiating stress-enduring cells (MUSE), reported for the first time in 2006 and 2010, respectively. VSELs are pluripotent stem cells developmentally linked to the primordial germ cells, while MUSE cells, initially described as multipotent, are now being defined as having pluripotent characteristics and further differentiate into MSCs. VSELs are the most primitive, virtually immortal and pluripotent stem cells that survive lifelong in all tissues in small numbers and undergo asymmetrical divisions to give rise to tissue-specific progenitors of different sizes and fates. VSELs are 5-7 μm in size, spherical in shape, with a cell surface profile of LIN-CD133 + CD45- while MUSE cells are 10-15 μm in size, with abundant cytoplasm, horseshoe/bean-shaped nuclei, cytoplasmic OCT-4 and are CD45+, like hematopoietic stem cells. In the mouse uterus, VSELs undergo cyclic changes in response to circulatory hormones, regenerate both the epithelial and stromal compartments in an atrophied uterus (upon bilateral ovariectomy, in the absence of macrophages) and also upon chronic injury. Exposure to endocrine-disrupting chemicals disrupts the functions of VSELs and results in various pathologies, including endometrial cancer. The crucial role of dysfunctional VSELs resulting in cancer initiation, progression, metastasis and recurrence was recently discussed. On the other hand, multiple clinical trials have reported the potential of MUSE cells for ensuring regeneration upon transplantation. VSELs regenerate damaged and diseased tissues when a healthy paracrine support is provided by the transplanted MUSE cells/MSCs; however, remain elusive due to their small size and scarce nature. In summary, the view that MUSE cells phagocytose damaged cells and subsequently differentiate into the same cell type is fundamentally challenged and requires careful re-evaluation.
Predictors of Procedural Pain in Office Hysteroscopy.
BACKGROUND Various strategies have been explored to mitigate office hysteroscopy, including oral or local analgesia, modified cervical dilation techniques, and patient positioning, but there is no consensus on the most effective approach for office hysteroscopy performed for diagnostic purposes. Thus, we aimed to identify independent clinical and other factors associated with increased pain during office hysteroscopy to inform targeted pain reduction strategies. MATERIAL AND METHODS This prospective observational study included 102 women aged 18-70 years who underwent office hysteroscopy at a tertiary center between August and October 2020. Pain intensity was measured using a 10-cm visual analog scale (VAS). Vital signs were monitored before, during, and after the procedure. RESULTS The mean age of participants was 41.62±7.94 years. The mean VAS pain score during hysteroscopy was 4.91±2.97. Patients without a history of vaginal delivery reported significantly higher VAS scores than those with prior vaginal births (P<0.001). Retroverted uterus was associated with greater pain than anteverted or normally positioned uteri (P<0.001). Procedure duration showed a moderate positive correlation with pain (P<0.001), while body mass index (BMI) had a weak negative correlation (P=0.041), and the number of cesarean deliveries had a weak positive correlation with pain intensity (P=0.047). In multivariable analysis, retroverted uterus (P=0.007), longer procedure time (P=0.004), and absence of vaginal delivery (P=0.001) were independently associated with higher pain scores. CONCLUSIONS Retroverted uterine position, nulliparity, and longer procedure duration are independent predictors of increased pain during office hysteroscopy. Awareness of these risk factors allows clinicians to tailor their approach through enhanced pre-procedural counseling, optimized analgesia, and modifications in procedural technique to reduce patient discomfort.
Effects of L-citrulline supplementation in the basal diet on reproductive performance, serum metabolites, and microbial community structure in Simmental cows.
The experiment was designed to evaluate the influence of dietary supplementation with L-Citrulline (L-Cit) on reproductive outcomes, serum reproductive hormone profiles, ruminal and intestinal microbial communities, serum metabolites, and their metabolic associations in Simmental cows, thereby providing a theoretical basis for its application in ruminant production. All cows were screened by vaginal mucus microscopy (no pathogenic bacteria detected) and B-ultrasound examination (no ovarian cysts or uterine inflammation observed) to confirm the absence of reproductive disorders. A total of 240 multiparous Simmental cows, 3-4 years of age with an average body weight of 470 ± 15 kg, were randomly allocated into three groups of 80 animals each: a control group, Group I, and Group II. The control group received only the basal diet, whereas Group I and Group II were provided with the basal diet supplemented with 7 g/d and 14 g/d of L-Cit per cow, respectively. Day 0 of the trial was defined by the first intramuscular injection of prostaglandin (PG, 0.4 mg per cow), after which L-Cit supplementation commenced at 72-h intervals. On day 10, a second PG injection (0.4 mg per cow) was administered, and supplementation was discontinued on day 11. Estrus expression was monitored, and pregnancy was assessed on day 35 post-insemination using B-mode ultrasonography. During the experimental period, six cows were randomly selected from each group. Blood samples were collected from the coccygeal vein prior to the morning feeding on days 1 and 11 using plain tubes (without anticoagulant). The samples were centrifuged at 720 × g (approximately 3,000 rpm for 15 min) to separate the serum. The obtained serum was stored at -20 °C for subsequent analysis of reproductive hormones. In addition, blood, rumen fluid, and fecal samples were obtained 10 h after mounting to support untargeted metabolomic profiling and microbial community analysis. Compared with the control group, the estrus rate in experimental Group I increased by 12.5% (p < 0.05). In experimental Group II, serum concentrations of gonadotropin-releasing hormone (GnRH) and follicle-stimulating hormone (FSH) measured 10 h after cows accepted mounting rose by 15.79% (p < 0.05) and 35.71% (p < 0.01), respectively, relative to the control group. Analysis of 16S rRNA sequences revealed significant differences in ruminal microbial taxa, including Verrucomicrobiota, Lentisphaeria, Oligosphaeraceae, vadinBE97_g_norank, vadinBE97, Eubacterium_ruminantium_group, and Prevotellaceae_g_norank. In the intestine, significant differences were observed in Lachnospiraceae, Lachnospirales, Marvinbryantia, Desulfovibrionia, Desulfovibrionaceae, and Desulfobacterota. KEGG enrichment analysis based on Liquid Chromatography-Mass Spectrometry (LC-MS) data indicated upregulation of the arginine biosynthesis pathway in the experimental groups, whereas metabolites associated with the tricarboxylic acid (TCA) cycle were notably elevated in experimental Group I. Within the experimental framework, dietary supplementation with L-Cit combined with the two-shot PG synchronization protocol reshaped the ruminal microbial community structure of Simmental cows, improved glucose metabolic efficiency and xenobiotic degradation capacity, and promoted the synthesis and release of reproductive hormones. Consequently, estrus and conception rates were enhanced.
A rare case of Mayer-Rokitansky-Küster-Hauser syndrome presenting with primary amenorrhea and chronic headaches: a case report.
Primary amenorrhea is defined as the absence of menarche by age 13 without secondary sexual characteristics or by age 15 with them. Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is the second most common cause of primary amenorrhea after ovarian failure. This case highlights a 15-year-old female with MRKH syndrome type I presenting with chronic headaches. A 15-year-old female presented with primary amenorrhea and chronic frontal headaches. She reported non-foul-smelling white per vaginal discharge but denied visual disturbances, limb weakness, or systemic symptoms. Examination revealed normal secondary sexual characteristics, a blind vaginal pouch (0.5 cm), and a low body mass index of 17. Pelvic ultrasonography and MRI confirmed uterine agenesis with normal ovaries and no renal anomalies, consistent with MRKH syndrome. Brain MRI and hormonal profiles were normal, suggesting her headaches were likely nutritional and dehydration related, which improved with dietary optimization. A multidisciplinary team performed McIndoe vaginoplasty using a split-thickness skin graft, successfully creating a 9-cm neovagina. On follow-up, the patient demonstrated good neovaginal patency, maintained regular self-dilatation, and reported improved psychological well-being. MRKH syndrome is a congenital disorder characterized by the absence or underdevelopment of the uterus and upper vagina despite normal external genitalia and a 46,XX karyotype. It affects approximately 1 in 5000 females. Diagnosis is confirmed through imaging, with MRI being the most reliable modality. Although headaches are not commonly associated with MRKH, in this patient, they were likely due to undernutrition, dehydration, and psychological stress. Management of MRKH includes both nonsurgical (vaginal dilation) and surgical approaches. The McIndoe procedure, involving a skin graft to create a neovagina, has shown high success rates in improving sexual function and psychological well-being. Postoperative outcomes highlight the importance of a multidisciplinary approach. This case highlights a rare presentation of MRKH syndrome with primary amenorrhea and headache in a young adolescent. Although headaches may mislead diagnosis, imaging is vital for confirmation and exclusion of other causes. The patient improved following McIndoe vaginoplasty, emphasizing timely diagnosis, multidisciplinary care, and psychological support for optimal outcomes.
