Implications of Dermatologic Disorders in Facial Cosmetic Surgery: A Systematic Review.
Invasive and minimally invasive cosmetic procedures in patients with autoimmune or inflammatory skin disorders can carry an increased risk of surgical site infection, disease exacerbation, atrophic/hypertrophic scarring, or suboptimal cosmetic results. As facial procedures continue to rise in popularity, increasing complications are being reported in association with underlying dermatologic disease. In this review, we aim to bring attention to these dermatologic conditions and the potential challenges that they pose to the facial plastic surgeon. A comprehensive literature search was conducted across PubMed, Embase, and MEDLINE for articles published before January 2025 describing one or more patients with complications following facial or scalp procedures relating to a pre-existing or underlying dermatologic disorder or systemic condition with dermatologic manifestations. Articles were screened according to PRISMA guidelines, and data extraction included dermatologic disease diagnosis and severity, facial procedure performed, perioperative or postoperative complications, and resolution/treatment. Our search criteria identified 2452 articles; 53 articles met the inclusion criteria. In a total of 469 patients with pre-existing dermatologic conditions, mild-severe complications were reported after blepharoplasty (41/469, 8.7%), rhinoplasty (254/469, 54.2%), rhytidectomy (25/469, 5.3%), hair transplantation (37/469, 7.9%), cosmetic injections (35/469, 7.5%), lasers (15/469, 3.2%), and microneedling (50/469, 10.7%). Dermatologic disorders included acne, atopic dermatitis, frontal fibrosing alopecia, lichen planopilaris, rosacea, and vitiligo; systemic conditions with dermatologic manifestations included Ehlers-Danlos syndrome, sarcoidosis, scleroderma, and systemic or discoid lupus erythematosus. Complications ranged from mild postoperative disease exacerbation to severe hypertrophic scarring, ulcerations, and wound healing defects. The existing literature, while limited to case reports and small case series, demonstrates potential risks following facial procedures in patients with dermatologic disorders. This review highlights the importance of consideration of dermatologic disease in facial surgery planning and decision-making and summarizes effective strategies to minimize any potential risk.
Frontal fibrosing alopecia and lichen planopilaris: epidemiology, treatment, and remission rate (2014-2024).
Frontal fibrosing alopecia (FFA) and lichen planopilaris (LPP) are cicatricial alopecia with overlapping but distinct epidemiological, clinical, and therapeutic features. This study evaluates their epidemiological and clinical characteristics, treatment approaches, and one-year remission rates. This retrospective cohort study was conducted at Holy Family Hospital, Karachi, Pakistan. Data from electronic records included demographics, clinical presentations, histology, treatments, outcomes, and follow-up. Patients of all ages and genders diagnosed with LPP or FFA between January 2014 and December 2024, with a minimum one-year follow-up, were included. LPP patients had a higher mean age at onset (53.1±9.42 years) than FFA patients (45.8±10.4 years; p=0.02). Scalp involvement was more common in LPP (95.9%), while body involvement was higher in FFA (85.7%; p<0.001). FFA patients had more symptoms, whereas LPP was often asymptomatic (p<0.001). Remission rates were similar at 12 months, but FFA showed higher long-term remission (59.0% vs. 39.0%; p=0.004). FFA patients sustained remission (55.7% vs. 23.4%; p<0.001), while LPP had more partial or no responses. This study highlighted key differences in demographics, clinical features, and treatments between FFA and LPP, with similarities in diagnostic delays and treatment approaches. Variations were noted in the age of onset, symptoms, clinical presentation, and outcomes.
Revisiting Contact Sensitization Among Patients With LPP/FFA: Cross-Sectional Findings From 55 Cases.
Lichen planopilaris (LPP) is an inflammatory disease results in scarring alopecia. Frontal fibrosing alopecia (FFA) is considered a variant due to similar histological features. A possible link has been suggested between exposure to various products and disease development, although research findings have been inconsistent. To assess the prevalence of delayed allergic reactions to commonly used topical agents in patients with LPP/FFA. A real-world observational study compared patch test results between LPP/FFA patients and a matched control group from routine clinical practice (2018-2023). Patch testing was conducted using the: European Baseline, Cosmetics, Hairdressing, and Sunscreen. The control group included patients without LPP/FFA underwent the same test panels. A total of 23 patients with FFA, 32 patients with LPP, and 171 controls were evaluated. Positive reactions in the European Baseline Series occurred in 26% of FFA patients, compared to 37% in the LPP patients and 56% in the control group. Common allergens included nickel and fragrance mix I, but their prevalence did not exceed rates in the general Israeli population. LPP/FFA patients had fewer positive reactions to cosmetics, hair products, and sunscreens. No clinically relevant reactions were demonstrated in the study. Patch test reactivity rates in LPP/FFA patients did not exceed those observed in the control group.
Different Patterns of Lichen Planus in Three Members of One Family.
The pathogenesis of Lichen Planus (LP) and its variants, despite many investigative efforts, remains incompletely understood. The occurrence of the disease in siblings suggests a potential role for both genetic and shared environmental factors. Here, three Iranian siblings-one male and two females-who presented with various clinical forms of LP were introduced. Case No. 1 exhibited bilateral facial pigmentation and was diagnosed with Lichen Planus Pigmentosus (LPPigm). Case No. 2 presented with facial pigmentation accompanied by facial papules and was diagnosed with LPPigm and Lichen Planopilaris (LPP). Case No.3 had frontotemporal hairline recession and eyebrow sparsening and was diagnosed with Frontal Fibrosing Alopecia (FFA). Histopathological examination confirmed the diagnoses in all three patients. Patients were treated with an individualized plan that included sunscreen use, potent topical corticosteroids, topical pimecrolimus, minoxidil, and systemic finasteride. To our knowledge, no other family in the literature has been reported to have such a wide range of LP variants. This series underscores the need for further research into genetic and environmental factors contributing to the development of LP and its variants.
In vivo differentiation of scarring alopecia using line-field confocal optical coherence tomography: a pilot study.
Diagnosing cicatricial alopecia is challenging. Even with histology, a definitive diagnosis cannot always be made. Line-field confocal optical coherence tomography (LC-OCT), a non-invasive imaging method with high resolution used in vivo, has proven helpful in diagnosing scarring alopecia. This study aims to evaluate the potential of LC-OCT for the differential diagnosis of different types of scarring alopecia. Images of 30 patients with scarring alopecia (10 Lichen planopilaris/Frontal fibrosing alopecia (LPP/FFA), 10 lupus erythematosus, 10 folliculitis decalvans/Folliculitis et perifolliculitis capitis suffodiens et abscedens (FD/FSA)) and 10 healthy controls were analyzed by two blinded independent examiners. Characteristic features and differential diagnoses were determined for each case. LC-OCT revealed distinct characteristics for each of the four subgroups, facilitating differential diagnosis with 100% sensitivity for lupus and 90% for LPP/FFA, FD/FSA, and the control group, and 100% specificity for LPP/FFA and 96.7% for the other groups. The features were used to generate a diagnostic algorithm tree. With LC-OCT, specific criteria can be used to differentiate between subtypes of scarring alopecia. These preliminary findings provide a basis for hypothesis generation, which should be further explored in larger cohorts and in longitudinal studies assessing treatment response over time.
