NLRP3 Inflammasome Activation-Induced Acute Papillitis as a Trigger of Acute Pancreatitis - A Novel Mechanism of Microlithiasis-Induced Acute Pancreatitis.

Obstruction of the pancreatic duct by impacted gallstones at the level of the papilla vateri causes acute pancreatitis. How non-obstructing stones such as microlithiasis or sludge cause pancreatitis has not been studied. We aimed to understand the pathomechanism of microlithiasis-induced acute pancreatitis. In human papillary biopsies from patients with microlithiasis-induced acute pancreatitis (n = 4), alcohol-induced acute pancreatitis (n = 5), and control subjects without pancreatobiliary disease (n = 4), the inflammatory infiltrate was quantified. Bone marrow-derived macrophages generated from C57BL/6 mice were treated in vitro with cholesterol monohydrate and calcium bilirubinate crystals, and NLRP3 inflammasome-mediated macrophage activation was quantified. Microlithiasis formation in the gallbladder was induced in mice through lithogenic high fat diet and devazepide. Acute pancreatitis was induced by supramaximal caerulein stimulation. Microlithiasis ejection from the gallbladder was achieved through low-dose caerulein i.p. Injections. Pancreatitis severity was compared between caerulein-induced pancreatitis and caerulein-induced pancreatitis after repetitive microlithiasis ejection. Significantly higher infiltration of CD45-positive leukocytes and increased NLRP3 expression were observed in papillary biopsies from patients with microlithiasis-induced acute pancreatitis compared with patients with alcohol-induced acute pancreatitis and control subjects. In line with this, significantly higher IL-1ß secretion and caspase-1 activation were observed in vitro in bone marrow-derived macrophages stimulated with cholesterol monohydrate and calcium billirubinate crystals. In vivo microlithiasis formation was achieved in all mice with high fat diet and devazepide. Compared to caerulein-induced pancreatitis, in caerulein + microlithiasis pancreatitis, higher LDH, GPT and ALP levels in serum were observed, but without an impact on pancreatitis severity. However, mice papilla mimicked the phenotype of microlithiasis-induced acute pancreatitis in humans. We propose a novel mechanism in which biliary microlithiasis induces a local inflammatory reaction at the papilla (acute papillitis) via NLRP3 inflammasome activation driven by bone marrow-derived macrophages, without causing pancreatic outflow obstruction.

Comparative Evaluation of HMG Family Proteins and miR-106a-5p in Low-Grade Non-Invasive and High-Grade Muscle-Invasive Papillary Urothelial Carcinoma.

Urothelial carcinoma (UC) of the bladder exhibits low- and high-grade papillary forms with distinct prognoses. High mobility group proteins (HMGA1, HMGA2, HMGB1) and miR-106a-5p are involved in tumor progression, but their interplay in UC remains incompletely understood. The aim of this study was to compare the expression of these parameters in low- and high-grade papillary UC. Tissue samples from 80 patients (40 low-grade and 40 high-grade) undergoing transurethral resection or cystectomy were analyzed, with control samples consisting of tumor-adjacent tissues without histopathological alterations obtained from the same patients. HMGA1, HMGA2, and HMGB1 protein expression was assessed immunohistochemically. Gene expression was quantified by real-time PCR, and miR-106a-5p levels were measured by droplet digital PCR. Statistical analysis was conducted using Statistica 13.3, applying one-way ANOVA with Tukey's post hoc test and correlation analysis, with p < 0.05 considered significant. Expression of HMGA1 and HMGB1 was reduced in low-grade papillary urothelial carcinoma compared to control tissues, whereas both proteins were significantly increased in high-grade lesions. HMGA2 expression was minimal in low-grade tumors but partially restored in high-grade tumors. Analysis revealed the highest levels of miR-106a-5p in normal urothelium, slightly decreased in low-grade tumors, and significantly reduced in high-grade cancers. HMG proteins and miR-106a-5p demonstrate distinct expression patterns in low- versus high-grade papillary UC, which correlates with tumor aggressiveness. These molecules may serve as diagnostic and prognostic biomarkers. Their potential as therapeutic targets requires further mechanistic and translational investigation.

Blindness after middle meningeal artery embolization for skull metastasis of follicular thyroid carcinoma with ophthalmic artery variant.

Preoperative embolization for intracranial tumors is widely used to minimize intraoperative blood loss; however, it carries the risk of ischemic or cranial nerve complications, particularly in patients with anatomical variants. A 47-year-old woman with a history of follicular thyroid carcinoma (FTC) was referred after an intracranial mass was detected on magnetic resonance imaging. The lesion was initially suspected to be a meningioma. However, contrast-enhanced computed tomography showed that the tumor was located lateral to the middle meningeal artery (MMA), suggesting an epidural lesion. Angiography confirmed a strong tumor blush from the MMA, and preoperative embolization was performed. During angiography, a branch of the MMA extending toward the orbit was identified; this branch was coil-occluded before Embosphere injection to prevent inadvertent embolization. The patient developed ipsilateral blindness immediately after the procedure. Subsequent internal carotid angiography revealed an anatomical variant in which the ophthalmic artery (OphA) did not originate from the internal carotid artery (ICA). In this context, retinal ischemia was attributed primarily to coil occlusion of the MMA branch supplying the retinal circulation, with possible exacerbation from subsequent Embosphere injection. The tumor was completely resected through craniotomy, and histopathology confirmed metastasis of FTC. At 5 years postoperatively, there was no evidence of intracranial recurrence, although the visual loss was permanent. Skull metastasis of FTC is an uncommon clinical entity. This case highlights a serious complication of preoperative embolization due to a vascular variant in which the OphA was absent from the ICA and the central retinal artery originated from the MMA, leading to irreversible blindness. Thorough angiographic evaluation is essential to identify anatomical variants before embolization. When high-risk anatomy is present, alternative strategies should be considered to minimize the risk of ischemic complications.

TrpC5 silencing reduces cell invasion and migration and enhances radiosensitivity in papillary thyroid carcinoma cells.

Carcinoma (PTC) is the most common malignant tumor derived from thyroid follicular cells and represents the most common pathological type of thyroid malignancy. Cancer metastasis and radiosensitivity are important factors that limit the treatment of PTC. This study aimed to investigate the regulatory effects of transient receptor potential channel C5 (TrpC5) on the proliferation, invasion, migration, and radiosensitivity of PTC. Human papillary thyroid carcinoma cell lines TPC-1 and B-CPAP were transfected with TrpC5 siRNA, pcDNA-TrpC5, or their corresponding negative control. PTC cells were stimulated by radiotherapy. The results showed that TrpC5 silencing weakened the proliferation, invasion, and migration of PTC cells, whereas TrpC5 overexpression promoted these cellular behaviors. Moreover, TrpC5 expression was progressively upregulated in PTC cells following exposure to irradiation (IR). TrpC5 silencing enhanced radiosensitivity of TPC-1 and B-CPAP cells. In addition, TrpC5 silencing enhanced the expression of DNA damage-related proteins p-ATM, p-CHK, and γH2AX in PTC cells under IR treatment. Overall, TrpC5 silencing weakened cell invasion and migration and enhanced the radiosensitivity of PTC cells. These findings suggest that TrpC5 may serve as a potential therapeutic target for PTC and warrant further investigation in vivo.

International Multidisciplinary Consensus Report on Definitions, Diagnostic Criteria, and Management of Fatty Pancreas: A Joint Statement Endorsed by EPC, APA, EASD, EASL, ESGAR, ESGE, ESP, ESPCG, ESPEN, ESPGHAN, IAP, JPS, KPBA, LAPSG, and UEG.

This international, multidisciplinary consensus report represents the first effort to systematically define and characterize fatty pancreas. A key outcome of this endeavor was the recommendation to adopt "fatty pancreas" as the standardized and inclusive term to describe all forms of fat accumulation in the pancreas. This terminological consensus provides a critical foundation for unified reporting and clinical communication. Another major contribution of the report is the consensus on diagnostic imaging findings, which was based on radiological and endoscopic modalities. The proposed criteria aim to enhance consistency in clinical assessment and support the development of standardized research protocols. In addition to establishing terminology and diagnostic frameworks, the report also synthesizes current knowledge across a wide range of relevant domains. These include the etiology and epidemiology of fatty pancreas, as well as its associations with alcohol consumption, smoking, acute and chronic pancreatitis, pancreatic exocrine insufficiency, type 2 diabetes mellitus, and surgical outcomes. The potential links between fatty pancreas and neoplastic conditions such as intraductal papillary mucinous neoplasms and pancreatic cancer are also addressed, alongside the current understanding of its metabolic implications (beta-cell function and glucose homeostasis) and treatment strategies. Throughout the consensus process, a consistent theme emerged: the limited availability of high-quality, prospective clinical data. Therefore, many of the recommendations in this report are based on expert consensus rather than strong empirical evidence. As such, the statements require rigorous prospective validation before they can be adopted into routine clinical practice. This underscores a critical need for further research, particularly studies aimed at clarifying causal relationships, validating diagnostic tools, and determining the clinical relevance of fatty pancreas across diverse patient populations. This report serves as both a summary of our current understanding and a roadmap for future investigations, aiming to close existing knowledge gaps and guide evidence-based clinical practice in this emerging field.
