Use of Gore Thoracic Branch Endoprosthesis in the Management of Dysphagia Lusoria.
Dysphagia lusoria is a rare clinical condition resulting from the compression of the esophagus by an aberrant retroesophageal right subclavian artery, often associated with a Kommerell diverticulum. This report highlights 3 cases in which the Gore Thoracic Branch Endoprosthesis (TBE) was used in a single-stage hybrid procedure to successfully manage this condition. Three patients presented with symptoms ranging from progressive dysphagia to symptomatic airway obstruction. They underwent single-stage thoracic endovascular aortic repair with TBE to revascularize the left subclavian artery and exclude the proximal right subclavian artery, followed by right subclavian artery debranching with transposition to right common carotid artery. All 3 patients achieved complete resolution of symptoms postoperatively with technical success on follow-up imaging. This report underscores the efficacy of the Gore TBE as a solution for addressing this complex vascular anomaly.Clinical ImpactAberrant arch configurations that need repair usually require complex extra-antomic debranching of head vessels and or adjunct endovascular maneuvers during TEVAR. Development of branched TEVAR devices have improved surgical approaches in complex aortic arch treatment. The technique described here utilizes the benefits of an off-the-shelf branched TEVAR device, the Gore TBE, in a single stage hybrid procedure to address an aortic pathology that often require multi-stage, multi-device treatment.
Aberrant right subclavian artery (ARSA) presenting with esophageal pseudo-neoplastic symptoms, dyspepsia, and hemodynamic findings.
Aberrant right subclavian artery (ARSA) is the most common congenital anomaly of the aortic arch. Though often asymptomatic, it may cause esophageal compression and mimic neoplastic lesions. Accurate diagnosis via CT angiography is essential, especially before thoracic or cervical surgery. A 68-year-old woman presented with dyspepsia, dyspnea, and epigastric pain. Endoscopy revealed a polypoid lesion suggestive of malignancy. CT imaging showed ARSA passing posterior to the esophagus, with no mass detected. Hemodynamic evaluation revealed inter-arm blood pressure discrepancy and bradycardia. ARSA can lead to dysphagia lusoria and other compressive symptoms. Its association with Kommerell's diverticulum and non-recurrent laryngeal nerve has surgical implications. Treatment options include open, endovascular, and hybrid approaches. ARSA should be considered in patients with atypical esophageal symptoms. Imaging plays a key role in diagnosis and guiding appropriate, individualized treatment.
From stridor to symptom relief; management of pediatric Kommerell's diverticulum and vascular rings in Palestine [2016-2024]: a single center cohort study.
Kommerell's diverticulum (KD) with vascular rings is an uncommon cause of pediatric airway and feeding symptoms, and data from the Arab region are scarce. We report a Palestinian cohort of all diagnosed and managed cases, describing presentation, diagnostics, and management. We retrospectively studied all children at Al-Makassed Hospital (Jerusalem) from 2016 to 2024 who were diagnosed with vascular rings + KD confirmed by imaging or intra-operative findings. We abstracted demographics, symptoms, test performance, anatomy, treatments, and peri-operative course and follow-up. Primary outcomes were diagnostic yield and improvement at last visit. Fourteen patients were included; median age at diagnosis was 8 months (IQR 2-15), symptom onset 15 days (0-83). Thirteen (92.9%) were symptomatic; stridor (64.3%), recurrent wheeze (35.7%), and dysphagia/choking (28.6%) predominated; 42.9% had prior misdiagnoses. Barium swallow was positive in 7/8 (87.5%), echocardiography in 5/9 (55.6%), bronchoscopy in 10/12 (83%), and CT angiography (CTA) confirmed anatomy in 13/13 (100%). Anatomy included right aortic arch (RAA) with aberrant left subclavian artery in 8/14 (57.1%), double aortic arch in 4/14 (28.6%), and innominate artery compression in 2/14 (14.3%); 57.1% had minor intracardiac anomalies. Thirteen children (92.9%) underwent surgery (ligamentum division, diverticulectomy ± subclavian transfer, aortopexy, or double-arch division); one mild innominate compression case was observed. All were discharged alive; one late death occurred. At last follow-up, 7/13 survivors were asymptomatic and 6/13 had residual but improved symptoms. Median hospital stay was 18 days (IQR 7-25). KD-associated rings present early yet are frequently delayed in diagnosis. CTA is definitive, and tailored surgery yields meaningful, often complete, symptom relief with acceptable morbidity.
Robotic resection of aberrant right subclavian artery for dysphagia lusoria: a case report.
Aberrant right subclavian artery (ARSCA) is a congenital anomaly of the aortic arch where the aberrant artery arises from the descending aorta and traverses the mediastinum, compressing the esophagus in the process. We present a case of a 16-year-old boy who presented with worsening dysphagia of 1 year duration. He was diagnosed with ARSCA causing mass effect on the esophagus. He underwent a simultaneous right common carotid (RCC) to right subclavian artery (RSCA) bypass with an 8-mm polytetrafluoroethylene (PTFE) ringed graft and robot (Da Vinci system)-assisted division of the aberrant artery. He had an uneventful postoperative period, without any complications, and was discharged after 4 days with complete resolution of his symptoms. This case report highlights that the robotic approach is safe and effective, with benefits of minimally invasive surgery like better precision, faster recovery, and return to daily activity post-surgery.
Robot-assisted thoracoscopic surgery-assisted aberrant right subclavian artery transposition for dysphagia lusoria.
Dysphagia lusoria is a rare esophageal compression syndrome related to aberrant right subclavian anatomy. Traditional surgical management often requires thoracotomy or sternotomy, whereas more contemporary approaches involve a hybrid approach with thoracic endovascular aortic repair and subclavian bypass or transposition. We present the case of a 44-year-old woman with hostile endovascular anatomy and progressive dysphagia to solids and liquids who underwent robot-assisted thoracoscopic surgery ligation of the proximal aberrant right subclavian artery with retro-esophageal pull-through and transposition to the right common carotid artery.
