A heartbeat in the neck: cervical aortic arch with Kommerell diverticulum in an adolescent.
We present a 14-year-old female with a history of ventricular septal defect repair who presented with a striking complaint of a heartbeat sensation in the neck and was subsequently diagnosed with a rare cervical aortic arch, classified as Zhong's B2 and Haughton's B type, associated with a Kommerell diverticulum. Comprehensive cross-sectional imaging and conventional angiography excluded true and pseudo-coarctation, highlighting the importance of meticulous anatomical assessment in directing treatment and preventing unnecessary interventions in asymptomatic or non-obstructive cases.
CTA of the adult native aorta: Everything you wanted to know (but were never told).
Computed tomography angiography (CTA) is the reference standard for comprehensive evaluation of the native aorta, providing simultaneous assessment of luminal, mural, and perivascular structures within a single rapid acquisition. Accurate diagnosis and appropriate management increasingly depend on standardized terminology, consistent anatomic segmentation, and reproducible measurement techniques. This review presents a practical framework for systematic interpretation of adult aortic CTA. Key aspects include detailed anatomic considerations, standardized measurement principles, contemporary acquisition protocols, and structured reporting strategies. The imaging spectrum of the main aortic disease categories-degenerative, congenital, and inflammatory-is reviewed, with emphasis on atherosclerosis, acute aortic syndromes, aneurysmal disease, coarctation, Kommerell diverticulum, and large-vessel vasculitis. Particular attention is given to morphologic patterns, prognostic imaging features, and common diagnostic pitfalls, as well as to terminology that more accurately reflects underlying pathophysiology. By integrating technical recommendations with disease-specific imaging criteria, this review aims to improve diagnostic precision, facilitate longitudinal follow-up, and enhance multidisciplinary communication. A structured approach to aortic CTA interpretation reinforces the central role of radiology in clinical decision-making and risk stratification in patients with aortic disease.
Use of Gore Thoracic Branch Endoprosthesis in the Management of Dysphagia Lusoria.
Dysphagia lusoria is a rare clinical condition resulting from the compression of the esophagus by an aberrant retroesophageal right subclavian artery, often associated with a Kommerell diverticulum. This report highlights 3 cases in which the Gore Thoracic Branch Endoprosthesis (TBE) was used in a single-stage hybrid procedure to successfully manage this condition. Three patients presented with symptoms ranging from progressive dysphagia to symptomatic airway obstruction. They underwent single-stage thoracic endovascular aortic repair with TBE to revascularize the left subclavian artery and exclude the proximal right subclavian artery, followed by right subclavian artery debranching with transposition to right common carotid artery. All 3 patients achieved complete resolution of symptoms postoperatively with technical success on follow-up imaging. This report underscores the efficacy of the Gore TBE as a solution for addressing this complex vascular anomaly.Clinical ImpactAberrant arch configurations that need repair usually require complex extra-antomic debranching of head vessels and or adjunct endovascular maneuvers during TEVAR. Development of branched TEVAR devices have improved surgical approaches in complex aortic arch treatment. The technique described here utilizes the benefits of an off-the-shelf branched TEVAR device, the Gore TBE, in a single stage hybrid procedure to address an aortic pathology that often require multi-stage, multi-device treatment.
Neonatal Kommerell diverticulum with mid-ductal aneurysmal patent ductus arteriosus: serial imaging documentation of early natural history.
We report an exceptionally early diagnosis of a Kommerell diverticulum on the first day of life, distinguished by unusual mid-ductal aneurysmal dilatation of the patent ductus arteriosus. Serial echocardiography documented the serial evolution of spontaneous ductal closure and thrombosis formation. CT confirmed a right aortic arch with a retroesophageal Kommerell diverticulum. The infant remains asymptomatic under close surveillance. This case represents one of the youngest reported cases with serial imaging documentation, providing unique insights into the early natural history and highlighting management challenges in asymptomatic neonatal patients.
[Strategies for aortic arch branch reconstruction during thoracic endovascular aortic repair].
Objective: To explore optimal approaches and strategies for supra-arch branch reconstruction during endovascular repair of aortic arch diseases. Methods: This retrospective study included 50 patients who underwent endovascular aortic arch repair with supra-arch branch reconstruction in the Department of Vascular Surgery, Zhongshan Hospital, Fudan University, from May 2023 to September 2024. The cohort comprised 40 males and 10 females with an age of (65.1±11.4)years (range:35 to 82 years). Underlying pathologies included aortic dissection in 16 patients (acute, n=5; chronic, n=8; post-TEVAR endoleak, n=3), aneurysm in 26, pseudoaneurysm in 1, intramural hematoma in 1, penetrating ulcer in 4, aberrant subclavian artery aneurysm in 1, and Kommerell diverticulum in 1. Surgical strategies were determined after multidisciplinary discussion, considering patients'overall condition, aortic arch anatomy, lesion location, spatial relationship with arch branches, vessel diameters and angles, available stent types, and proximal/distal landing zones. Baseline characteristics, operative details, and postoperative complications were recorded. Six-month survival rate and freedom from major aortic-related adverse event rate were analyzed using Kaplan-Meier methods. Results: Among 50 patients, 75 branches were reconstructed using various techniques, including debranching (2 cases, 4.0%); in-situ fenestration (ISF, 20 cases, 40.0%); branched endovascular aortic repair (4 cases, 8.0%); physician-modified endografting (17 cases, 34.0%); chimney technique (2 cases, 4.0%); and ISF combined with the chimney technique (5 cases, 10.0%). Technical success was achieved in 98.0% (49/50) of patients. Two perioperative deaths occurred because of cerebral hemorrhage (1 case) and spinal cord ischemia (1 case), respectively. The median follow-up period (M(IQR)) was 7.0(6.0) months(range:1.0 to 19.0 months). The 6-month follow-up survival rate was 96.0%, and the freedom from aortic-related adverse events rate was 92.0%. Conclusion: Reasonalbe branch reconstruction strategies could help reduce severe complications of endovascular repair of the aortic arch and achieve better clinical outcomes. 目的: 探讨主动脉弓疾病患者行腔内修复时弓上分支重建方式的选择策略。 方法: 本研究为回顾性病例系列研究。回顾性分析2023年5月至2024年9月复旦大学附属中山医院血管外科收治的50例接受主动脉弓腔内修复及弓上分支重建患者的临床资料。男性40例,女性10例,年龄(65.1±11.4)岁(范围:35~82岁)。其中主动脉夹层16例(急性5例,慢性8例,胸主动脉腔内修复术后内漏3例),动脉瘤26例,假性动脉瘤1例,壁间血肿1例,穿透性溃疡4例,迷走锁骨下动脉瘤1例,Kommerell憩室1例。结合患者全身状态、主动脉弓部解剖特点、主动脉病变位置及其与弓上分支的位置关系、弓上分支直径及成角、临床可获得支架类型、近远端锚定区部位及直径等多因素,经多学科团队讨论后确定手术方案。收集患者基线资料、手术信息和并发症情况。采用Kaplan-Meier法绘制生存曲线,计算患者术后6个月生存率及免于主动脉相关严重事件发生率。 结果: 50例患者共重建75根弓上分支。行去分支患者2例(4.0%);原位开窗20例(40.0%);分支覆膜支架4例(8.0%);体外开窗17例(34.0%);平行支架2例(4.0%);原位开窗联合平行支架5例(10.0%)。98.0%(49/50)的患者获得技术成功,围手术期死亡2例;脑出血和脊髓缺血各1例。随访时间[M(IQR)]为7.0(6.0)个月(范围:1~19个月),术后6个月生存率为96.0%,免于主动脉相关严重不良事件生率为92.0%。 结论: 采用合理分支重建策略,制定个性化手术方案,有助于减少主动脉弓部腔内修复严重并发症,可获得较好的临床疗效。.
