Unicentric Castleman Disease as a Sufficient and Reversible Cause of Steroid-Refractory Autoimmune Hemolytic Anemia in a Child.
Breast-implant associated unicentric castleman disease: a case report.
ALK-negative anaplastic large cell lymphoma is a well-established hematologic complication of prosthetic breast implantation, but other rare conditions should also be considered. We report a case of a 36-year-old female with bilateral retropectoral breast implants since age 20 who presented with fatigue, lower extremity edema, fevers, and a slowly growing left breast mass. Ultrasound of the left breast revealed a 6 × 6 cm left upper quadrant mass. Surgical excision identified pathologic features consistent with hyaline-vascular unicentric Castleman disease (UCD). This is the first case of breast-implant associated UCD, expanding the differential diagnosis for masses associated with prosthetic breast implantation.
How I diagnose Castleman disease.
Castleman disease represents a heterogeneous group of hematologic conditions with a broad spectrum of histopathologic features. Historically, 2 major Castleman disease subgroups have been described-unicentric Castleman disease and multicentric Castleman disease, based on the extent of the disease. More recently, oligocentric Castleman disease, a subtype that falls between unicentric and multicentric, has been identified. Due to the overlapping features of each category, similarities to other conditions, and a lack of objective diagnostic biomarkers, the diagnosis of Castleman disease remains a challenge. Here, we review the diagnostic criteria of each entity and discuss the features that distinguish them from overlapping diseases. This review highlights the clinicopathologic features of multiple cases to depict the wide spectrum of clinical and laboratory characteristics of Castleman disease. It also outlines the diagnostic workup for each subtype. In Castleman disease, the morphologic findings are nonspecific and represent a histologic pattern rather than a pathognomonic feature that allows definitive separation from other morphologically overlapping entities; therefore, correlation with clinical history, radiologic studies, and laboratory findings is essential to arrive at the correct diagnosis. Castleman disease represents a spectrum of clinical syndromes with overlapping morphological features.
Unicentric Castleman Disease of the Mesentery Mimicking a Gastrointestinal Stromal Tumor: A Case Report.
Unicentric Castleman disease (UCD) is a rare benign lymphoproliferative disorder commonly found in the mediastinum and retroperitoneum, with mesenteric involvement being exceptionally uncommon. We present a case of a 21-year-old male with a history of constipation, fatigue, and recurrent right iliac fossa pain for six months. Initial imaging suggested a gastrointestinal stromal tumor (GIST); however, the final diagnosis of UCD hyaline vascular (HV) variant was made based on immunohistochemical (IHC) analysis and histopathological reports. Surgical excision led to complete resolution of symptoms. This unique case makes UCD a differential diagnosis for a right iliac fossa highly vascular mass and underlines the necessity for comprehensive histological and IHC analysis. Successful surgical intervention and exclusion of multicentric Castleman disease (MCD) have resulted in a favorable patient outcome.
Clinical diagnosis and treatment analysis of 11 cases of unicentric Castleman disease in the retroperitoneum.
To investigate the diagnosis, treatment, and prognosis of unicentric Castleman disease (UCD) located in the retroperitoneum. A retrospective analysis was conducted on the clinical data of 11 patients with pathologically confirmed retroperitoneal UCD at Peking University People's Hospital between 2010 and 2025. Data collected included gender, age, clinical manifestations, routine blood tests, tumor markers, treatment modalities, pathological characteristics, and treatment outcomes. Postoperative survival status was assessed via outpatient records and telephone follow-up. The cohort comprised 3 males and 8 females. One male patient had a history of long-term smoking and alcohol use, while the other 10 patients had no such history. One patient presented with lower back discomfort, one with abdominal discomfort, and the remaining 9 patients were incidentally diagnosed during health check-ups. All patients had serum inflammatory markers and tumor markers within normal ranges. Preoperative abdominal contrast-enhanced CT/MRI localized the lesions in all cases. The mean maximum tumor diameter was 5.23 ± 2.05 cm. Five patients underwent laparoscopic surgery, and six underwent open surgery. The mean operative time was 2.73 ± 1.21 hours, with an intraoperative blood loss of 100 ml (IQR 20-100 ml). No severe postoperative complications occurred. The mean hospital stay was 5.00 ± 1.79 days, and all patients recovered well and were discharged. Pathological examination revealed the hyaline vascular type in 9 cases, the plasma cell type in 1 case, and the mixed type in 1 case. The mean follow-up duration was 68.00 ± 30.41 months (range: 25-112 months). By the end of follow-up, no recurrence was observed, and all patients remained alive and healthy. Retroperitoneal unicentric Castleman disease is rare and lacks specific clinical manifestations. Complete surgical resection is the optimal treatment, and patients exhibit excellent long-term prognosis upon follow-up.
