Enteral feeding during cyclooxygenase inhibitor treatment for patent ductus arteriosus: A systematic review.
This study evaluated the effect of modifying enteral feeding during cyclooxygenase inhibitor (COXI) treatment on patent ductus arteriosus (PDA) in preterm infants. We searched MEDLINE, EMBASE, CENTRAL, CINAHL, and ICHUUSHI databases for randomized and nonrandomized controlled trials, checked the references lists and major clinical trials. Non-human and non-English studies were excluded. Evidence was assessed using the Grading of Recommendations, Assessment, Development, and Evaluation. Preterm infants born before 37 weeks of gestation are treated with COXIs for PDA, including indomethacin and ibuprofen. The primary outcomes were the composite outcome of necrotizing enterocolitis (NEC) or gastrointestinal perforation and its components (NEC and gastrointestinal perforation). Two randomized controlled trials (n = 303) and one retrospective cohort study (n = 261) were included (total: n = 564). No significant differences were observed in the composite gastrointestinal outcome (risk ratio [RR]: 1.10, 95% confidence interval [CI]: 0.51-2.37; 1 study, 177 infants, very low certainty of evidence [CoE]), NEC (RR: 1.01, 95% CI: 0.47-2.15; 2 studies, 300 infants, very low CoE), or gastrointestinal perforation (RR: 1.94, 95% CI: 0.25-14.81; 2 studies, 300 infants, very low CoE) between discontinuation and continuation of enteral feeding. Discontinuation of enteral feeding resulted in a significantly lower rate of surgical closure for PDA compared to continuation (RR: 0.56, 95% CI: 0.37-0.86; 1 study, 177 infants, low CoE). Neither discontinuation nor continuation of enteral feeding significantly affected the composite gastrointestinal outcome and its components (NEC and gastrointestinal perforation).
Patent ductus arteriosus: emerging trial evidence firmly supports conservative management in preterm infants.
To summarize recent evidence from randomized clinical trials (RCT) and meta-analysis of patent ductus arteriosus (PDA) closure therapies versus conservative/expectant management on key outcomes such as bronchopulmonary dysplasia (BPD) or death in very preterm (<32 weeks' gestation) infants. Of 10 completed RCTs in the past decade of medical therapies before 2 weeks of age to close a PDA, none demonstrated benefit in reducing moderate-severe BPD/death by 36 weeks postmenstrual age as compared to placebo treatment. When combined, the meta-analysis of the 10 RCTs showed an elevated risk of BPD/death [relative risk (RR): 1.10, 95% confidence interval (CI), 1.01-1.19) and higher risk of death by 36 weeks postmenstrual age or discharge (RR: 1.25; 95% CI, 1.01-1.56). A 2025 Cochrane meta-analysis also found a similar concerning trend of increased mortality following early medical treatment for PDA. Despite their recent popularity, there are no published trials demonstrating safety or efficacy of transcatheter PDA closure. We now have ample evidence for all clinicians to finally acknowledge that conservative PDA management is the correct path forward. Clinical experimentation is no longer acceptable, and expectant PDA management must be adopted as the current standard if we are to adhere to our foundational principle, 'first, do no harm'.
Premature foramen ovale restriction/closure in pregnancy: echocardiographic features and overlap with aortic coarctation.
Evaluation of the foramen ovale (FO) is essential for understanding right heart predominance in the fetal circulation during the third trimester. Rather than considering the FO as a simple orifice, it should be assessed as a complex anatomical and functional system. Premature FO restriction or closure represents a clinically relevant but diagnostically challenging condition in fetal echocardiography. We conducted a monocentric retrospective study including 600 uncomplicated pregnancies between 22 and 38 weeks of gestation that underwent fetal echocardiography at the Cardiology Service of ASP Reggio Calabria between January and December 2023. The FO was assessed by evaluating septum primum morphology and mobility, ostium secundum configuration, and interatrial shunt direction and magnitude on color Doppler, together with indirect hemodynamic markers. All examinations were conducted by a single operator. All pregnancies resulted in term deliveries with normal neonatal outcomes, and postnatal follow-up lasted 6-9 months. Restrictive or premature closure of the FO was identified in 9 fetuses (incidence 1:66 pregnancies), with FO restrictiveness observed in seven cases and premature closure in two cases. Reduced interatrial shunting was defined as a limited or absent right-to-left flow across the FO on color Doppler, associated with impaired septum primum mobility and/or altered FO geometry. The most consistent indirect echocardiographic sign was right atrial dilation (100% of cases), followed by right ventricular and pulmonary artery predominance with a widely patent ductus arteriosus. In some fetuses, the FO appeared morphologically wide but functionally restrictive. All cases showed favorable early postnatal outcomes. This study evaluates the incidence of FO restriction or premature closure in physiologically normal pregnancies during the third trimester and reviews echocardiographic features useful for its identification.
Fetal Intervention for Giant Chorangioma with Prenatal Ductus Arteriosus Closure: A Case Report.
Introduction We present a case of spontaneous premature ductus arteriosus (DA) closure following modified interstitial laser ablation (ILA) of a giant chorangioma (GC) in a fetus with evidence of hydrops. Case presentation A 26-week ultrasound revealed a 10-cm GC in the posteriorly located placenta with signs of fetal hydrops, polyhydramnios, and elevated middle cerebral artery peak systolic velocity (MCA PSV) of 1.9 MoM. Preoperative fetal echocardiogram showed elevated combined cardiac output (753 ml/kg/min) without structural abnormalities. Modified ILA targeting the branches of the feeder artery with an extended-pulse technique (30 seconds at 40 Watts) followed by intrauterine fetal transfusion were performed without complications. On post-operative day (POD) 1, new onset of ductus venosus flow reversal was noted, which was attributed to post-procedural hemodynamics. Fetal echocardiography on POD 5 suggested constriction of DA; follow-up confirmed complete DA closure and secondary right heart dysfunction. The pregnancy was managed expectantly with at least weekly echocardiograms. Digoxin and hyperoxygenation for fetal heart failure did not change DA caliber. Acetaminophen and fluoxetine were discontinued without improvement. After an uncomplicated term cesarean delivery (indication: breech), the female neonate was discharged home on day of life 3. Currently, the child is 6 months old and healthy. Conclusion Modified ILA for GC followed by expectant management of associated DA closure yielded a term delivery with a favorable outcome.
Favorable Procedural Outcomes in Lower Versus Higher Weight Premature Infants Undergoing Transcatheter Patent Ductus Arteriosus Closure.
Timing of device closure of patent ductus arteriosus (PDA) in preterm infants has been a topic of debate. Limited data exist comparing procedural outcomes based on infant weight at the time of the procedure. This study aimed to compare procedural outcomes and adverse events between premature infants weighing <2 kg and those weighing 2 to 5 kg undergoing PDA device closure. We performed a retrospective review of premature infants (<37 weeks gestational age) who underwent transcatheter PDA closure at 2 institutions between 2013 and 2022. Patients were divided into 2 groups: group 1 (<2 kg) and group 2 (2-5 kg). Demographic characteristics, procedural characteristics, success rates, and adverse events were analyzed. Procedural success was defined as successful device placement with no more than small residual shunt and no device embolization within 24 hours. A total of 177 patients were included: 81 in group 1 (<2 kg) and 96 in group 2 (2-5 kg). Procedure success was significantly higher in group 1 than in group 2 (94% vs 83%, P = .031). Group 1 also had shorter median procedure time (50 vs 67 minutes, P = .002) and fluoroscopy time (6.4 vs 11.4 minutes, P < .0001). Adverse events occurred in 11.9% of patients, with a lower rate in group 1, but was not statistically significant (8.6% vs 14.6%, P = .22). No procedure-related mortality occurred. Procedural outcomes of transcatheter PDA closure in premature infants are more favorable among smaller infants with higher success rates and fewer adverse events. Lower weight alone should not delay transcatheter PDA closure in preterm infants with hemodynamically significant PDA.
