A epilepsia do lobo temporal é o tipo mais comum de epilepsia de início focal em adultos. Os sintomas e o comportamento das crises distinguem as crises originadas no lobo temporal medial das crises originadas no lobo temporal lateral (neocortical). O diagnóstico depende de estudos eletroencefalográficos (EEG) e de neuroimagem. Medicamentos anticonvulsivantes, cirurgia de epilepsia e tratamentos dietéticos podem melhorar o controle das crises.

Ablation of Unilateral Hippocampal GABAergic Neurons: A Novel Mouse Model of Mesial Temporal Lobe Epilepsy With Hippocampal Sclerosis.
While various animal models of mesial temporal lobe epilepsy (MTLE) exist, a validated model based solely on hippocampal GABAergic interneuron loss is lacking. We aimed to establish and characterize a novel MTLE with hippocampal sclerosis (HS) model by selective ablation of these neurons. Using AAV vectors (flex-DTA or DIO-taCasp3-TEVp), we performed unilateral, partial ablation of GABAergic neurons in the dentate gyrus (DG) or CA1 subregions of VGAT-Cre mice. Spontaneous recurrent seizures (SRS) were monitored by video-EEG. Behavioral tests and histopathological analyses were conducted to assess comorbidities and HS features. DG-ablated mice all developed SRS, whereas 50%-70% of CA1-ablated mice did. Lethal SRS occurred in around 50% of DG-ablated mice. Both ablation models exhibited sustained SRS from Days 8-28, distinct from kainic acid model severity in some parameters. The model recapitulated anxiety-like behaviors, cognitive deficits, and hallmark HS pathology including gliosis, granule cell dispersion, and mossy fiber sprouting. Selective unilateral hippocampal GABAergic interneuron loss is sufficient to drive chronic epilepsy with HS. This model provides direct causal evidence and a precise platform for investigating MTLE-HS mechanisms and therapies.
Influence of epilepsy syndrome on cancer comorbidity.
The relationship between specific epilepsy syndromes and cancer is unclear. We aimed to analyze cancer frequency and its patterns in adults with Idiopathic Generalized Epilepsies (IGE) and Mesial Temporal Lobe Epilepsy with Hippocampal Sclerosis (MTLE-HS). We retrospectively studied 719 adults (471 IGE; 248 MTLE-HS) followed at a tertiary epilepsy clinic (2016-2023). Data included demographics, epilepsy characteristics, antiseizure medications (ASM), and oncological history, excluding intracranial tumors. Cancer occurred in 4.9% of IGE and 12.9% of MTLE-HS patients (p < 0.001); however, this difference disappeared after age adjustment. Endocrine tumors predominated in IGE (p = 0.031), whereas hematologic malignancies, exclusively of the B-cell lineage, were more common in MTLE-HS (p = 0.033). Five patients (1 IGE; 4 MTLE-HS) developed skin squamous cell carcinoma after long-term sodium channel blocker therapy. Our findings reveal different, syndrome-specific associations between epilepsy and cancer types, likely reflecting a shared genetic susceptibility, with convergent dysregulated immune pathways potentially underlying these patterns. Understanding these relationships may offer insights into pathophysiology and guide the prevention of cancer comorbidities in these patients.
Hippocampal Gene Expression in Mesial Temporal Lobe Epilepsy With Hippocampal Sclerosis: Assessment of Human Herpesvirus 6B DNA Status.
Mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS) is an intractable form of epilepsy involving the hippocampus, and temporal lobectomy remains an effective treatment. Human herpesvirus 6B (HHV-6B) establishes latency in the hippocampus and may contribute to MTLE-HS pathogenesis by altering host gene expression; however, transcriptomic data from healthy controls remain limited. This study investigated the role of HHV-6B to MTLE-HS pathogenesis by analyzing gene expression in resected hippocampal tissues. Samples were collected from 12 to 43 HHV-6 DNA-positive and -negative patients, respectively, and three controls. RNA sequencing was performed on eight representative samples, followed by RT-qPCR validation of nine selected genes in 58 samples. RNA sequencing identified 600 differentially expressed genes (210 upregulated, 390 downregulated) between HHV-6B-positive MTLE-HS and controls. Pathway enrichment analysis revealed involvement of synaptic signaling and inflammatory responses, with prostaglandin biosynthesis specifically upregulated in HHV-6B-positive tissues. Two genes were significantly upregulated in HHV-6B-positive compared with HHV-6B-negative samples. RT-qPCR confirmed elevated cholesterol 25-hydroxylase and interleukin 1 beta expression in HHV-6 DNA-positive samples (both p = 0.031). These findings suggest that HHV-6B may contribute to MTLE-HS pathogenesis by modulating the expression of host inflammatory genes, supporting a role for neuroinflammation and the potential benefits of anti-inflammatory therapies.
Volumetric MRI study of the brain in patients with neurocysticercosis and mesial temporal lobe epilepsy.
Neurocysticercosis (NCC) is a common parasitic infection of the central nervous system and a known cause of focal epilepsy. Its potential role in triggering or contributing to mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS) is suggested, but the impact on brain volumetry remains unclear. This study aimed to assess volumetric differences in the brain, with a particular focus on the hippocampus and temporal lobes, in patients with NCC, MTLE, MTLE-HS, and their combinations. This was an observational, cross-sectional, retrospective, single-center exploratory study including five groups: healthy controls; isolated MTLE; isolated NCC; MTLE-HS; and MTLE-HS combined with NCC (MTLE-HS plus NCC). All participants had at least one magnetic resonance imaging (MRI) examination suitable for volumetric post-processing. Volumetric analysis was performed on cerebral cortex regions (frontal, parietal, temporal, and occipital lobes), cerebellum, total and white matter limbic system, amygdala, hippocampus, and thalamus using the T1 MultiAtlas Segmentation tool (Brain GPS©). We also assessed sensitivity by estimating the minimum detectable difference from adjusted models and performed an exploratory analysis of NCC lesion topography with Welch t-tests on lobar/subcortical volumes. After adjusting for age and sex, no significant group differences were found in hippocampal or temporal lobe volumetrics (all p > .10). Effect sizes were small (partial η2 ≤ .10), and MANOVA confirmed the absence of a global effect (Pillai's Trace = .114, p = .623). Adjusted means showed only modest variation with overlapping confidence intervals, indicating that differences likely reflect individual variability rather than systematic group effects. The findings indicate that NCC, whether isolated or combined with MTLE-HS, does not cause significant alterations in brain volume. This suggests the absence of an additive or synergistic effect of NCC on brain morphostructural changes in patients with MTLE-HS. These results contribute to understanding the pathophysiology of epilepsy related to NCC and hippocampal sclerosis, supporting that volumetric brain changes may not underlie their association.
The interacting etiologies of hippocampal sclerosis in epilepsy: A scoping review.
According to the International League Against Epilepsy classification, mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS) is considered an epilepsy syndrome. Several etiologies may precede HS, but there is little overview in the literature about these etiologies. We conducted a scoping review to systematically search, select, analyze, and synthesize the existing knowledge about the etiology of HS in patients with epilepsy. MEDLINE, Embase, Web of Science Core Collection, and Cochrane Library were searched using a systematic search strategy. Relevant data were selected according to strict criteria. A bibliometric analysis was done by VOSviewer software. A total of 907 studies were selected and classified. The main classes of etiologies we found are prolonged (febrile) seizures, genetic predisposition, infection, autoimmunity, trauma, neurodegeneration, developmental aspects, toxins (e.g., domoic acid), and hypertensive events (e.g., Posterior reversible encephalopathy syndrome), with several interactions between different etiologies. Autoimmunity and neurodegeneration were the most recently described etiologies. We found that the etiology of MTLE-HS is complex, with several risk factors influencing susceptibility and a heterogenous pathophysiology between patients. Future, prospective studies addressing the interactions of these etiologies in the development of HS are warranted. A better understanding of the interacting etiologies of MTLE-HS could have a major impact on the development of more precise diagnostic biomarkers, preventive strategies, and treatment options for this epilepsy syndrome.
