Circumferential venous sinus: an innovative separation method for total vertical type II craniopagus twins.
Craniopagus twins, particularly the total vertical type II variant, represent one of the rarest and most complex congenital malformations, with significant surgical challenges. This technical note presents the successful staged separation of craniopagus twins at 3 years 9 months of age, the oldest known to be separated to date. The twins shared a prominent circumferential venous sinus, requiring a novel surgical strategy. Advanced imaging, 3D modeling, and virtual reality planning in the metaverse-between Brazil and the United Kingdom-guided the 7 staged surgeries. Initially, partial separations focused on the more vulnerable twin. However, arteriographic evidence of venous dependency led to a paradigm shift, in which the authors performed the final stages on the other twin to preserve critical venous outflow. This innovative approach allowed division of the shared venous sinus without neurological compromise. The final separation, completed in 2 stages over 31 hours, resulted in successful independent circulations and recovery. This case highlights the importance of individualized surgical planning, collaborative international simulation, and the potential for adapting separation protocols based on venous architecture.
Patient-centred care of cephalo-omphalopagus conjoined twins.
Conjoined twins are rare, occurring in 1 in 250 000 live births, with cephalopagus twins reported in 1 in 3 000 000 live births. In cases of conjoined twins, early diagnosis is crucial for the health of the mother and pregnancy management and counselling. We present a case of cephalo-omphalopagus conjoined twins, with imaging findings, delivery considerations and patient-centred care recommendations. A patient in her 30s, gravida 2 para 0, was transferred to our care after an anatomical ultrasound demonstrated fusion of the fetal heads, chests and abdomens. Due to a complex congenital heart defect with a shared heart, the family was counselled on the anticipated prognosis and management options. Twin girls were delivered at 34+4 weeks via caesarean delivery, and the family was able to spend time with them as desired. This case demonstrates that prenatal diagnosis is helpful, and family involvement through patient-centred care and shared decision-making can improve perceived outcomes.
Partial craniofacial duplication in a resource-limited setting: a case report.
Diprosopus, or craniofacial duplication, is an extremely rare congenital malformation, representing only 0.4% of conjoined twins with an estimated prevalence of 1 in 15 million births. Fewer than 40 cases have been reported worldwide. We describe a case of partial craniofacial duplication in a child from rural Ethiopia, highlighting the challenges of delayed diagnosis and management in a resource-limited setting. A 7-year-old girl presented with a congenital mandibular mass that had remained untreated since birth due to a lack of antenatal care (ANC) and delayed health-seeking. Examination revealed a pedunculated soft tissue lesion containing duplicated rudimentary facial elements, including tooth-like structures and lip-like tissue. Laboratory investigations were normal, and skull radiography showed no bony abnormalities. Advanced imaging was not feasible. The mass was excised surgically, and gross and histopathological evaluation confirmed partial craniofacial duplication. Postoperative recovery was uneventful, with improved psychosocial outcomes. Unlike complete diprosopus, which is typically incompatible with life, partial forms may permit long-term survival. The embryopathogenesis remains debated, with proposed mechanisms including incomplete embryonic disc division and neural crest duplication. While management in well-resourced settings is often facilitated by advanced imaging and multidisciplinary planning, this case demonstrates that direct surgical excision can achieve favorable outcomes even where resources are limited. This rare case contributes to the limited literature on diprosopus and underscores the importance of strengthening ANC, anomaly screening, and community awareness. Timely surgical intervention, even in low-resource environments, can restore function and enhance psychosocial outcomes.
"Separation of Craniopagus Twins: A 30-year Systematic Review of Surgical Outcomes".
Craniopagus twins, conjoined at the calvarium, are the rarest and most complex form among conjoined twins, comprising ~3.4% of all cases. Surgical separation presents a significant challenge due to shared venous structure, brain parenchyma, and brain tissue. Despite advancements in surgical techniques, factors associated with successful separations remain poorly understood. A systematic literature review was conducted to identify all reported craniopagus separation cases from 1995 to 2025, using Ovid MEDLINE and Scopus. A supplemental search of news articles was additionally performed via Google News to capture any recent or unpublished cases. A total of 33 cases met the inclusion criteria. Extracted variables included sex, age of separation, anatomical classification (O'Connell, Winston, Stone-Goodrich), surgical staging, use of tissue expanders, and urgency of procedure. The primary outcome measure was successful separation defined as the survival of both twins through 30 days postoperatively. Associations were analyzed using Fisher's exact test (p<0.05). Among the three anatomical classifications, only the Stone-Goodrich system showed a significant association with surgical outcome (p=0.006), with total vertical (TV) cases achieving the highest success rate (90%). Statistically significant associations with successful outcome were also observed for multi-stage surgery (p=0.037), use of tissue expanders (p=0.006), and non-emergent procedures (p=0.039). Anatomical configuration, surgical staging, utilization of tissue expanders, and urgency of operation were significantly associated with craniopagus separation outcomes. These findings highlight the importance of procedural factors and surgical advancements that may improve the survival of twins during separation.
Conjoined Parasitic Twins (Cephalo-Thoracopagus Parasiticus) in a Free Ranging Northern Bat (Eptesicus nilssonii): A Micro-CT Anatomic and Genetic Survey.
The detailed anatomical situation of male stillborn cephalo-thoracopagus twins in a wild ranging northern bat (Eptesicus nilssonii) is described by means of full body micro-CT scans in high resolution with three-dimensional computational reconstruction. The external anatomy was characterised by supernumerary wings and legs and marked cranial disfiguration. The most striking features of the internal anatomy were the presence of two hearts with different characteristics, three lungs with two tracheas, three kidneys as well as a doubled liver. Vascular aberrations and other organ deformities are described in detail. The monozygotic origin of the present parasitic conjoined twin was shown by genetic sequencing, analysis and relatedness calculations based on individual tissue samples from the autosite and the parasite. As the free ranging 1 year old mother of this twin returned every year to the same well monitored nursery colony in Norway to give birth to its pups, we can present the life history of the mother bat including the reproductive data of five more consecutive years. This enabled us also to evaluate the birthing process of the conjoined twin in the light of physiological behavioural birthing data.
