Anti-MRPS27 Antibody: A Novel Diagnostic Biomarker for Autoimmune Hepatitis Identified via a Human Proteome Microarray.
To identify autoimmune hepatitis (AIH)-specific autoantibodies using a high-content human protein microarray and evaluate their diagnostic performance. Pretreatment sera from untreated patients with AIH (n = 68), primary biliary cholangitis (PBC, n = 19), metabolic dysfunction-associated steatotic liver disease (MASLD, n = 39), chronic hepatitis C (CHC, n = 27), and drug-induced liver injury (DILI, n = 18) and healthy controls (HC, n = 37) were screened with a wheat-germ, cell-free microarray displaying > 16,000 proteins. Candidate antigens were validated by ELISA, and antibody titers were correlated with clinical variables, treatment response, and outcomes. Mitochondrial ribosomal protein S27 (MRPS27) emerged as the dominant AIH-associated antigen. The mean anti-MRPS27 antibody titer in untreated patients with AIH (50.2 AU) exceeded that in patients with PBC (9.3 AU), MASLD (12.8 AU), CHC (24.8 AU), DILI (19.8 AU), and HC (7.5 AU, all p < 0.01). A cutoff of 35.4 AU (HC mean + 2 SD) yielded a sensitivity of 61.8%, a specificity of 87.1%, a positive predictive value of 70.0%, and a negative predictive value of 82.4%. Positivity was unrelated to baseline histology, biochemistry or IgG, and titers were unchanged after corticosteroid induction. Interestingly, patients with low titers experienced more liver-related events during follow-up. The anti-MRPS27 antibody is a promising adjunctive serological marker that enhances the specificity of AIH diagnosis. The pathogenic significance and prognostic value of this antibody warrant prospective validation.
Geographic distribution and demographic patterns of primary biliary cholangitis and autoimmune hepatitis in Colombia: a national population-based administrative study (2018-2024).
Autoimmune liver diseases, including primary biliary cholangitis (PBC) and autoimmune hepatitis (AIH), are rare chronic conditions with significant morbidity. However, population-based epidemiological data from Latin America remain limited. This study aimed to describe the geographic distribution and demographic patterns of PBC and AIH in Colombia between 2018 and 2024. A descriptive, cross-sectional population-based study was conducted using data from the Comprehensive Social Protection Information System (SISPRO) of the Colombian Ministry of Health. Individuals with at least 1 recorded diagnosis coded as K74.3 (PBC) or K75.4 (AIH) according to the International Classification of Diseases, 10th Revision (ICD-10), were identified. Administrative prevalence estimates were calculated per 100,000 population, using official population projections from the National Administrative Department of Statistics (DANE). Age- and sex-specific distributions were analyzed, and geographic variation was visualized using choropleth maps at the regional level. A total of 6504 PBC cases and 11,225 AIH cases were identified. The estimated national administrative prevalence was 14.7 per 100,000 population for PBC and 17.0 per 100,000 for AIH. Both conditions showed marked female predominance. PBC was more frequent in women aged 50-69 years. However, AIH demonstrated a broader age distribution, affecting individuals from young adulthood to older age. Higher prevalence estimates were observed in multiple northern regions, particularly in the Caribbean region. This first nationwide administrative assessment of PBC and AIH in Colombia demonstrates marked sex differences and regional variation, highlighting the need to improve diagnostic awareness and strengthen epidemiological surveillance of autoimmune liver diseases in Latin America.
Integrative omics and phase IIa clinical trial identify TNF as key node in autoimmune hepatitis.
Autoimmune hepatitis (AIH) patients experience increased mortality and severe side effects from non-specific immunosuppressive therapy, highlighting an urgent need for targeted treatment approaches. Here, we aimed to delineate the cellular and molecular network underlying AIH within its spatial context and to validate a key therapeutic target in a clinical trial. We employed computational modelling, multi-omics analyses, and functional experiments to map the immune landscape of AIH. In addition, we conducted a steroid-free open-label phase IIa clinical trial using infliximab, a TNF-targeting antibody, in AIH patients. Our studies revealed that myeloid cell and hepatocyte-derived IL-15 promotes cytotoxicity and proliferation of liver auto-aggressive CD8+ T cells. Full execution of their cytotoxic program is licensed by TNF derived from clonally expanded liver-resident CD4+ T cells. AIH hepatocytes respond to TNF by increasing expression of adhesion molecules making them targets for both CD8+ and CD4+ T cells. In the clinical trial, targeting TNF with infliximab demonstrated efficacy as an entirely steroid-free AIH treatment. These findings elucidate the immune network in AIH and identify TNF as one of the central network nodes. Accordingly, our findings provide the basis for novel targeted, steroid-free immune therapies such as the use of infliximab. These findings have significant implications for the treatment of autoimmune hepatitis. By mapping the spatial and functional immune network within the AIH liver, this study identifies IL-15 and TNF as central drivers of T cell-mediated cytotoxicity, offering new precision targets for intervention. The successful use of infliximab as a steroid-free therapy in a phase II trial marks a pivotal step toward safer, more specific treatment options for AIH patients. This research not only advances our understanding of AIH pathogenesis, but also sets the stage for broader application of immune-targeted therapies in autoimmune liver diseases.
Association between serum uric acid and non-alcoholic fatty liver disease (NAFLD): an observational cross-sectional study in an Egyptian outpatient cohort.
Hepatic steatosis, characterized by excessive triglyceride accumulation within hepatocytes, is the central feature of non-alcoholic fatty liver disease (NAFLD). This spectrum encompasses simple steatosis, non-alcoholic steatohepatitis (NASH), progressive fibrosis, and cirrhosis. NAFLD pathogenesis involves an intricate interplay between nutritional factors, metabolic dysregulation, and genetic predisposition. Evidence suggests hyperuricemia is an independent risk factor for NAFLD, with elevated serum uric acid (SUA) levels associated with increased steatosis severity and fibrosis advancement. This study investigated the association between SUA levels and liver involvement—specifically ultrasound-assessed steatosis grading and elastography-measured liver stiffness—in patients with NAFLD. This cross-sectional study enrolled 70 patients aged ≥18 years with ultrasound-diagnosed NAFLD (Group I) and 70 age-, sex-, and residence-matched healthy controls (Group II). Participants were recruited from Minia University Liver Hospital between March 2022 and February 2023. Exclusion criteria included excessive alcohol consumption, viral or autoimmune hepatitis, established cirrhosis, and medications affecting uric acid metabolism. The primary outcome was presence of NAFLD. Evaluations comprised detailed medical history, anthropometric measurements, laboratory testing (including SUA), abdominal ultrasonography for steatosis grading, and two-dimensional shear wave elastography for liver stiffness assessment. Hyperuricemia prevalence reached 55.7% (n=39) among NAFLD patients compared to 0% in controls (p=0.001). Mean SUA concentrations were significantly elevated in the NAFLD group versus controls (5.98 ± 1.9 mg/dL vs. 4.34 ± 0.7 mg/dL, p=0.001). A significant association emerged between hyperuricemia and hepatic steatosis grades (p=0.032). Multivariate logistic regression identified SUA as independently associated with NAFLD after adjustment for confounders (Adjusted OR 1.45; 95% CI: 1.12–1.88, p=0.005 per 1 mg/dL increment), alongside BMI, waist circumference, triglycerides, and creatinine. Receiver operating characteristic (ROC) analysis demonstrated an optimal 5.3 mg/dL SUA cut-off for NAFLD discrimination (AUC 0.74, 64% sensitivity, 94% specificity, p=0.001). Elevated SUA levels were consistently associated with NAFLD presence, steatosis severity, and increased liver stiffness. The 5.3 mg/dL threshold warrants further validation. Notable limitations include single-center recruitment potentially limiting generalizability, a cross-sectional design precluding temporal relationship and causality determination, and the possibility of residual confounding from unmeasured variables. Autoimmune Hepatitis Unmasked by Acute Viral Hepatitis: A Diagnostic and Therapeutic Challenge.
Autoimmune hepatitis (AIH) is a rare disease with unclear pathogenesis. Acute viral hepatitis (AVH) can trigger autoimmunity and unmask underlying AIH, which can be difficult to recognize due to overlapping clinical features. This case series included 10 patients diagnosed with AIH following confirmed AVH. Demographics, clinical presentation, investigations, biopsy results, and treatment outcomes were analyzed. Diagnosis of AIH was established using the simplified AIH score. The cohort included seven patients with hepatitis A virus, two with hepatitis E virus, and one with hepatitis B virus. Mean age was 26.9 ± 10.6 years with male predominance (n = 7,70%). Median time to AIH diagnosis was 82 (68-125) days after initial symptoms. Serum immunoglobulin G was elevated in all patients [median: 1834.5(1138-3858 mg/dL); normal <1700 mg/dL]. Anti-nuclear antibody was positive in 40%, anti-smooth muscle antibody in 30%, with 40% being seronegative. Liver biopsies showed interface hepatitis (90%), lobular inflammation (100%), and lymphoplasmacytic infiltrate (100%). Fibrosis (Metavir scoring system) stages were F0 (30%), F1 (20%), F2 (30%), and F3 (20%). All patients achieved complete biochemical remission with prednisolone (dose: 0.5-1 mg/kg/day, and azathioprine (dose: 1-2 mg/kg/day, median initiation time: 45 days after prednisolone start). One attempt at immunosuppression withdrawal resulted in flare. AVH can unmask underlying AIH and protracted course beyond the expected course of AVH for prompt evaluation of AIH. Standard immunosuppressive therapy is generally effective and long-term maintenance treatment may be required.
