Cardiac Transplantation in a Patient With Situs Inversus Totalis and Extracardiac Fontan.
This case report describes a successful dextrocardia-preserving orthotopic heart transplant in a patient with situs inversus totalis and an extracardiac Fontan, utilizing a levocardia donor heart. The donor heart was rotated 90° counterclockwise along its longitudinal axis to optimize inferior venous connection and incorporate existing extracardiac Fontan. Superior venous connections were secured using a harvesting technique preserving the innominate vein. A GORE-TEX graft was wrapped around the innominate vein to prevent its compression as it passed through the aortic arch. Counterclockwise rotation requires adequate pulmonary artery length. In the absence of extra aortic tissue to allow mild aortic bowing and prevent superior venous obstruction, a GORE-TEX graft offers an effective alternative to maintain unobstructed superior venous drainage. Dextrocardia-preserving transplantation is feasible with a 90° counterclockwise rotation. Reusing the extracardiac Fontan reduces ischemic time; reinforcing the innominate vein with GORE-TEX prevents compression, securing venous integrity.
A Young Patient With Gunshot Wounds.
Cardiac herniation secondary to pericardial rupture is a rare but life-threatening complication of severe chest trauma. Prompt recognition is critical, as the initial imaging findings can be subtle, and delayed diagnosis often results in rapid clinical deterioration. To highlight the importance of early diagnosis and intervention in cases of cardiac herniation following traumatic pericardial rupture, with a focus on recognizing key radiographic and computed tomography findings that can guide emergent management. A 20-year-old male presented after sustaining multiple gunshot wounds, including penetrating injuries to the left chest and back. On arrival, he was in hemorrhagic shock, exhibiting tachycardia and hypotension. Initial chest radiography demonstrated mediastinal and cardiac displacement into the left hemithorax, with the absence of the right heart border. Further evaluation with chest CT confirmed levocardia with cardiac levorotation, right pulmonary artery kinking, left atrial compression, and pneumopericardium. The patient subsequently underwent an emergency thoracotomy where a large pericardial rupture with cardiac herniation was confirmed. The heart was manually repositioned and the pericardial defect was securely sutured. This case underscores the critical role of early imaging in diagnosing cardiac herniation, a condition often associated with high mortality.
Total anomalous pulmonary venous connection - prenatal echocardiography and neonatal follow-up.
Total anomalous pulmonary venous connection is a rare congenital heart defect that can be diagnosed prenatally and might be very important for perinatal management. In addition to other cardiac abnormalities (levocardia, situs inversus, small left ventricle, double outlet right ventricle, parallel great vessels, and hypoplastic aortic arch), total anomalous pulmonary venous connection of a subdiaphragmatic type was diagnosed during a prenatal echocardiography examination in the second half of pregnancy. Fetal echocardiography monitoring showed no signs of congestive heart failure. The neonate was born at 38 weeks of gestation at our tertiary center. Postnatal echocardiography revealed significant progression in neonatal hemodynamics, and early cardiac surgery, involving repair of the pulmonary veins, pulmonary artery banding, and aortic arch reconstruction, was performed with a good outcome. This case is an excellent example of the value of prenatal echocardiography.
Incidental finding of levocardia with situs inversus: case report.
"Situs inversus with levocardia" refers to the mirror-image lateral orientation of the abdominal organs with a normally oriented, left-sided heart. This anatomical anomaly arises from abnormalities in the biochemical signaling systems mediating embryological development. We present a case of situs inversus with levocardia incidentally discovered in a healthy 24-year-old male during workup following a motor vehicle collision. The patient had no significant prior medical history and was unknown to harbor such anatomical anomalies. Although our presented patient had no pathology associated with his anatomical abnormalities, situs inversus with levocardia is highly associated with concomitant cardiac malformations. These associated cardiac malformations are often incompatible with life, and many patients die in early childhood; nevertheless, we present a rare case of situs inversus with levocardia discovered incidentally in a healthy young adult with no associated cardiac pathology.
