A Case of Nevus of Ota Associated with Agminated Blue Nevus.
Dermal melanocytosis is characterized by the presence of spindle-shaped melanocytes in the dermis. Common forms include Mongolian spots, nevus of Ota, nevus of Ito, and blue nevus. Nevus of Ota typically presents as bluish-brown patches involving the sclera and the cutaneous regions innervated by the ophthalmic and maxillary branches of the trigeminal nerve unilaterally. Blue nevus usually manifests as a solitary lesion. Agminated blue nevus is a rare variant that often presents as grouped, linear, or blaschkoid-distributed deep blue macules or papules. Only a few cases have reported the co-occurrence of nevus of Ota and agminated blue nevus. We herein report a case of extensive nevus of Ota combined with agminated blue nevus involving all three branches of the trigeminal nerve.
Clinical and dermoscopic characteristics of facial hyperpigmentation disorders in skin of color.
Facial hyperpigmentation disorders (FHD) are common in dermatology, particularly in people with skin of color (SOC). They include melasma, post-inflammatory hyperpigmentation, and less frequent conditions such as exogenous ochronosis, lichen planus pigmentosus, Riehl's melanosis, acanthosis nigricans, seborrheic melanosis, pigmentary demarcation lines, drug-induced pigmentation, and nevus of Ota, among others. These conditions may significantly affect quality of life. They can present similar clinical and dermoscopic features, making diagnosis challenging. A thorough medical history, physical examination, and careful dermoscopic evaluation are essential to achieve an accurate diagnosis. In this article, we present a narrative review of the clinical and dermoscopic characteristics of FHD in SOC and provide diagnostic clues for the early diagnosis of these conditions.
Uveal melanoma associated with Nevus of Ota in a German Shepherd dog.
A 4-year-old male German Shepherd dog was presented with a 3-day history of ocular pain in the right eye. The owner reported facial skin hyperpigmentation on the right side since puppyhood, consistent with a congenital condition known as Nevus of Ota or (oculo)dermal melanocytosis. Ophthalmological examination of the right eye revealed diffuse corneal oedema, vascular scleral congestion, diffuse iris hyperpigmentation, iris bombè and dyscoria; intraocular pressure was markedly elevated (41 mmHg). Ipsilateral facial skin was diffusely hyperpigmented along ophthalmic and maxillary branch innervation. The left eye showed no abnormalities. Ultrasonography of the right eye revealed severe iris thickening. Despite medical management consisting of topical anti-inflammatory and antiglaucoma therapy, clinical signs progressively worsened over the following 4 months, and enucleation was ultimately performed; the globe was subsequently submitted for histopathological examination. Preoperative thoracic radiography and abdominal ultrasonography were declined by the owner. Histopathologically, the anterior uveal architecture was completely affected by an infiltrating neoplasia, with transscleral extension to surrounding orbital tissues. An anterior uveal mixed cell type, scarcely melanotic melanoma was diagnosed. The dog died because of suspected pulmonary metastases 2 months after enucleation. To the best of our knowledge, this is the first report of a uveal melanoma associated with the nevus of Ota in a dog.
Investigating the efficacy and safety of the 755-nm picosecond alexandrite laser in treating nevus of Ota: A systematic review and meta-analysis.
The nevus of Ota is an uncommon pigmentary skin condition that causes bluish-gray pigmentation and is traditionally treated with a Q-switched laser. Emerging evidence suggests that the 755-nanometer picosecond alexandrite laser is a safe and effective therapeutic option. We sought to determine the overall effectiveness and safety profile of the 755-nanometer (nm) picosecond alexandrite laser for managing nevus of Ota. We reviewed 6 databases for studies that addressed the safety as well as efficacy profile of the 755-nm picosecond alexandrite laser for nevus of Ota. Ten studies, including 558 individuals with nevus of Ota, were included. The main outcomes were the rate of excellent clinical response and reported side effects encompassing erythema as well as post-inflammatory pigmentary alterations, including both hyperpigmentation and hypopigmentation. Review Manager Version 5.4 and Comprehensive Meta-Analysis v3 software were used for data analysis. The pooled excellent clinical response rate for the 755-nm picosecond alexandrite laser was found to be 36.8% (95% confidence interval [24.4-51.2%], P = .642). Post-inflammatory hyperpigmentation and hypopigmentation were each observed in 20.6% and 12.2% of the cases (P-value < .001), respectively. Post-inflammatory erythema, assessed in 2 studies (n = 23), demonstrated a pooled prevalence of 45.2% (95% confidence interval: 12.9-82.2%). The 755-nm picosecond alexandrite laser demonstrated moderate effectiveness and a favorable safety profile in the treatment of nevus of Ota. Pigmentary complications occurred at relatively low rates across the included studies. Further high-quality prospective and randomized controlled trials are required to confirm long-term effectiveness and safety.
Bilateral nevus of Ota series treated with picosecond laser.
Nevus of Ota (NO) "oculodermal melanocytosis" is prevalent in Asians. Bilateral nevus of Ota (BNO) is a rare condition and comprises 5% of the cases seen. Picosecond laser (PS) is useful in treating various cutaneous benign pigmentary disorders including NO. We analyzed the clinical data on BNO in Vietnamese patients and evaluated the efficacy of treatment of the 1064 nm Nd:YAG picosecond laser (PSNY). Twenty-nine Vietnamese patients (ages 2-67 years, mean 23.36 ± 17.5) with BNO received at least 3 treatment sessions with 1064-nm PSNY (4-5 mm spot size; 1.5-2.4J/cm2, 4-week intervals). Improvement was documented through serial photographs that were taken at baseline (T0), after 4 weeks of the 3rd session (T1), 6th session (T2), 9th session (T3), and more than 10 sessions (T4). Response to the treatment was graded based on a 5-point grading scale. The participants were predominantly female (23 females, 6 males). Sclera, nasal mucosa, and pharyngeal NO lesions were observed in 24 patients (51.7%, 69%, and 6.9%, respectively). Two cases (6.89%) had lesions combined with Port-Wine-Stains. In the majority of cases, more than two branches of the trigeminal nerve were involved (89.66%). Onset of the lesions at <10 years old was 69% and involvement of more than two branches of the trigeminal nerve in these patients was 89.66%. Overall, 100% of patients demonstrated an improvement of the BNO lesions following treatment. A total of 88.9% of patients demonstrated a score compatible with good improvement to complete clearance after 9 treatment sessions on the treatment scoring scale. No severe adverse events or complications were observed. The 1064-nm PSNY laser has the potential to achieve faster clearance for treating dermal pigmentary disorders in Asians. To our knowledge, this is the largest reported series of BNO treated with a laser. Our results suggest that 1064-nm PSNY laser treatment is efficacious and an appropriate therapeutic modality for the treatment of BNO with minimal downtime and minimal adverse side effects.
