A perda auditiva retrococlear é um tipo de perda auditiva caracterizada pelo comprometimento das vias auditivas centrais ou do nervo vestibulococlear, estruturas localizadas após a cóclea. Distingue-se da perda auditiva periférica por afetar especificamente a transmissão neural do sinal sonoro em direção ao tronco encefálico e ao córtex auditivo, preservando geralmente a função coclear. A fisiopatologia decorre da interrupção na condução dos impulsos elétricos gerados na cóclea. Os fatores etiológicos podem envolver: compressão neural, desmielinização, degeneração axonal e dessincronização neural.

Patologia otorrinolaringológica rara caracterizada por perda auditiva súbita de pelo menos 30 decibéis em três frequências contíguas, dentro de 72 horas ou menos, devido a lesão aguda das células ciliadas da cóclea, resultante de trauma acústico ou mecânico durante cirurgia ou ocorrendo sem qualquer causa. A perda auditiva pode afetar um ou ambos os ouvidos. Os sintomas concomitantes típicos são plenitude auricular, zumbidos e vertigens. Os doentes podem recuperar-se totalmente ou permanecer com perda auditiva de intensidade variável.

Impact of contralateral sensorineural hearing loss on prognosis in idiopathic sudden sensorineural hearing loss.

The aim of this study was to investigate the impact of contralateral sensorineural hearing loss on prognosis in idiopathic sudden sensorineural hearing loss. A retrospective analysis was conducted on 445 ISSNHL patients treated between January 2020 and January 2024. The patients were divided into two groups: recovery (234 cases, including complete and partial recovery) and no-recovery (211 cases, no recovery). Pure-tone audiometry was used for evaluation. Clinical characteristics were compared between the groups, and multivariate logistic regression was performed to identify risk factors for poor prognosis. Significant differences were observed between the two groups regarding contralateral hearing level, age, duration of illness, diabetes, hypertension, and audiometric curves. An increase in the hearing threshold of the contralateral ear was positively correlated with the risk of ineffective treatment. Patients with contralateral sensorineural hearing loss prior to the onset of ISSNHL exhibited a 2.757-fold increased risk of treatment failure compared with ISSNHL patients who had normal contralateral ear hearing. Multivariate logistic regression analysis revealed that contralateral sensorineural hearing loss exhibited a significant multiplicative interaction with age over 60 years, hypertension, and a disease duration exceeding 4 days. The presence of contralateral sensorineural hearing loss before the onset of ISSNHL is an independent risk factor for poor prognosis in ISSNHL. It interacts significantly with age over 60 years, hypertension, and disease duration exceeding 4 days. A thorough assessment of contralateral hearing status, in conjunction with factors such as age, hypertension, and disease duration, is essential for developing personalized treatment plans to improve prognosis.

Microcirculatory Dysfunction and Oxidative Stress in Sudden Sensorineural Hearing Loss: Insights From a Case-Control and Experimental Study.

BACKGROUND Sudden sensorineural hearing loss (SSNHL) is an otological emergency with incompletely understood pathophysiology. Evidence suggests microcirculatory dysfunction and oxidative stress contribute to cochlear injury; however, their interrelationship remains insufficiently characterized. MATERIAL AND METHODS This prospective case-control study enrolled 100 patients with SSNHL and 100 age- and sex-matched healthy controls. Perfusion-related surrogate parameters, including cerebral blood flow (CBF), cerebral blood volume (CBV), and mean transit time (MTT), were assessed using dynamic contrast-enhanced magnetic resonance perfusion imaging. Peripheral microcirculation was evaluated by laser Doppler flowmetry. Systemic oxidative stress markers malondialdehyde (MDA), superoxide dismutase (SOD), and glutathione peroxidase (GSH-Px) were quantified in serum. Experimental validation was performed using a rat ischemia-reperfusion model to assess cochlear oxidative stress, apoptosis, and auditory brainstem response thresholds. RESULTS Compared with controls, SSNHL patients exhibited reduced CBF and CBV, prolonged MTT, and lower peripheral microcirculatory perfusion (all P<0.05). Oxidative stress was increased in SSNHL, reflected by elevated serum MDA levels and reduced SOD and GSH-Px activities. Animal model showed ischemia-reperfusion induced parallel alterations, including increased cochlear oxidative stress, reduced antioxidant enzyme activity, increased apoptotic cell burden, and elevated auditory brainstem response thresholds. Perfusion-related parameters correlated with oxidative stress markers. Multivariable regression analysis demonstrated independent associations between perfusion profiles, oxidative imbalance, and hearing thresholds. CONCLUSIONS SSNHL is associated with unfavorable perfusion-related surrogate parameters and heightened oxidative stress accompanied by reduced antioxidant defenses. Clinical and experimental findings suggest microcirculatory dysfunction and oxidative imbalance are interrelated features of SSNHL-associated cochlear injury. Causal inference cannot be established, and further mechanistic studies are warranted.

Clinical impact of adherence to a standardized treatment algorithm for idiopathic sudden sensorineural hearing loss: a multicenter cohort study.

Although systemic corticosteroids remain the mainstay of therapy for idiopathic sudden sensorineural hearing loss (SSNHL), evidence of adherence to standardized treatment algorithms and their impact on hearing outcomes is limited. We conducted a retrospective analysis of a prospectively collected multicenter cohort comprising one university hospital, 16 affiliated hospitals, and two community clinics in Nagano Prefecture, Japan (2018-2024). Consecutive patients diagnosed with SSNHL were treated according to a unified algorithm derived from national epidemiological surveys. Adherence was defined as compliance with the initial and salvage therapies specified in the algorithm, and hearing outcomes were evaluated using the five-frequency pure-tone average (250-4,000 Hz) according to national criteria. Multivariable logistic regression with robust standard errors was used to identify determinants of adherence and factors associated with marked recovery (≥30 dB improvement). The overall adherence rate was 60.3% among 373 patients. Non-adherence was more prevalent in severe cases (Grade ≥3), among older patients (≥65 years), individuals with diabetes, and secondary referrals. Adherent cases showed better final hearing recovery than non-adherent ones (47.7% vs. 31.3%, p < 0.01). In cases of Grade ≥3, intravenous steroid therapy during initial treatment yielded greater early improvement than oral therapy (Grade 3: 46.0% vs. 25.0%, p = 0.018); however, there was no significant difference in final outcomes. Of the 110 patients requiring salvage treatment, intratympanic steroid injection was performed in 51.8% and was independently associated with superior recovery vs. observation (adjusted odds ratio, 14.35; 95% confidence interval, 1.60-128.42; p = 0.017). In this multicenter cohort, adherence to a standardized algorithm was moderate but was associated with improved hearing outcomes. In severe SSNHL, intravenous steroids enhance the early response-most notably in Grade 3-while salvage intratympanic steroid therapy considerably improves the final recovery rate. These findings support the implementation and optimization of the algorithm as well as the refinement of systemic dosing. Additionally, they highlight the importance of structured, guideline-based management in routine practice to improve outcomes in real-world SSNHL care.

Inflammation and Oxidative-Stress Pathways Are Associated with Idiopathic Sudden Hearing Loss: A Genome-Wide Association Study in 15,494 Japanese Individuals.

The etiology of idiopathic sudden sensorineural hearing loss (iSSNHL) remains unclear, and genome-wide genetic evidence is limited. We conducted a multicenter Japanese case-control genome-wide association study including 192 clinically defined iSSNHL cases and 15,302 controls aged ≥80 years without a history of hearing loss. After cross-platform SNP harmonization and imputation (Eagle/Minimac4), association testing was performed using dosage-based logistic regression in PLINK 2.0, adjusting for sex and principal components (PC1-PC10). Gene- and pathway-level analyses were conducted using MAGMA and the PANTHER overrepresentation test. Genomic inflation was modest (λ_GC = 1.04). Eight loci reached genome-wide significance (p < 5 × 10-8), led by FHIT, with additional loci near LHX2, TRMT1L, MEGF10, SPATS1, SAMD5, MYT1L, and ID4; 21 loci met the suggestive threshold (p < 1 × 10-6). MAGMA identified eight genes at FDR < 0.05 (FHIT, TRMT1L, MEGF10, RNF2, SWT1, VAMP1, TAPBPL, and C9orf3). These findings suggest that immune-inflammatory and cellular stress-homeostasis mechanisms may contribute to iSSNHL susceptibility and provide candidate loci for future replication and functional studies.

Efficacy evaluation and clinical value exploration of secondary inpatient treatment for total deafness-type SSNHL: a single-center prospective study.

Total deafness-type sudden sensorineural hearing loss (SSNHL) represents one of the most challenging subtypes of SSNHL due to its poor response to initial therapy and uncertain prognosis. Secondary inpatient treatment has been proposed as a potential salvage strategy; however, its efficacy and predictors of favorable outcomes remain poorly defined. This study included 120 patients with unilateral total deafness-type SSNHL, divided into secondary treatment and control groups. Hearing thresholds at low, middle, high, and full frequencies, pure-tone average (PTA) at speech frequencies, and speech recognition rate were evaluated across six time points (T1-T6). Tinnitus Handicap Inventory (THI) scores and improvement rates were also analyzed. Univariate and multivariate logistic regression analyses were performed to identify independent predictors of marked hearing recovery. A nomogram was constructed to predict the hearing prognosis of patients with SSNHL. Compared with the control group, the secondary treatment group exhibited significantly earlier onset and greater magnitude of improvements in hearing thresholds and speech recognition rate (all p < 0.05), with distinct frequency-specific patterns. Recovery initiated at 4-8 weeks and stabilized after 12 weeks, while the control group showed delayed improvement. Tinnitus relief occurred earlier in the secondary treatment group. Multivariate analysis identified age ≤50 years, disease duration ≤3 days, absence of vertigo, and normal vestibular function (vHIT and caloric test) as independent predictors of marked recovery (all p < 0.05). The area under the receiver operating characteristic (ROC) curve was 0.876 (95% confidence interval [CI]: 0.762-0.989). The calibration curve showed good agreement with the standard curve. The decision curve analysis demonstrated that the prediction model yielded positive net benefits across nearly all threshold probability ranges. Secondary inpatient treatment offers a significant auditory benefit for patients with total deafness-type SSNHL by accelerating and amplifying recovery. Young age, early intervention, and well-preserved vestibular function are key determinants of a favorable prognosis. The predictive model constructed hereby can effectively predict the prognosis of patients.
