Predictors of mortality in CNS tuberculosis: clinical and radiological parameters.
Central nervous system tuberculosis (CNS-TB) is a rare but highly lethal form of TB with heterogeneous clinical and radiological features. We evaluated objective clinical grading, CSF indices, and MRI findings associated with in-hospital mortality. In this single-center retrospective cohort (2010-2023), all consecutive adults with CNS-TB were included (n = 15). Patients were grouped as survivors (n = 6) and non-survivors (n = 9). We extracted BMRC stage, baseline GCS, symptom-to-presentation and symptom-to-treatment durations, CSF cyto-biochemistry and microbiology (culture/PCR), and a standardized MRI checklist (basal exudates, hydrocephalus, ventriculitis, infarcts, tuberculoma/abscess, cranial nerve enhancement, spinal involvement). Treatment protocols (ATT, dexamethasone indications, neurosurgery) were documented. Results are primarily reported as counts/proportions with exploratory effect sizes. The mortality rate was 60%. Non-survivors were significantly older (57.8 ± 22.8 vs. 29.2 ± 6.9 years; P = 0.029, d = 1.56) and had higher serum AST levels (28.9 ± 6.4 vs. 15.0 ± 2.9 U/L; P = 0.007, d = 2.14). CSF leukocyte count was also significantly elevated in non survivors (P = 0.041, d = 0.95). Cerebral ischemia was present only in non-survivors (56% vs. 0%; P = 0.04, h = 0.98). PCR positivity in non-CSF samples (0% vs. 78%) was significantly associated with mortality (P = 0.007). Other parameters showing moderate to large effect sizes included focal neurological deficits, ventriculitis, and increased CSF opening pressure. Advanced age, elevated AST levels, increased CSF leukocyte count, cerebral ischemia, and extraparenchymal PCR positivity emerged as major predictors of mortality in CNS-TB. These factors may aid early risk stratification and treatment planning. Larger prospective studies are warranted to validate these findings.
Inflammation-induced Neuronal Damage in CNS Tuberculosis: Molecular Mechanism and Therapeutic Targets.
Central nervous system tuberculosis (CNS-TB), most frequently manifesting as tuberculous meningitis, is associated with high mortality and significant long-term neurological morbidity. Increasing evidence suggests that disease severity and neurological damage are driven largely by dysregulated host neuroinflammatory responses rather than direct Mycobacterium tuberculosis-mediated cytotoxicity. However, the mechanistic links between glial activation, inflammatory signaling, and neuronal injury remain incompletely defined. A comprehensive literature review was conducted using PubMed, Scopus, and Web of Science databases to identify experimental, clinical, and translational studies investigating neuroimmune mechanisms in CNS-TB. Studies focusing on glial activation, cytokine signaling, oxidative stress, excitotoxicity, mitochondrial dysfunction, and neuronal death were included. Recent advances in single-cell transcriptomics, immunometabolism, and host-directed therapeutic strategies were also analyzed and integrated. The reviewed evidence indicates that CNS invasion by M. tuberculosis leads to sustained activation of microglia and astrocytes, resulting in excessive production of pro-inflammatory cytokines such as TNF-α, IL-1β, and IL-6. This inflammatory milieu disrupts blood-brain barrier (BBB) integrity, promotes leukocyte infiltration, and induces oxidative and nitrosative stress. Astrocyte dysfunction further contributes to excitotoxicity through impaired glutamate clearance. These converging inflammatory, oxidative, and excitotoxic pathways drive mitochondrial dysfunction, synaptic impairment, and activation of regulated neuronal cell death pathways, culminating in neurodegeneration. CNS-TB-associated neuronal injury arises primarily from maladaptive host neuroimmune responses rather than direct mycobacterial effects. A unifying framework centered on glial-driven inflammation and mitochondrial dysfunction provides critical insight into disease pathogenesis. Targeting these convergent pathways through host-directed therapies, alongside antimicrobial treatment, represents a promising strategy to mitigate neuroinflammation and improve long-term neurological outcomes in CNS tuberculosis.
I saw a different "bunch of grapes" sign in two patients with neurotuberculosis.
Neurotuberculosis is one of the most severe forms of tuberculosis, often posing diagnostic challenges due to its variable imaging presentation. We report two cases demonstrating the characteristic "bunch of grapes" sign on brain magnetic resonance imaging (MRI), representing coalescent ring-enhancing tuberculomas. The first patient was a 44-year-old woman presenting with fever and aphasia, while the second was a 28-year-old HIV-positive woman on irregular antiretroviral therapy with a history of impaired gait, dizziness and nausea. MRI revealed multiple T2-hypointense nodular lesions with ring-like gadolinium enhancement forming grape-like clusters. Stereotaxic biopsy confirmed central nervous system tuberculosis in both cases. Standard antituberculous therapy combined with corticosteroids led to progressive clinical improvement. Recognition of the "bunch of grapes" appearance can facilitate early diagnosis of neurotuberculosis and guide appropriate treatment, particularly in endemic regions where overlapping imaging findings with other infectious or neoplastic diseases may occur.
Role of Low-Dose Infliximab for Inflammatory Complications of Central Nervous System Tuberculosis: A Retrospective Cohort Study.
We evaluated low-dose infliximab (5 mg/kg) as adjunctive therapy in 20 patients with severe central nervous system tuberculosis. At 3 months, 60% achieved disability-free survival (modified Rankin Scale ≤ 2), and 75% showed clinically meaningful improvement. Our outcomes matched reports of high-dose infliximab. Randomized trials to optimize dosing are needed.
Case Report: Immune reconstitution-related neurological deterioration in advanced HIV infection with multiple opportunistic infections: a diagnostic challenge.
Immune reconstitution inflammatory syndrome (IRIS) is a serious complication following antiretroviral therapy (ART) initiation in patients with advanced HIV infection, particularly when the central nervous system is involved and multiple opportunistic infections coexist. We report the case of a 26-year-old man with newly diagnosed advanced HIV infection who developed rapid neurological deterioration shortly after ART initiation during hospitalization. Neuroimaging revealed a necrotic central nervous system mass lesion with extensive edema and mass effect. Serological testing demonstrated prior exposure to Toxoplasma gondii and active Treponema pallidum infection. Despite broad antimicrobial therapy, corticosteroids, and supportive care, the patient experienced progressive clinical deterioration and died. Retrospective reassessment of the clinical course, imaging findings, epidemiological context, and treatment response suggested an IRIS-related inflammatory process, with central nervous system tuberculosis-associated IRIS representing the most plausible underlying mechanism, while toxoplasmosis and syphilis were considered potential concomitant or confounding conditions. This case underscores the diagnostic complexity of IRIS in advanced HIV infection and highlights the importance of a cautious, probabilistic, and evidence-based approach to avoid etiologic misclassification in severe neurological presentations.
