Trends in net survival for patients with brain tumors in Japan, 2000-14 (CONCORD-3).
Brain tumors are a serious health issue, and survival is an effective indicator of how the healthcare system manages them. We examined net survival trends for patients diagnosed with a primary brain tumor during 2000-2014 in Japan, using data from 16 regional population-based cancer registries participating in the CONCORD-3 study. We included patients aged 15-99 years, with follow-up completed either 5 years after diagnosis or up to 31 December, 2014. We estimated 5-year net survival by age group and morphological subtype using the Pohar Perme estimator, and age-standardized with International Cancer Survival Standard weights. Five-year net survival for brain tumors increased from 27.9% for patients diagnosed during 2000-2004 to 46.3% in 2010-2014. During 2010-2014, 5-year net survival for astrocytic tumors was 22.3%, but ~90% for neuronal and mixed neuro-glial tumors, germ cell tumors, and other specified tumors. Five-year net survival reached 62.1% for oligodendroglial and oligoastrocytic tumors. Five-year net survival for brain tumors in Japan improved during 2000-2014, with remarkable variations by morphologic type. Survival improved for all age groups, and particularly for younger patients, highlighting the need for enhanced survivorship care. Japan's focus on comprehensive data collection and supportive care for longer-term survivors remains key for further progress. When comparedinternationally, Japan's gains sit at the higher end of reported survival ranges, underscoring how robust registry infrastructure and equitable access to care could inform strategies for brain tumor care.
Stereotactic intracranial implantation of patient-derived glioblastoma cells in rats: A xenograft modeling approach.
Glioblastoma (GBM) is an aggressive primary brain cancer with a poor prognosis. Preclinical animal models are essential for studying GBM pathophysiology and therapy; however, existing models often fail to fully capture the tumor's heterogeneity and the partially immunodeficient microenvironment critical to its progression. We developed a novel GBM model using the stereotactic implantation of primary patient-derived GBM cells from various donors into the brains of rats subjected to transient, partial immune suppression. This model combines the biological heterogeneity of patient-derived cells with the anatomical advantages of a larger rodent brain, improving spatial tumor localization. The use of a mixed primary cell population better recapitulates human GBM heterogeneity. Furthermore, the model's partially preserved immune environment allows for the investigation of tumor-immune interactions. Unlike fully immunodeficient models, our approach maintains a more physiologically relevant, partially intact immune system. Compared to murine models, the rat brain offers superior spatial resolution for tumor analysis and intervention. This method provides a reliable and translational platform that enhances the fidelity of preclinical GBM research. It offers an improved tool for drug evaluation and the development of personalized therapeutic strategies by more accurately mimicking the complex and heterogeneous nature of human GBM.
A functionally relevant model for interrogating brain tumor-endothelial cell interactions.
Intercellular interactions, particularly those between tumor cells and the surrounding vasculature, are central to the biology of the tumor microenvironment. Approaches for studying these interactions often rely on limited patient samples or time- and resource-intensive xenograft tissues in combination with histological or single-cell omics profiling. While informative, these models capture only static snapshots and limit mechanistic interrogation. Studying the mechanisms behind these interactions requires viable co-culture models for culturing different cell types together in vitro, while preserving the phenotypic integrity of each cell type. To achieve this, we developed and validated optimal in vitro culture conditions to support the co-culture of human Group 3 medulloblastoma (G3 MB) cells and microvascular brain endothelial cells (BECs) as an ideal screening model for mechanistic and interventional studies. Supported by a new optimized 1:1 mixed media formulation, this model preserves native cellular morphology and phenotypic characteristics. When cultured alone in the new optimized media, G3 MB cells retained expression of stemness markers (SOX2 & OTX2), self-renewal capacity, and undifferentiated morphology, while BECs retained tight junction formation and migratory behavior. This co-culture platform permits real-time, dynamic, and mechanistic studies of tumor-endothelial cell interactions, overcoming the limitations of fixed-tissue analyses and facilitating precise experimental manipulation. This well-characterized model provides a physiologically and functionally relevant platform for further dissecting the reciprocal interactions present between various brain cancer cells and vascular endothelial cells, supporting the development of targeted therapeutic strategies and advancing our understanding of brain tumor biology.
Vaccine immunotherapy for glioblastoma: How can previous attempts guide us?
Yearly, glioblastoma (GBM) is the cause of death of thousands of people in the US. It is paradoxically one of the most lethal and frequent central nervous system malignancies, occupying a unique position in cancer research. Among various attempted management strategies, vaccine immunotherapy has undergone multiple investigations, with heterogenous results. A systematic review was undertaken on two databases to identify studies evaluating vaccine immunotherapy survival response in glioblastoma or high-grade malignant glioma. A subgroup meta-analysis of overall survival hazard ratios, a meta-regression and multi-trial Kaplan-Meier curves were delineated via reconstructed survival data. 537 studies were found, and 36 were eligible for inclusion. Out of these, 12 had comparable arms for meta-analysis. There was a 49 % [26 %; 64 %] death hazard reduction in the pooled analysis. Only the dendritic cell-based vaccines showed benefit when groups were tested separately. The multi-trial curve suggested a disparity among different approaches. For newly diagnosed GBM, the median overall survival was 18.9 months, ranging from 15.36 to 31.46 months. For recurrent GBM or mixed trials, the median overall survival was 13.9 months, ranging from 9.71 to 15.53 months. Vaccine immunotherapy for GBM appears to depend on dendritic cells to achieve significant improvements in survival. The incorporation of innate immunity modulators such as GM-CSF and Poly-ICLC, along with stratification by MGMT-methylation, are promising. Although an expressive HR reduction was achieved, there are still primary scientific validity concerns, the residual hazard leads to a reserved prognosis and should be addressed by multimodal treatment approaches.
The Endoscopic ExtraForaminal TransChoroid (EFTC) Approach in the Management of Pineal Tumors: Gross Total Resection through a Single Burr Hole.
Pineal region tumors pose a significant surgical challenge due to their deep location and proximity to critical neurovascular structures. Endoscopic techniques, particularly the transchoroid approach, have expanded the possibilities for minimally invasive access to the posterior third ventricle. In select cases, this route may not only allow simultaneous biopsy and third ventriculostomy but also meaningful tumour resection while limiting morbidity. We report the case of an 11-year-old male presenting with headache, drowsiness, vomiting and diplopia. Imaging demonstrated a heterogeneous pineal mass with features suggestive of teratoma. Serum markers revealed markedly elevated β-HCG with normal AFP. The radiological picture coupled with the tumour markers was consistent with a mixed malignant germ cell tumour. An endoscopic third ventriculostomy (ETV) and endoscopic extraforaminal transchoroid (EFTC) approach were planned through a single burr hole with the aim of cerebrospinal fluid (CSF) diversion, providing a tissue sample and debulking if possible and appropriate. The identification of mature teratomatous components on smear and a favorable dissection plane enabled and justified gross total resection through the single burr hole. Histopathology confirmed mature and immature teratomatous tissue with a 5-10% germinomatous component. The postoperative course was uneventful, and the patient completed chemotherapy followed by proton beam radiotherapy, remaining recurrence-free at the latest follow-up of over two years. This case represents the first reported gross total resection (GTR) of a pineal tumour achieved via an endoscopic EFTC approach and highlights its potential as a safe and effective minimally invasive route for lesions in the posterior third ventricle, enabling complete resection in select patients.
