Existem dois tipos principais: pancreatite aguda e pancreatite crónica. Os sinais e sintomas de pancreatite mais comuns são dor na parte superior do abdómen, náuseas e vómitos. Em muitos casos a dor espalha-se para as costas e é geralmente muito intensa. Na pancreatite aguda, pode ocorrer febre e os sintomas geralmente desaparecem ao fim de alguns dias.

Clinicopathological features of localized-type IgG4-related cholecystitis: a nationwide multicenter study in Japan.
Immunoglobulin G4 (IgG4)-related cholecystitis represents gallbladder involvement in IgG4-related disease (IgG4-RD). However, localized-type IgG4-related cholecystitis is rare and remains poorly characterized. We conducted a nationwide, multicenter study to clarify the clinicopathological features of localized-type IgG4-related cholecystitis in Japan. We collected clinical information, imaging data, and tissue slides from suspected cases of IgG4-related cholecystitis for central review, and evaluated their clinicopathological features. Fifteen patients were diagnosed with localized-type IgG4-related cholecystitis. The incidence of localized-type IgG4-related cholecystitis was 0.59% (15/2560) among all IgG4-related gastroenterological diseases across 30 institutions. The median age was 62 years (range, 54-70 years), and 10 patients were men. Elevated serum IgG4 levels and other IgG4-RD lesions were observed in all 15 patients. Imaging findings demonstrated subepithelial lesions (SELs) of the gallbladder in all patients, and cystic lesions within SELs suggestive of adenomyomatosis (ADM) in nine patients. Among the 11 patients with available histopathological analyses, two distinct types of mass-forming lesions (inflammatory pseudotumors) characterized by storiform fibrosis and obliterative phlebitis were identified: ADM-type in six cases and mural hypertrophy-type in three cases. Eleven patients underwent surgery and four received glucocorticoids therapy. Within 2-4 weeks of initiating glucocorticoid therapy, complete resolution was achieved in one patient, and partial improvement in three patients. Localized-type IgG4-related cholecystitis is characterized by imaging findings of SELs. It shares histopathological features of IgG4-RD, and often has unique lesions centered around ADM. It responds to glucocorticoid therapy.
Multicenter Validation Study of the Clinical Diagnostic Criteria for IgG4-Related Sclerosing Cholangitis 2020 in Japan.
The diagnostic performance of clinical diagnostic criteria for IgG4-related sclerosing cholangitis 2020 (IgG4-SC2020) has not been fully validated since its proposal as a revision of the 2012 criteria (IgG4-SC2012). We conducted a multicenter validation study to evaluate the diagnostic performance of IgG4-SC2020 using clinical data collected from 1034 patients with IgG4-SC and 447 patients with mimickers, including 143 with pancreatic cancer, 157 with primary sclerosing cholangitis, and 147 with cholangiocarcinoma in Japan. The sensitivity of IgG4-SC2020 was significantly higher than that of IgG4-SC2012 (99.0% vs. 89.1%; p < 0.001). The specificities of both IgG4-SC2020 and IgG4-SC2012 were 100% for pancreatic cancer and cholangiocarcinoma. For primary sclerosing cholangitis, the specificities of IgG4-SC2020 and IgG4-SC2012 were 97.5% and 100%, respectively, with no significant difference (p = 0.123). A total of 113 patients who could not be diagnosed according to the IgG4-SC2012 were successfully diagnosed using IgG4-SC2020. These diagnostic improvements were attributed to the inclusion of MRCP findings (n = 97), the absence of neoplastic cells on histology (n = 15), and the presence of IgG4-related kidney lesions (n = 1). This Japanese multicenter validation study demonstrated that the diagnostic performance of IgG4-SC2020 was superior to that of IgG4-SC2012. UMIN Clinical Trials Registry (UMIN-CTR), UMIN000052984.
Clinical features, responses to therapy and prognosis of 107 patients with IgG4-related cholangitis: a retrospective study.
IgG4-related cholangitis (IRC) is an important hepatobiliary manifestation of IgG4-related diseases (IRD), frequently accompanied by other organ involvement of IRD especially autoimmune pancreatitis (AIP). Diagnosis of IRC requires comprehensive evaluation. The patients respond well to glucocorticosteroids and have good long-term prognosis regardless of relapse. This retrospective observational study described the clinical features, responses to therapy and prognosis of patients with IRC. The aim of our study was to describe and analyze the clinical features, responses to treatment and prognosis of 107 patients with IRC at our hospital, exploring the difficulties and clinical experience in the diagnosis and treatment of IRC. We conducted a retrospective study of 107 patients with IRC (88 males and 19 females; mean age, 60.8 years) treated at Peking Union Medical College Hospital between January 2013 and April 2023. Data on demographic, clinical, serologic, pathologic and imaging characteristics as well as treatment responses and prognosis were collected and analyzed. Statistical Package for Social Sciences software (SPSS, version 25.0) and Python (version 3.11) were used for the data analysis. Survival was assessed using Kaplan–Meier curves. Univariate and multivariate logistic regression analysis was used to analyze risk factors of relapse. A two-sided probability P < 0.05 was considered statistically significant. Receiver operating characteristic (ROC) curve was used to predict the value of steroid maintenance duration for relapse. The most common symptoms at presentation were jaundice (76.6%) and weight loss (72.9%). AIP was the most prevalent comorbidity (96.3%). Serum IgG4 levels were elevated in 99% of the patients, and isolated stenosis of the lower part of the common bile duct (type 1 IRC) was the most common imaging finding (49.5%). Nearly 90% of the patients were treated with steroids, and all presented with an excellent response. A relapse rate of 37.5% was observed during the follow-up period, most of which occurred after steroid withdrawal (47.2%). Steroid maintenance duration was the significant independent risk factor of relapse in patients with IRC (P = 0.001) (odds ratio, 0.875; 95% CI, 0.811–0.944). The patients were followed up for a median of 63.8 months. Four patients (4.0%) died, and none of them had cholangiocarcinoma. Overall 5-year and 10-year survival rates were 97.0% and 96.0%, respectively. The disease-specific survival rate was 100%. Among the 107 patients examined, IRC was found to be benign and exhibited an excellent response to steroid treatment. No patient progressed to cholangiocarcinoma, and relapse had no impact on the overall survival rate. The online version contains supplementary material available at 10.1186/s12876-025-04598-0.
Metachronic development of cholangiocarcinoma during treatment for IgG4-related sclerosing cholangitis.
We report a case of obstructive jaundice due to recurrent distal biliary stricture during 3 years of treatment for immunoglobulin G4 (IgG4)-related sclerosing cholangitis (IgG4-SC) associated with autoimmune pancreatitis. Although a relapse of IgG4-SC was initially suspected, imaging findings, laboratory tests, and histopathological examinations led to the diagnosis of metachronous cholangiocarcinoma. The patient underwent pancreaticoduodenectomy, and no cancer recurrence was noted 6 months postoperatively. Distal cholangiocarcinoma and IgG4-SC remission were observed in the resected specimen. In patients with recurrent biliary strictures during IgG4-SC treatment, comprehensive evaluations are essential because of the risk of disease relapse and development of metachronous cholangiocarcinoma.
Peripancreatic Vascular Involvement and Gastric Varices in Autoimmune Pancreatitis: A Multicenter Retrospective Study.
Autoimmune pancreatitis (AIP) could induce peripancreatic vascular involvement, such as portal system stenosis, through inflammatory extension, resulting in the development of collateral vessels. In severe cases, gastric varices may develop, leading to a life-threatening risk of variceal rupture. However, these complications remain underrecognized and are not explicitly addressed in the current clinical guidelines. This study aimed to evaluate the frequency of vascular involvement in AIP, assess therapeutic effects of glucocorticoids, and identify key vessels associated with gastric varices. This multicenter retrospective study included 230 patients diagnosed with AIP at 17 institutions in Japan between 2010 and 2021. We analyzed clinical data, contrast-enhanced computed tomography images, and endoscopy findings. All images were evaluated individually by 3 professionals at a central facility. The primary outcome was the frequency of peripancreatic vascular involvement. At diagnosis, 51.3% of the patients exhibited peripancreatic vascular involvement, primarily in the splenic vein (48.3%), superior mesenteric vein (23.9%), and portal vein (9.6%). Collateral vessel formation was observed in 40.0% of patients, with 24.3% being severe; 2 gastric variceal ruptures occurred prior to therapy. Glucocorticoid therapy significantly improved venous stenosis (response rates: 95.6%, 93.8%, and 93.1% in the splenic, portal, and superior mesenteric veins, respectively). Additionally, 80.9% of patients exhibited collateral vessel improvement. Among 55 patients who underwent upper endoscopy, 10.9% had gastric varices strongly associated with severe splenic vein stenosis. Peripancreatic vascular involvement represents a common complication of AIP and significantly responds to glucocorticoid therapy, underscoring the importance of careful monitoring to prevent potentially fatal outcomes.
