Endoftalmite, ou endoftalmia, é uma inflamação da cavidade interna do olho, geralmente causada por uma infecção. É uma possível complicação de todas as cirurgias intraoculares, particularmente da cirurgia de catarata, e pode resultar em perda de visão ou perda do próprio olho. A infecção pode ser causada por bactérias ou fungos e é classificada como exógena ou endógena. Outras causas não infecciosas incluem toxinas, reações alérgicas e corpos estranhos intraoculares retidos. Injeções intravítreas são uma causa rara, com uma taxa de incidência geralmente inferior a 0,05%.

Pathogen-Associated Visual Outcomes Following Postprocedure Endophthalmitis.
To report the anatomic and functional outcomes of a large, multicenter series of postprocedure acute endophthalmitis, stratified by pathogen virulence. Retrospective, interventional case series. Two hundred forty adult patients diagnosed with postprocedural acute endophthalmitis from 2012 to 2022 at four tertiary, university-based care centers. Of these, 208 patients had adequate follow-up data for comparative analyses. Medical records were reviewed for demographics, microbiology, treatment, and visual outcomes of patients that developed endophthalmitis following any intraocular procedure. Organisms were categorized into surface commensals (coagulase-negative Staphylococcus sp.) and nonsurface commensals (eg, Streptococcus, Enterococcus sp.) as a proxy for virulence. Visual acuity (VA) outcomes and complication rates were compared across groups using Fisher's exact test and regression models adjusting for age, sex (male or female), comorbidities, and treatment modality for postinjection endophthalmitis. Final VA, number of interventions, and rates of vision-threatening complications, including phthisis, retinal detachment, enucleation/evisceration. Among culture-positive cases, patients infected with nonsurface commensal organisms had significantly worse final VA than those with coagulase-negative Staphylococcus (mean logMAR 2.14 vs 0.77, P < .0001), as well as higher rates of third interventions (27.5% vs 10.1%, P = .017), phthisis (15.4% vs 1.1%, P = .0032), and enucleation/evisceration (12.8% vs 0%, P = .0022). In multivariable regression of postinjection endophthalmitis, virulent, nonsurface commensal organisms remained independently associated with poorer visual outcomes (P = .0005). Complication patterns were consistent whether culture-negative cases were included or not in the nonvirulent group. Most patients were initially treated with intravitreal injections (92.1%). An initial vitrectomy was uncommon and not significantly associated with better outcomes when performed. Infections with virulent, nonsurface commensal organisms such as Streptococcus and Enterococcus are associated with significantly worse outcomes in postprocedure endophthalmitis, independent of presenting vision or treatment delay. These findings support a shift toward pathogen-informed management and underscore the need for timely identification of virulent organisms in postprocedural acute endophthalmitis.

Visual Outcomes of Early Intravitreal Antibiotics and Factors Affecting Presumed Acute Endophthalmitis Post Cataract Surgery in Tertiary Eye Hospital: An Observational Study.
Acute pofl-cataract infectious endophthalmitis is a rare yet serious complicationthat can lead to significant intraocular inflammation and potential loss of vision following cataract surgery. Early diagnosis and prompt treatment are essential to prevent irreversible damage to photoreceptors. While the Early Vitrectomy Study (EVS) provided initial treatment guidelines, clinical approaches have since evolved. In this observational, retrospective single-center study at Biratnagar Eye Hospital, we analyzed data from patients diagnosed with presumed endophthalmitis within six weeks of cataract surgery in 2023. All patients received intravitreal antibiotics (vancomycin and ceftazidime) and underwent tap biopsy for microbiological analysis. Data included demographics, clinical features, treatment (intravitreal antibiotics ± vitrectomy), and visual acuity (LogMAR) at baseline and 60 days. The data were recorded in Excel and analyzed using Statical Package for the Social Sciences version 29.0 (IBM Corp., Armonk, NY, USA). The study found that 44 (87.70%) were between 40-80 years old, with males 28 (57.14%). Symptoms appeared primarily within the firfl two weeks pofl-surgery in 31 (62%) of cases. Phacoemulsification 24 (48%) and Small Incision Cataract Surgery 26 (52%) were represented in the patient population. Out of total patients,44 (88%) of the patients received only intravitreal antibiotics. A notable improvement was observed in visual acuity with mean Log MAR values decreasing from 1.395± 0.632 on Day 1 (Snellen equivalent 6/120) to 0.441±0.553 on Day 60 (Snellen equivalent 6/15). No notable variations in visual acuity was observed in age, gender, surgical technique (phacoemulsification vs. SICS), or adjunctive dexamethasone. Findings suggefled that early therapeutic intervention with intravitreal antibiotics in acute pofloperative endophthalmitis may improve visual outcomes when evaluated at 60 days pofl-treatment.
Endogenous Endophthalmitis Secondary to Prostate Abscess.
Endophthalmitis is a severe ophthalmic emergency that necessitates immediate intervention. Most cases are exogenous, originating from eye surgeries, intravitreal injections, or penetrating injuries. Endogenous endophthalmitis occurs when the infection is spread hematogenously, usually as a result of bacteremia. We present a healthy individual with no predisposing factors who presented to the emergency department with decreased left vision. A male in his late sixties with a medical history of hypertension and hyperlipidemia presented to the ophthalmology clinic with complaints of left eye pain, redness, and decreased vision. Diagnosis of endogenous endophthalmitis was made through fundoscopy, and the patient received intravitreal injections of vancomycin and ceftazidime in the left eye. He was subsequently admitted to the hospital, where blood and urine cultures grew methicillin-resistant Staphylococcus aureus (MRSA). Upon further investigation, the patient reported experiencing obstructive urinary symptoms, prompting the initiation of intravenous (IV) vancomycin and linezolid. CT of the abdomen and pelvis revealed multiple prostatic abscesses. During treatment, the patient's vision gradually improved, and he was discharged with IV antibiotics. This case emphasizes the importance of prompt diagnosis and treatment in managing this ophthalmologic emergency. A thorough review of systems was essential in identifying the source of bacteremia, enabling timely intervention. We describe the role of magnetic resonance imaging in diagnosing endophthalmitis and highlight the treatment modalities.
