Diagnostic Accuracy of a CRISPR-Based Assay in Smear- and Culture-Negative Fungal Keratitis.
Diagnosing fungal keratitis (FK) in patients with negative smear and culture results remains clinically challenging, highlighting the need for alternative diagnostic approaches. To determine the diagnostic accuracy of the clustered regularly interspaced short palindromic repeats (CRISPR)-based Rapid Identification of Mycoses using CRISPR (RID-MyC) assay for detecting FK in patients with negative smear and culture results using in vivo confocal microscopy (IVCM) as the reference standard. This prospective diagnostic accuracy study was conducted from December 2024 to March 2025 at Aravind Eye Hospital, a tertiary ophthalmology referral hospital in Coimbatore, India. Consecutive patients clinically suspected to have microbial keratitis with negative smear and culture results were eligible for inclusion. Data were analyzed from March 2025 to June 2025. All included participants underwent corneal scraping for RID-MyC assay and imaging by IVCM. The primary outcomes were sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic concordance of the RID-MyC assay compared with IVCM results. Of 245 consecutive patients clinically suspected to have microbial keratitis, 82 were smear negative. After exclusions due to contraindications or positive subsequent cultures, 41 patients with smear- and culture-negative results were ultimately included in the final analysis. Of these 41 patients (mean [SD] age, 51.0 [14.6] years; 21 [51.2%] women), RID-MyC demonstrated sensitivity of 82.1% (95% CI, 63%-94%) and specificity of 76.9% (95% CI, 46%-95%). Positive predictive value was 88.5% (95% CI, 74%-95%) and negative predictive value was 66.7% (95% CI, 46%-82%). Concordance between RID-MyC and IVCM was observed in 33 cases (80.5%). Notably, prior antifungal treatment was most frequent (4 of 5 [80%]) among patients with positive IVCM but negative RID-MyC results. Conversely, all patients (3 of 3 [100%]) with negative IVCM but positive RID-MyC findings had smaller, peripheral, or paracentral lesions. In this diagnostic study, in patients with smear- and culture-negative FK, the RID-MyC assay showed good diagnostic accuracy comparable with IVCM and was feasible in all cases, including those in whom imaging was not possible. With its rapid turnaround and minimal equipment needs, RID-MyC may serve as a practical adjunct to conventional diagnostics, particularly in high-burden, resource-limited settings where IVCM is unavailable or contraindicated.
Detection of Endothelial Plaque in Microbial Keratitis Using Anterior Segment Optical Coherence Tomography.
To evaluate the intra- and inter-grader concordance of anterior segment optical coherence tomography (ASOCT) grading for detection of endothelial plaque in microbial keratitis, and to compare endothelial plaque detection via ASOCT grading versus in-person slit lamp examination. Diagnostic concordance study of 150 consecutive patients with microbiologically confirmed bacterial or fungal keratitis at a high-volume tertiary eye hospital in India. Two masked ophthalmologist graders independently evaluated ASOCT images for presence of two morphologically distinct endothelial plaque subtypes noted during image review (round and flat plaques). We assessed intra-grader and inter-grader concordance for each endothelial plaque morphology and for presence of either morphology. Diagnostic agreement between ASOCT and in-person slit lamp examination was evaluated using percent agreement, Cohen's kappa, sensitivity, and specificity. Univariable and multivariable logistic regression was used to assess odds of disagreement between ASOCT and slit lamp examination for endothelial plaque detection. ASOCT detection showed near perfect inter-grader agreement for round endothelial plaques (kappa 0.88, 94.7% agreement), flat endothelial plaques (kappa 0.84, 92.0% agreement), and either plaque (kappa 0.88, 94.0% agreement). Intra-grader agreement was substantial to near perfect for both graders across all plaque types (kappa 0.70-0.86). Ophthalmologist slit lamp examination identified endothelial plaque in 6.0% eyes, while ASOCT detected round plaques in 32.7%, flat plaques in 43.3%, and either plaque in 55.3% of eyes. Using ASOCT as reference, slit lamp examination demonstrated sensitivity of 16.3% for round plaques, 6.2% for flat plaques, and 9.6% for either plaque, with specificity exceeding 94% for all. Poor visual acuity (logMAR ≥1.0) was associated with increased disagreement for round plaques (adjusted OR 5.04), flat plaques (adjusted OR 3.63), and either plaque (adjusted OR 3.98). Bacterial infection was associated with increased disagreement for any endothelial plaque (adjusted OR 4.56). Slit lamp examination substantially under-detects endothelial plaque compared to ASOCT, while ASOCT enables reproducible detection with excellent intra- and inter-grader agreement. These findings support incorporation of ASOCT imaging into microbial keratitis evaluation protocols. Differences in round and flat endothelial plaque morphologies warrant further investigation.
Evaluating Photochemical Interactions Between Rose Bengal Photodynamic Antimicrobial Therapy and Topical Antifungals in Fungal Keratitis Isolates.
The purpose of this study was to evaluate the in vitro effects of combining rose bengal (RB) photodynamic antimicrobial therapy (PDAT) with conventional topical antifungals (amphotericin B, natamycin, and voriconazole) on clinical Fusarium and Aspergillus isolates and to determine the antifungal stability under experimental conditions. Clinical isolates of Fusarium spp. (n = 3) and Aspergillus spp. (n = 3) were cultured using standard microbiology techniques. Agar plates were prepared with three concentrations of amphotericin B, natamycin, or voriconazole along with plates containing no antifungals. The following groups were tested in both dark and light conditions: (1) antifungal monotherapy (antifungal only), (2) RB monotherapy (RB only), and (3) antifungal and RB combination therapy. Plates were incubated for 7 days and percent growth inhibition was quantified. UV-visible spectroscopy and mass spectrometry was performed to assess antifungal stability. Overall, Fusarium isolates were more susceptible to RB-PDAT than Aspergillus isolates. RB-PDAT combination therapy with natamycin reduced growth inhibition, whereas combination therapy with voriconazole increased growth inhibition compared to antifungal monotherapy. UV-visible spectroscopy demonstrated 70% degradation of amphotericin B and full degradation of natamycin when combined with RB-PDAT, whereas voriconazole remained stable. Across both Fusarium and Aspergillus isolates, treatment responses to antifungals with or without RB-PDAT demonstrated strain-specific effects. The increased fungal inhibition and maintained stability of voriconazole when combined with RB-PDAT highlight its potential compatibility with RB-PDAT. In contrast, the reduced inhibition and degradation of natamycin and amphotericin B with RB-PDAT suggests that drug stability may be a factor to consider when integrating RB-PDAT into fungal keratitis management.
Ion-activated mucoadhesive in situ gel incorporating terbinafine-loaded NLCs promotes ocular penetration and retention for enhanced fungal keratitis therapy.
Fungal keratitis (FK) is a sight-threatening infection with poor outcomes due to rapid tear clearance and insufficient corneal penetration. To overcome the limitations, we developed an ion-activated mucoadhesive in-situ gelling system (TH-NLC-ISG) integrating terbinafine-loaded nanostructured lipid carrier (TH-NLC) into a gellan gum/carboxymethyl chitosan matrix. The optimized formulation exhibited a particle size of 25.27 ± 0.75 nm. Upon contact with simulated tear fluid, the formulation underwent rapid sol-gel transition, achieving a 70-fold viscosity increase (34 °C, shear rate 1 s-1), critical for prolonged ocular residence. Fluorescence imaging demonstrated ocular retention of fluorescein sodium-labeled TH-NLC-ISG exceeding 60 min, significantly longer than the non-gelling control. In vivo pharmacokinetic studies revealed markedly improved ocular bioavailability, with higher area under the concentration in ocular tissues-time curve of TH in the TH-NLC-ISG group compared to TH-NLC. Corneal penetration studies confirmed that TH-NLC-ISG exhibited longer corneal retention time and higher corneal drug retention compared to TH-NLC. The formulation demonstrated excellent ocular biocompatibility in vitro and in vivo. TH-NLC-ISG exhibited potent in vitro antifungal activity against Fusarium solani. and Aspergillus flavus., with a lower MIC50 value than TH suspension. In rabbits and mice FK models, TH-NLC-ISG group achieved complete corneal healing, reduced clinical scores and corneal perforation rates, displaying efficacy superior to conventional natamycin eye drops. By synergistically augmenting retention and corneal permeation, TH-NLC-ISG provides a transformative strategy for enhanced fungal keratitis therapy.
Severe fungal keratitis caused by Lasiodiplodia theobromae following grass trimmer-related ocular trauma: a case report.
Lasiodiplodia theobromae is an uncommon but emerging filamentous fungus primarily recognized as a plant pathogen. We report a case of severe L. theobromae keratitis in a 57-year-old man who presented with left eye pain, redness, and progressive visual loss following grass trimmer-related ocular trauma. Ocular examination revealed diffuse corneal haziness, dense stromal infiltrate, total epithelial defect, and descemetoceles. The organism was isolated from corneal scrapings and identified by microscopy, culture, MALDI-TOF mass spectrometry, and confirmatory ITS sequencing. Management required intensive topical, intravitreal, and systemic antifungal therapy in addition to surgical intervention. Visual acuity remained at light perception at three weeks postoperatively, with no further deterioration observed following combined surgical and medical management. Early diagnosis remains challenging due to overlapping morphological features with other filamentous fungi, necessitating molecular confirmation.
