Detection of Echinococcus multilocularis in coyotes in Washington State, USA highlights need for increased wildlife surveillance.
Echinococcus multilocularis is a zoonotic cestode that uses canids as definitive hosts and rodents as intermediate hosts. In humans, this parasite is the causative agent of alveolar echinococcosis. Recently, its range has been expanding across the Northern Hemisphere, and it is increasingly detected in wild canids, domestic dogs, and humans across Canada and the United States. While this expansion has been documented in isolated studies across the continent, a lack of routine sampling in wildlife hinders our ability to anticipate and mitigate further spread of E. multilocularis. We confirmed the presence of E. multilocularis in Washington State, USA, using a combination of morphological and molecular techniques across carcasses and field-collected scats of coyotes (Canis latrans), this region's most common wild canid. Morphological identification of adult worms was confirmed by next-generation sequencing. Over a third of all samples tested positive for E. multilocularis when all methodologies were combined. Sequencing revealed a haplotype of E. multilocularis matching a documented haplotype originally of European origin in British Columbia, Canada. Our study provides the first confirmation of E. multilocularis in a wild host on the west coast of the contiguous United States and provides additional haplotype information crucial to tracking the geographical expansion of the parasite. We also provide a new next-generation sequencing primer targeting cestodes of canids. The difference in amplification between intestinal and fecal samples suggests that non-invasive fecal sampling using DNA metabarcoding - a popular method of helminth surveillance - may lead to underestimation of prevalence, hindering control measures. The global significance of these findings extends beyond North America; E. multilocularis is a major public health concern in Europe and Asia, where alveolar echinococcosis is increasingly diagnosed in humans. Our study highlights the urgent need for increased surveillance and improved diagnostic strategies worldwide, particularly in regions with significant human-wildlife contact.
Canonical WNT signalling governs Echinococcus metacestode development.
Alveolar echinococcosis (AE) is a lethal zoonosis caused by infiltrative growth of the metacestode larva of the tapeworm Echinococcus multilocularis in host organs. We previously showed that the Echinococcus metacestode is an evolutionarily unique, broadly posteriorized tissue, leading us to hypothesize that canonical WNT (cWNT) signalling, which patterns the body axis across metazoans, might be critical for metacestode formation. Here, we report effective RNAi-mediated knockdown of the E. multilocularis β-catenin gene (bcat-1), the central effector of cWNT signalling, in a primary parasite cell culture system that produces metacestode vesicles. bcat-1(RNAi) cultures were markedly impaired in vesicle formation, exhibited stem-cell hyperproliferation, and displayed changes in muscle-fibre organisation. Genome-wide transcriptomics revealed a general anteriorization of gene expression, and in situ hybridization showed an overproduction of cells expressing head-inducing factors such as sfrp upon bcat-1 knockdown. Conversely, metacestode-specific genes including the tegumental factors muc-1, TNFR, and antigen B as well as the posterior marker post2b were significantly downregulated, consistent with the observed vesicle-formation defects. In situ analyses further identified anterior markers frizzled-10, nou-darake, notum, and follistatin that were overexpressed in bcat-1(RNAi) cultures and localized to the future anterior pole at the earliest stages of protoscolex formation. Together, these findings establish a central role for cWNT signalling in directing Echinococcus body-axis formation and the posteriorization events driving metacestode growth within the host, providing insight into asexual parasite proliferation mediated by this biologically unique larval stage and pointing to potential targets for chemotherapy against AE.
Burden of cystic and alveolar echinococcosis in the Kyrgyz Republic.
The purpose of this study is to assess some aspects of the burden of cystic and alveolar echinococcosis in the Kyrgyz Republic. From 2015-2020 a total of 5568 cases of primary cystic echinococcosis (CE) were reported and a further 1008 recurrent cases. Over the same time period 880 primary cases of primary alveolar echinococcosis (AE) were reported and a further 343 recurrent cases. The estimated DALYs for CE was 7849, that is 1308 per annum or 19.5 per 100,000 per annum. For AE was there were 3809 DALYs, that is 634 per annum or 9.5 per 100,000 per annum. The proportion of fatal cases of AE was 3.6 times higher than from CE. The total economic damage in terms of purchasing power equivalent (PPE) from CE amounted to more than 10,5 million international US dollars. For AE it amounted to more than 5.1 million international US dollars. Most of the financial burden falls on the patients themselves and amounts to 68.4%, i.e., 1223 PPE dollars per case out of total spending 1615 PPE dollars per case. To reduce the social and economic burden of echinococcosis, it is necessary to timely identify the disease, prevent the development of complications and multiple lesions, and promote preventive chemotherapy where appropriate. A deep understanding of the etiology, epidemiology, clinical manifestations, and tactics of early diagnosis of various forms of echinococcosis is also important for timely detection, effective control, and, most importantly, the organization of preventive measures to combat this disease.
Population-based ultrasound prevalence and risk factors for cystic echinococcosis in endemic Kazakhstan.
Cystic echinococcosis (CE), caused by Echinococcus granulosus sensu lato, remains a significant public health concern in endemic areas of Kazakhstan. Despite global control efforts, CE persists due to insufficient deworming of dogs, the absence of control programs, and low public awareness. A cross-sectional study was conducted between September 2023 and June 2024 in 51 remote villages across two endemic regions of Kazakhstan. Participants underwent abdominal ultrasound screening and completed a structured questionnaire to assess potential risk factors. Cyst staging was performed according to the WHO - Informal Working Group on Echinococcosis guidelines. Statistical analyses included both univariate and multivariate models to identify significant predictors of infection. The prevalence of CE was 0.34%, with higher rates observed in young people (82.1%) than in adults (17.9%), particularly among those aged 9-14 years. Major potential risk factors included dog ownership (OR = 3.17, p = 0.012), failure to deworm dogs (OR = 11.12, p = 0.018), feeding raw offal to dogs (OR = 3.06, p = 0.012), and consumption of unwashed vegetables and fruits among women (OR = 5.25, p = 0.005). All identified CE cases were newly diagnosed. Of these, 92.85% were active cysts, predominantly found in young individuals, while inactive cysts accounted for 7.14%, distributed equally between adults and youths. These findings underscore the urgent need for regular deworming programs for dogs, public education on hygiene practices, and stricter management of livestock offal. Further research is needed to investigate transmission through water and environmental contamination.
A neuregulin-like ligand and EGF receptor underpin Echinococcus multilocularis development.
Alveolar echinococcosis (AE), a neglected zoonotic disease, is caused by the infiltrative growth of the metacestode larval stage of the cestode Echinococcus multilocularis within host organs. We previously showed that metacestode development relies on the mitotic activity of a population of parasite stem cells known as germinative cells. However, the molecular mechanisms that control Echinococcus stem cell dynamics such as cell-cycle progression, self-renewal, and differentiation remain poorly understood. Building on earlier reports implicating epidermal growth factor (EGF) signalling in stem cell regulation, we here characterize the parasite's repertoire of three EGF receptors. Using RNAi and inhibitor assays, we identify one receptor, EmER1, as essential for the formation of metacestode vesicles from germinative cells. We further demonstrate that EmER1 is targeted by afatinib, an EGF receptor inhibitor with potent anti-parasitic activity both in vitro and in vivo. Through bioinformatic analyses and membrane-bound yeast two-hybrid assays, we identified a parasite-derived, neuregulin-like ligand for EmER1, termed EmNRG, whose expression is strongly upregulated in metacestode vesicles during clonal expansion of germinative cells. RNAi-mediated knockdown of the EmNRG encoding gene severely impaired the capacity of germinative cells to generate metacestode vesicles. Our data support that EmNRG and EmER1 constitute a cognate ligand - receptor system regulating the balance between asymmetric and symmetric stem cell division in E. multilocularis. These findings provide new insights into Echinococcus stem cell biology and highlight EGF signalling as a promising avenue for developing novel anti-echinococcosis therapeutics.
