Use of tumor treating fields for a malignant brain tumor in a pregnant woman: Case report.
Brain tumors diagnosed during pregnancy are exceptionally rare, and their clinical progression is not yet fully understood. Managing intracranial tumors during pregnancy necessitates a specialized approach, balancing neuro-oncological considerations with obstetric concerns to evaluate therapeutic options effectively.Tumor Treating Fields (TTF) therapy is a non-invasive, regionally-applied, anti-cancer treatment modality approved for adults with newly diagnosed and recurrent glioblastoma. To date, there is no data on the safety and efficacy of TTF therapy in pregnant patients harbouring malignant brain tumors. To describe a case of TTF-use during pregnancy and provide a first insight into its feasibility. We describe the first reported case of a pregnant women treated with TTF. Medical records and imaging data were analysed. Relevant literature concerning the management of malignant brain tumors in pregnancy was reviewed. The patient diagnosed with a posterior fossa high-grade anaplastic ganglioglioma (CNS WHO grade 3-4) received surgical resection followed by radiotherapy with fetal shielding combined with TTF therapy during pregnancy. Since TTF therapy represents a topical treatment without systemic application, we suggest it as a feasible option for pregnant patients diagnosed with malignant brain tumors.
SpikeSpector software for MultiElectrode Arrays: Field potential analysis and spatial integration of electrophysiological and histological data.
The analysis of extracellular recordings from MultiElectrode Arrays (MEAs) is central to understanding the spatio-temporal dynamics of neuronal network activity in both physiological and pathological conditions. The detection of local field potential (LFP) events (of variable amplitude and shape, embedded in noisy, overlapping signals) remains particularly challenging when relying solely on visual detection or on automated tools. We developed SpikeSpector, a standalone, Python-based graphical user interface (GUI) that enables comprehensive LFP detection, precise manual curation, spatial mapping, and multimodal integration with immunohistochemical markers. SpikeSpector tools are described using MEA data recorded from a mouse brain slice harboring a ganglioglioma tumor. SpikeSpector enables users to refine automatically detected events through interfaces, improving the reliability of field potential identification in complex datasets. The platform offers visualization of voltage traces aligned to MEA layouts, customizable spike sorting and deletion tools, and quantification of waveform metrics such as amplitude, slope, and half-width. In addition, SpikeSpector introduces novel modules for correlating spike metrics with histological staining intensities, offering a powerful framework to spatially relate electrophysiological patterns to tissue architecture. Most existing tools for MEA analysis are limited by licensing costs, dependence on proprietary environments, and lack of integration with histological data, crucial for studies on brain diseases. Overall, SpikeSpector represents a flexible and accessible solution for bridging electrophysiological data with histological insights, enabling a more accurate and context-rich analysis of neural dynamics.

Multinodular and vacuolating neuronal tumor: molecular genetics and DNA methylation analysis of 12 cases.
Multinodular and vacuolating neuronal tumor (MVNT) was recognized as a distinct neuronal tumor entity in the revision of the 2021 World Health Organization (WHO) Classification of Tumors of the CNS. In this study, we retrospectively analyzed 12 surgical cases, two of which exhibited ganglioglioma (GG)-like components. The cohort consisted of eight male and four female patients, with a median age of 31 years (age range: 18-53 years). Seizures were the most common clinical presentation, followed by headache and dizziness. Three patients were incidentally identified during physical examination. Ten tumors were located in the cerebral hemisphere, and the remaining two were found in the cerebellum and thalamus, respectively. Histopathological examination revealed clusters of neuroepithelial cells with large amphophilic vacuolated cytoplasm and eccentrically placed round nuclei containing prominent nucleoli. Immunohistochemically, these vacuolated cells were positive for OLIG2, MAP2, SYN and SOX10, and negative for GFAP and NEUN. DNA sequencing analysis identified no mutations in IDH1, IDH2, BRAF V600E, TERT promoter, or EGFR genes. Among the cohort, three cases harbored FGFR2 mutations, and FGFR2::INA gene fusion was detected in both MVNT and GG-like components. Two cases carried BRAF mutation, and one case exhibited MAP2K1 mutation. A novel BCAN::NTRK1 (exon 12-exon 9) gene fusion was identified in one case. DNA methylation profiling of eight cases revealed that none matched with a known CNS tumor type. Six cases formed a separate methylation cluster, suggesting a potential novel molecular subtype, while the remaining two cases exhibited transcriptional similarities to supratentorial pilocytic astrocytoma and rosette-forming glioneuronal tumor (RGNT), respectively. Postoperatively, all patients remained seizure free with no evidence of tumor progression. Only one patient died 16 months after surgery due to an unrelated traffic accident. © 2026 The Pathological Society of Great Britain and Ireland.
Endoscopic management of cranial screw migration to cerebral aqueduct causing hydrocephalus.
Intracranial migration of cranial fixation hardware is a rare complication of neurosurgical procedures, and migration into the ventricular system is exceedingly uncommon. When such migration leads to obstruction of cerebrospinal fluid (CSF) pathways, it can result in acute hydrocephalus requiring urgent intervention. A 10-year-old girl with a history of left frontotemporal ganglioglioma underwent synthetic Medpor cranioplasty for radiation necrosis-associated calvarial erosion. Intraoperatively, a fixation screw inadvertently plunged through a bone defect into an area of encephalomalacia. Postoperative imaging revealed the screw within the antrum of the left lateral ventricle. The patient remained asymptomatic and was discharged with close monitoring. At 3-week follow-up, she developed progressive headaches and fatigue. Neuroimaging demonstrated migration of the screw into the cerebral aqueduct, with resulting obstructive hydrocephalus. She underwent endoscopic third ventriculostomy and successful endoscopic removal of the screw, with full clinical recovery. This case illustrates a rare but serious complication of cranioplasty: delayed migration of a cranial screw into the ventricular system causing aqueduct obstruction. Endoscopic management allowed for both CSF diversion and removal of the foreign body and was associated with an excellent clinical outcome.
