A transcription regulator atlas identifies TOX3 as an Atoh1 coactivator in cerebellar development and tumorigenesis.
Organ development and function are orchestrated by intricate transcriptional circuits. Here, we present a comprehensive atlas profiling 1,904 transcription regulators in the brain, cerebellum, heart, kidney, liver, ovary, and testis of fetal, neonatal, and adult mice. Using this dataset, we uncover Thymocyte Selection-Associated High Mobility Group Box Family Member 3 (TOX3) as a potential coactivator of Atoh1 in cerebellar granule neuron progenitors (GNPs). Tox3-deficient mice display severe ataxia and cerebellar hypoplasia, driven by depletion of GNPs, diminished Atoh1 expression, and impaired primary cilia. Single-nucleus RNA-sequencing analyses reveals compromised maintenance of the progenitor pool. TOX3 is also highly expressed in subsets of medulloblastoma, and its deletion reduces cerebellar neoplasia and prolongs survival in a mouse model. Mechanistically, how lineage-defining factors such as Atoh1 drive robust gene expression despite weak intrinsic transactivation activity remains unclear. We show that Tox3 physically associates with Atoh1 and co-occupies shared regulatory elements, converting an otherwise weak single-copy Atoh1-responsive E-box into a highly active enhancer that drives transcriptional activation by up to 120-fold, including at an ultraconserved E-box downstream of Atoh1 itself. Cross-species single-cell comparisons further show an association between Tox3 expression and cerebellum expansion during vertebrate evolution. Together, this work supports Tox3 as a critical Atoh1 coactivator in cerebellar development, tumorigenesis, and evolution, while providing an atlas and screening strategy as a valuable resource for exploring novel transcriptional regulators in organogenesis and tissue physiology.
Temporal clustering of neuroblastic tumours in children and young adults from Scotland.
Neuroblastic tumours (neuroblastoma and ganglioneuroblastoma) are the most common childhood solid tumours outside the central nervous system, with a median age of diagnosis of 2 years. Temporal clustering of neuroblastic tumours in northern England and Ontario, Canada has been previously reported. We extracted data from the Scottish Cancer Registry to determine whether there was evidence of temporal clustering of neuroblastic tumours. Cases diagnosed in children and young adults aged 0-24 years between 2000 and 2020 were analysed. A modified version of the Potthoff-Whittinghill method was used to test for temporal clustering. Estimates of extra-Poisson variation (EPV) and standard errors (SE) were derived. One hundred and sixty-one cases of neuroblastic tumours, aged 0-24 years, were diagnosed during the study period. Overall, there was statistically significant temporal clustering between years within the full study period (EPV = 9.13, SE = 0.22, P < 0.001). In addition, for cases aged < 18 months, there was significant temporal clustering between months within quarters (EPV = 0.77, SE = 0.41, P = 0.044). For cases aged 18 months - 24 years, there was significant temporal clustering between fortnights within months (EPV = 1.00, SE = 0.47, P = 0.012). The finding of temporal clustering is consistent with the involvement of one or more, as yet unknown, transient environmental agents in the aetiology of neuroblastic tumours.
Prognostic Impact of Treatment Modalities, Including Targeted Compartmental Radio-Immunotherapy, in a Cohort of Neuroblastoma Patients With CNS Metastases at Relapse.
Neuroblastoma (NB) with central nervous system (CNS) metastases is rare at diagnosis, but occurs more often during relapse/progression. Patients with CNS metastases face a dismal prognosis, with no standardized curative treatment available. Novel therapeutic approaches, such as intraventricular radio-immunotherapy with 131I-omburtamab (Omb), have been developed. In this study, we report a retrospective, single-tertiary center analysis of a 23-year cohort of NB patients with CNS metastases, highlighting current treatment strategies. Retrospective data analysis of all NB patients with CNS metastases treated at Hospital Sant Joan de Déu, Barcelona, from January 2000 to January 2023. Patient characteristics at diagnosis, first-line treatment, relapse patterns, and CNS metastasis management were analyzed in search of risk variables and survival outcomes. CNS metastases at relapse were identified in 39/185 (21.1%) patients. Median age at diagnosis was 2.7 years, and 24/39 were male. Stage 4 NB with multisite compartment metastases accounted for most cases (92.2%). CNS events occurred predominantly at first relapse (29/39, 74.4%) and with neurological symptoms (23/38, 60.5%). MCYN amplification and concomitant extra-CNS metastases at CNS relapse were associated with poorer overall survival (OS) (p = 0.018 and p = 0.0059, respectively). Neurological symptoms upon relapse significantly increased the risk for subsequent CNS events (p = 0.028). Curative-intent treatment was attempted in 34/39 (87.2%) patients. After adjusting for immortal time bias, RT plus Omb significantly improved OS (p < 0.0001). In our experience, MYCN amplification and concomitant extra-CNS metastases at CNS relapse significantly decrease OS. Multimodal treatment, including 131I-omburtamab radioimmunotherapy, significantly improves survival outcomes.
A functionally relevant model for interrogating brain tumor-endothelial cell interactions.
Intercellular interactions, particularly those between tumor cells and the surrounding vasculature, are central to the biology of the tumor microenvironment. Approaches for studying these interactions often rely on limited patient samples or time- and resource-intensive xenograft tissues in combination with histological or single-cell omics profiling. While informative, these models capture only static snapshots and limit mechanistic interrogation. Studying the mechanisms behind these interactions requires viable co-culture models for culturing different cell types together in vitro, while preserving the phenotypic integrity of each cell type. To achieve this, we developed and validated optimal in vitro culture conditions to support the co-culture of human Group 3 medulloblastoma (G3 MB) cells and microvascular brain endothelial cells (BECs) as an ideal screening model for mechanistic and interventional studies. Supported by a new optimized 1:1 mixed media formulation, this model preserves native cellular morphology and phenotypic characteristics. When cultured alone in the new optimized media, G3 MB cells retained expression of stemness markers (SOX2 & OTX2), self-renewal capacity, and undifferentiated morphology, while BECs retained tight junction formation and migratory behavior. This co-culture platform permits real-time, dynamic, and mechanistic studies of tumor-endothelial cell interactions, overcoming the limitations of fixed-tissue analyses and facilitating precise experimental manipulation. This well-characterized model provides a physiologically and functionally relevant platform for further dissecting the reciprocal interactions present between various brain cancer cells and vascular endothelial cells, supporting the development of targeted therapeutic strategies and advancing our understanding of brain tumor biology.
Fluid shear stress activates a targetable mechano-metastatic cascade to promote medulloblastoma metastasis.
Biofluid flow generates fluid shear stress (FSS), a mechanical force widely present in the tissue microenvironment. How brain tumour growth alters the conduit of biofluid and impacts FSS-regulated cancer progression is unknown. Dissemination of medulloblastoma (MB) cells into the cerebrospinal fluid initiates metastasis within the central nervous system. Here, by simulating cerebrospinal fluid dynamics based on magnetic resonance imaging of patients with MB, we discover that FSS is elevated at the cervicomedullary junction. MB-relevant FSS promotes metastasis along the mouse spinal cord. Mechanistically, FSS induces metastatic cell behaviours, including weakened cell-substrate adhesion, increased motility, cell clustering and plasma membrane localization of glucose transporter 1 (GLUT1) to enhance glucose uptake. FSS is perceived by the mechanosensitive ion channel PIEZO2, which drives actomyosin contractility-dependent GLUT1 recruitment at the plasma membrane. Genetic targeting of PIEZO2 or pharmacologic inhibition of GLUT1 mitigates metastasis. Collectively, these findings define a targetable FSS-activated mechano-metastatic cascade for the treatment of MB metastasis.
