Acute febrile illness surveillance using TaqMan Array Cards in two urban health facilities, Monrovia, Liberia, December 2018-March 2020.
Fever is a common symptom of infectious diseases including for those with epidemic potential. Beyond malaria, the causes of undifferentiated (i.e., non-respiratory, non-diarrheal) acute febrile illnesses are not well characterized in Liberia. From December 2018 through March 2020, we established two acute febrile illness (AFI) sentinel surveillance sites in urban Monrovia at Redemption Hospital and Star of the Sea Health Centre, health facilities that were among the first to have Ebola cases during the 2014-2015 West Africa epidemic. Enrolled AFI patients were two (2) years of age or greater, had a measured fever of ≥37.5oC or history of fever within the past week, and without a known cause of fever. A standardized survey was administered to collect demographic, clinical characteristics, and risk factors. Whole blood was taken, nucleic acid material was extracted and ran on TaqMan Array Cards (TAC), a real-time polymerase chain reaction (RT-PCR) testing platform for 28 pathogens. Data were analyzed using descriptive statistics, and multivariate regression models of any TAC detections stratified by site and age. We enrolled 1506 AFI patients, 1206 (80%) from Redemption Hospital and 300 (20%) from Star of the Sea. AFI patients were predominantly female (69%) and had a median (interquartile range) age of 18 (7-27) years. Among the 699 (46%) that were TAC positive, 627 were detected from Redemption Hospital and 72 were detected from Star of the Sea Health Centre. Overall Plasmodium spp. (malaria) (96%) were the majority of detections followed by dengue virus (2%), Streptococcus pneumoniae (2%), and Rickettsia spp. (1%). We detected 19 co-infections [malaria co-infections (84%) being the most common]. Two pathogens with epidemic potential, Neisseria meningitidis (detected at Star of the Sea Health Centre) and Lassa virus (detected at Redemption Hospital), were also found. Patients with non-malaria TAC detections (n = 29) were higher at Star of the Sea Health Centre than Redemption Hospital (4% versus 1% respectively, p < 0.05). In multivariate regression for those ages 15 years and older at Redemption Hospital (adjusting for sex, age, pregnancy status, education, occupation, any medication use, measured fever at enrollment, headache, abdominal pain, vomiting/nausea and joint pain), any medication use (aOR=0.6, 95% CI = 0.4-0.9), measured fever at enrollment (aOR=3.5, 95% CI = 1.1-12.0), headache (aOR=1.7, 95% CI = 1.1-2.6 were statistically significant with any TAC detection. In multivariate regression for those ages 2-14 years at Redemption Hospital (adjusting for sex, age, abdominal pain, cough, vomiting/nausea, runny nose, and any animal exposure), having abdominal pain (aOR=1.9, 95% CI = 1.3-2.8), vomiting/nausea (aOR=0.6, 95% CI = 0.4-1.0), and any animal exposure (aOR=1.5, 95% CI = 1.0-2.3) were statistically significant with any TAC detection. This is the first laboratory evidence of dengue and rickettsial disease in humans in Liberia. Liberia's incipient AFI platform was successful exploring causes of fever in emerging infections and detected circulating pathogens beyond malaria. AFI surveillance data can assist in the prioritization of public health diagnostic and clinical capabilities to prevent, detect, and respond to emerging infectious disease threats in Liberia.
Targeted mutagenesis in Ehrlichia canis deleting the phage head-to-tail connector protein gene and its assessment as a vaccine candidate preventing canine ehrlichiosis.
Ehrlichia canis is primarily a Rhipicephalus sanguineus tick-borne rickettsial pathogen initially identified as causing canine monocytic ehrlichiosis, and human infections have also been reported in Venezuela, Mexico, and parts of Europe. It is of high importance to have a vaccine suitable in protecting the canine host, which will aid in lessening E. canis infections also in humans. Gene inactivation mutations in the phage head-to-tail connector protein genes (phtcp) from E. chaffeensis and A. marginale caused attenuated growth, and prior infection with the mutated bacteria induced protective immunity against wild-type bacterial infections in the natural hosts, independent of infection acquisition from blood-borne infection or tick-transmission. In the current study, we describe the development of targeted mutagenesis for the first time in E. canis genome and with a novel modification to avoid introducing antibiotic resistance cassettes to delete the phtcp ortholog from E. canis. The mutated E. canis was then assessed for its in vivo growth and the induction of host immunity exerted following the mutant infection aiding to protect against wild-type infection challenge in the canine host. We assessed systemic infection, hematological parameters, immune responses, and plasma cytokines following the mutant infection relative to uninfected dogs. Similarly, the assessments were carried out following wild-type pathogen infections in dogs with or without prior mutant infection challenges. The study demonstrates that prior infection of dogs with the mutant induces immunity to prevent infection establishment by wild-type E. canis. Similarly, the mutant infection resulted in clear biological differences compared to the wild-type infection. This study establishes that the molecular genetic methods are broadly applicable to pathogens belonging to the family Anaplasmataceae and that the modified live vaccines with functional defects in phtcp gene orthologs are valuable in reducing the diseases caused by the tick-borne rickettsial pathogens belong to Anaplasmataceae, including E. canis.
Metagenomic sequencing identifies potential respiratory pathogens in PCR-negative subset of surveillance samples.
Respiratory pathogens are a significant source of global morbidity, mortality, and economic burden, with the COVID-19 pandemic driving increased interest in and funding for respiratory disease surveillance. Syndromic panel multiplex nucleic acid amplification tests (NAATs) such as the BioFire Respiratory Panel (RP) are designed to identify the most common etiologic agents of respiratory illness. Untargeted metagenomic sequencing is a powerful tool for pathogen-agnostic detection, enabling the recovery of complete genomes for genomic epidemiology and variant tracking. In this study, we performed untargeted metagenomic sequencing of 305 samples previously negative by BioFire RP and SARS-CoV-2 testing and 26 samples that were previously positive by either of the diagnostic tests. A subset of 78 samples underwent probe-capture enrichment sequencing targeting human viruses. Using these methods, we identified human respiratory viruses in 16 of the 305 previously negative samples (5%). The most common viruses identified were Influenza C virus, Human Bocavirus, Rhinovirus A and C, and SARS-CoV-2. Consensus genomes were recovered for 14 viruses with > 90% coverage breadth, revealing closely related Bocavirus strains from neighboring counties and distinct Rhinovirus strains across samples. We also identified 21 samples with a single predominant bacterial or fungal species in the previous negative cohort. These findings underscore the challenges of identifying causal agents from multiplex NAAT-negative cases and highlight the utility of metagenomics for expanding the scope of pathogen surveillance.
Epidemiological and clinical profiles of ocular involvement in rickettsiosis in southern Tunisia.
The ocular manifestations of rickettsial diseases are polymorphic. The goal of our work was to characterize the ocular manifestations of rickettsial diseases and to determine the contribution of ophthalmic examination to the diagnosis. We conducted a retrospective descriptive analysis including consecutive patients with ocular involvement from confirmed rickettsial disease after ruling out other infectious or inflammatory etiologies. Demographic, clinical, multimodal imaging (fluorescein angiography, optical coherence tomography [OCT]), and therapeutic intervention data were systematically analyzed. This study included 24 patients with serologically confirmed rickettsial ocular involvement (mean age 40 years; range 16-67). Presentation with ophthalmologic symptoms occurred in 54.2% (n=13), demonstrating unilateral (58.4%, n=14) or bilateral (41.7%, n=10) ocular involvement, totaling 34 affected eyes. Posterior uveitis predominated (70.5%, 24 eyes), while other significant ophthalmologic signs included fine keratic precipitates (17.6%, 6 eyes), retinitis (64.7%, 22 eyes), retinal vasculitis (20.6%, 7 eyes), and neuroretinitis (8.8%, 3 eyes). Systemic manifestations featured febrile episodes with cutaneous eruptions in 70.8% (n=17). All patients received antibiotic treatment. Oral corticosteroid therapy was added in 12.5% (n=3). Complete resolution of inflammation occurred after a mean follow-up of 3 months. In our series, ocular manifestations in rickettsial infections presented symptomatically in only half of cases, with retinitis and retinal vasculitis emerging as the predominant findings. These observations highlight the importance of systematic posterior segment examination in suspected rickettsial cases to facilitate prompt diagnosis and therapeutic intervention.
Pneumonia with no eschar: an atypical presentation of rickettsial disease.
A middle-aged male construction worker presented with a 7-day history of fever, night sweats and malaise, unresponsive to oral cefuroxime. He developed pale stools and dark urine but denied abdominal pain or neurological symptoms. Examination revealed mild jaundice with stable vital signs and no focal findings. Laboratory tests showed markedly elevated C-reactive protein, thrombocytopenia and significantly deranged liver enzymes. Extensive infectious workup was negative except for a positive PCR for Rickettsia species. Imaging revealed a right upper lobe consolidation on CT, though chest X-ray was initially normal. He was empirically treated with doxycycline before confirmation of rickettsial infection, based on the clinical picture. The patient's symptoms and liver function improved with doxycycline, supporting the diagnosis of a systemic rickettsial illness with hepatic and pulmonary involvement. This case is notable for the rare pulmonary involvement in rickettsial infection along with the absence of the typical rickettsial rash or eschar, both of which highlight the diagnostic challenge in this case.
