Electromagnetic exposure changes human Schwann cell motility and transcriptomic profile of hearing-loss-related genes.
Schwann cells (SCs) are glial cells of the peripheral nervous system with functional plasticity in physio-pathological conditions, such as injury and oncotransformation. Interestingly, human SCs seem to develop, mature, and respond differently to other species to environmental challenges. Recently, concerns have been raised about the expansion of environmental electromagnetic fields (EMFs) and the increasing incidence of vestibular schwannoma (VS). We previously demonstrated that SCs respond to EMF by acquiring a state prone to oncotransformation. However, the biodependence of such transformation on the species (rodents versus humans) deserves clarification. Here, we obtained and characterized human SCs, showing that EMF affects proliferation, migration, and transcriptomic profile. The expression of motility proteins increased, while that of cell-to-cell adhesion proteins decreased. The functional clustering of genes differentially expressed matched those involved in hearing loss. In summary, we provide insights into the effects of chronic EMF on human SCs, highlighting its potential role in VS onset.
Radiation-induced cavernous malformation in the brainstem after Gamma Knife radiosurgery for vestibular schwannoma: A case report and literature review.
Radiation-induced cavernous malformation (RICM) is an uncommon late complication of radiation therapy. There are even fewer cases reported after stereotactic radiosurgery (SRS). In this study, we investigated the clinical characteristics, management considerations, and outcomes of RICM following SRS. What are the clinical characteristics, management considerations, and outcomes of RICM following SRS? We describe a case of a 50-year-old woman previously treated with Gamma Knife® radiosurgery for a vestibular schwannoma. Almost two decades later, brain magnetic resonance imaging revealed a brainstem cavernous malformation in the brainstem, in the dose fall-off region adjacent to the target volume. A literature review was subsequently conducted to identify comparable cases, associated therapeutic strategies, and clinical outcomes. We identified 32 reported cases of RICM following SRS in the literature. The mean age at SRS was 65 years, with 57% being female. Mean latency to RICM development was 7.3 years. Our case demonstrated one of the longest latency periods reported for this complication. The patient had mild symptoms and was managed conservatively with surveillance imaging. Literature review revealed that 78% of cases underwent surgical management, predominantly those presenting with hemorrhage or progressive neurological symptoms. RICM represents a rare but clinically significant late complication of SRS that can occur after extended latency periods, even in adults. Management should be individualized based on symptoms, hemorrhage history, and lesion location. Further research is needed to develop more evidence-based management of RICM, and to better define the true incidence through long-term follow-up studies.
Health-related quality of life in patients with vestibular schwannoma managed with observation, stereotactic radiosurgery or microsurgery: a systematic review and single-arm meta-analysis.
Health-related quality of life (HRQoL) is a key outcome in the management of vestibular schwannoma (VS). Although wait-and-scan (W&S), stereotactic radiosurgery (SRS), and microsurgery (MS) are established management strategies, their comparative effects on HRQoL remain unclear. This systematic review and meta-analysis aimed to synthesize HRQoL outcomes using the Penn Acoustic Neuroma Quality of Life (PANQOL) questionnaire and to pool PANQOL scores for W&S, SRS, and MS. A systematic search of PubMed and Embase was conducted up to February 2025. Eligible studies included patients with unilateral sporadic VS managed with W&S, SRS, or MS, with HRQoL assessed by PANQOL at least one year after diagnosis or intervention. Pooled mean PANQOL scores were calculated using single-arm meta-analyses. Minimal clinically important differences (MCIDs) were applied to assess relevance. 16 studies including 3745 patients were analyzed. The pooled PANQOL total score was 69.1 (95% CI 66.0-72.2) for W&S (n = 1,430), 66.9 (95% CI 62.7-71.2) for SRS (n = 864), and 61.3 (95% CI 57.2-65.4) for MS (n = 1451). Across domains, scores were the lowest for hearing and energy and the highest for facial function and anxiety. None of the between-strategy differences in total or domain scores exceeded established MCID thresholds. Substantial heterogeneity was present across all analyses (I2 > 75%). Patients with unilateral VS report broadly comparable HRQoL following W&S, SRS, or MS. Although numerical differences in PANQOL scores exist, they are not clinically meaningful. The observed heterogeneity highlights the need for standardized, prospective studies and international collaboration to better inform patient-centered decision-making.
Prior surgical resection is a predictor of facial nerve function decrement after radiosurgery for vestibular schwannomas.
Gamma Knife radiosurgery (GKRS) can serve as adjunctive treatment for vestibular schwannomas (VS) for residual or recurrent tumor after surgical resection. However, it is poorly understood whether prior surgical resection influences the risk of facial nerve injury from GKRS. An institutional database of GKRS treating adult VS was queried for patients with > 2 years of follow-up post-GKRS. Two cohorts were identified: patients with surgical resection prior to GKRS and patients receiving only GKRS. Facial nerve function was graded with the House-Brackmann (HB) scale pre-GKRS and until last follow-up. There were 31 patients in the surgery-GKRS cohort and 107 in the GKRS-only cohort. The surgery-GKRS cohort had a significantly lower median age (57.1 vs 63.6 years) and higher median tumor volume at time of GKRS (1.96 vs 0.84 cm3). Facial nerve function worsened within 2 years post-GKRS in 7/31 (22.6 %) of surgery-GKRS patients and 7/107 (6.5 %) of GKRS-only patients. At last follow-up, facial nerve function worsened in 5/31 (16.1 %) of surgery-GKRS patients and 3/107 (2.8 %) of GKRS-only patients. After controlling for patient, tumor, and radiosurgical parameters, prior surgical resection was significantly predictive of facial nerve function decrement after GKRS within 2 years and at last follow-up. Larger tumor volume was predictive of facial nerve function decrement at last follow-up. Pre-GK HB score was not predictive of new/worsened facial palsy. Patients with prior surgical resection may be at increased risk of new or worsened facial paresis after adjuvant or salvage GKRS compared to patients receiving upfront GKRS.
Correlation of mastoid pneumatization and pneumatization of the posterior lip of internal auditory meatus and its implications during retrosigmoid craniotomy.
This study aimed to evaluate the relationship between the degree of mastoid pneumatization and the pneumatization of the posterior lip of the internal auditory meatus (IAM) in patients with vestibular schwannoma (VS). A retrospective analysis was performed on 224 patients who underwent retrosigmoid approach for VS excision (2011 to 2021). Mastoid pneumatization was graded based on Han et al classification on computed tomography (CT). Presence of pneumatization of the posterior lip of the IAM was assessed, and its correlation with mastoid and frontal sinus pneumatization, dominance of sigmoid sinus and jugular bulb position was analyzed. Pneumatization of the posterior lip of the IAM was observed in 39 patients (17.4%). Higher grades of mastoid pneumatization corresponding to grade 3 or 4 were found in 156 (69.6%) of patients. On univariate analysis, Manjila et al type 3 or 4 jugular bulb and higher grade of mastoid pneumatization were associated with pneumatization of the posterior lip of IAM. On multivariate analysis, only higher grade of mastoid pneumatization was independently associated with a pneumatization of the posterior lip of the IAM (OR, p = 0.003). Higher grades of mastoid pneumatization increased the risk of mastoid air cell entry during craniotomy (p = 0.0001) and was linked to postoperative cerebrospinal fluid (CSF) oto-rhinorrhoea. The degree of mastoid pneumatization has a strong independent correlation with pneumatization of the posterior lip of the IAM. Preoperative thin-slice temporal bone CT is essential in cases of extensive mastoid pneumatization to assess the posterior lip of the IAM for potential pneumatization.
