Poland Syndrome With Dextrocardia: A Rare Association Leading to Complex Cardiopulmonary Challenges.
Patients with Poland syndrome, especially those with left-sided defects accompanied by dextrocardia, may be predisposed to significant respiratory and cardiac complications. This case underscores the importance of early recognition, thorough cardiopulmonary assessment, and proactive planning for airway management and ventilatory support to optimize outcomes during acute illness.
Papillary urothelial tumor of low malignant potential in a pediatric patient: case associated with Poland syndrome.
Papillary urothelial tumor of low malignant potential is rare in pediatric patients, accounting for only 0.1-0.4 % of bladder tumors under 20 years of age. We present the case of a 14-year-old female with Poland syndrome and history of laryngeal, nasal, and colonic polyposis presented with intermittent gross hematuria, suprapubic pain, and urinary incontinence. Transurethral resection (TUR) identified a papillary urothelial neoplasm of low malignant potential. Genetic testing revealed an ARMC4 variant (primary ciliary dyskinesia), with no known association to PUNLMP. This case highlights the importance of a stepwise approach including early imaging, histologic confirmation, and endoscopic monitoring.
ADAMTSL4 ectopia lentis associated with Poland syndrome: a case report.
Poland Syndrome is primarily characterized by musculoskeletal anomalies, such as unilateral absence of the sternocostal head of the pectoralis major. Ocular associations with Poland syndrome are rare, and ADAMTSL4 mutations, typically linked to autosomal recessive ectopia lentis, have not been previously associated with this condition. We describe a 20-month-old female with left-sided Poland syndrome who presented with intermittent right eye pain and a history of progressive corneal clouding. Examination revealed bilateral ectopia lentis, buphthalmos, and elevated intraocular pressure. Genetic testing identified a homozygous ADAMTSL4 variant (c.767_786del20), a novel association with Poland syndrome. The patient underwent successful bilateral lensectomy and anterior vitrectomy, and management of glaucoma was initiated. Her monochorionic diamniotic (MCDA) twin, also harboring the same ADAMTSL4 mutation, exhibited bilateral ectopia lentis and underwent surgical intervention. This report is the first to document an association between Poland syndrome and an ADAMTSL4 mutation, potentially suggesting a shared underlying defect in microfibril assembly. These findings expand our understanding of the genetic and developmental complexities of Poland syndrome and underscore the importance of early ophthalmological evaluation in such patients. This case highlights the significance of genetic and ocular investigations in Poland syndrome, contributing to knowledge on its broader phenotypic spectrum and potential genetic etiologies. Further research is warranted to elucidate the role of ADAMTSL4 in microfibril-related anomalies.
Congenital Upper Extremity Anomalies Misdiagnosed as Ulnar Longitudinal Deficiency.
Patients with a congenital hand plate or upper limb anomaly, a dysplastic ulna, or a syndrome may present with morphologic forms commonly misdiagnosed as ulnar longitudinal deficiency (ULD). Patients who had been misdiagnosed with ULD over the past 35 years were identified from the hospital database. Twenty-six patients with photographs and/or conventional radiographs that could be adequately reproduced were enrolled in the study. In 16 unilateral cases, the final diagnosis changed to symbrachydactyly (6 cases), Poland syndrome (3 cases), transverse deficiency (1 case), radial longitudinal deficiency (2 cases), and congenital pseudarthrosis of the ulna with osteolysis in patients with neurofibromatosis type 1 (4 cases). In 10 bilateral cases, the final diagnosis changed to constriction ring sequence (3 cases), brachydactyly (6 cases), and congenital bilateral negative ulnar variance in a patient with central metacarpal medullary stenosis. This research presents the findings of the 7 most illustrative cases and focuses on their differential diagnosis. Additionally, the literature review summarizes the most common congenital anomalies included in the differential diagnosis of ULD, presenting their clinical and/or radiographic features as detected in patients referred to our service. A detailed evaluation of patients with even simple unilateral congenital hand or upper limb anomalies is invaluable to the diagnostic investigation. The treating pediatrician, clinician, or surgeon should also be sufficiently familiar with the clinical and radiographic features of congenital anomalies and the long list of syndrome-related deficiencies that the differential diagnostic process of ULD includes. Missed or misinterpreted findings in patients with congenital hand or upper limb anomalies, usually recorded after single diagnosis documentation, may result in a misdiagnosis of ULD and delayed detection of associated congenital, medical, or syndromal disorders.
Acute Pericarditis in a Patient With Poland Syndrome.
Poland syndrome is a rare congenital anomaly characterized by unilateral absence or hypoplasia of the pectoralis major muscle. We report the case of a 32-year-old man with previously undiagnosed Poland syndrome who presented with acute, severe, tearing chest pain radiating to the back, which worsened with sitting up and improved with lying down. The cardiac physical exam was negative for murmur and friction rub. Electrocardiograms showed focal ST-segment elevations in precordial leads V2-V4, and serial troponin levels were negative. Life-threatening cardiac causes of chest pain, such as acute coronary syndrome, aortic dissection, pulmonary embolism, and pneumothorax, were ruled out by a CT (computed tomography) scan. A transthoracic echocardiogram demonstrated a trivial anterior pericardial effusion with preserved ventricular function. Three out of four criteria for acute pericarditis were met. Treatment with colchicine 0.6 mg daily and indomethacin 25 mg three times daily resulted in complete resolution of symptoms within one week. To our knowledge, there are no reported cases of pericarditis in patients with Poland syndrome. However, hypothetically altered chest wall anatomy in Poland syndrome may predispose patients to pericardial irritation leading to pericarditis, which is idiopathic in nature.
