Quantitative Assessment of Fluorescein Angiography Leakage via Deep Learning in Pediatric Uveitis: Correlation with Clinical Parameters.
Develop a deep learning algorithm for automated segmentation of retinal vascular and macular edema (ME) leakage in fluorescein angiography (FA) of pediatric uveitis. Model trained on internal FA images with external validation. One hundred eighty-three pediatric uveitis patients (3144 FA images) at the Eye Hospital of Wenzhou Medical University. External validation used 45 patients (396 FA images) from The First Affiliated Hospital of Soochow University and The Affiliated Eye Hospital of Nanjing Medical University. The model was trained, validated, and applied to 28 independent pediatric cases to correlate leakage area with clinical metrics and determine clinically significant change thresholds. Segmentation performance (Dice) and pixel-based leakage area for analysis. In fivefold cross-validation on 2459 internal images (155 patients), the best mean Dice scores were 0.7241 (retinal leakage) and 0.7273 (macular leakage). On external data, the best mean Dice reached 0.7015 and 0.5918, respectively. In the cohort of 28 pediatric uveitis patients, vitreous cells (VCs) and central macular thickness (CMT) were the most consistent parameters associated with retinal leakage across multiple analyses. After multiple comparison correction, both VC and CMT remained significantly associated with total and posterior pole leakage areas at all time points. For ME leakage, CMT and best-corrected visual acuity showed consistent associations, while VC was significant only at follow-up, whereas other examined variables showed no consistent association after correction for multiple comparisons. Receiver operating characteristic analysis identified a -54.8% relative change in total visible retinal leakage area as the optimal cutoff, indicating quiescent inflammation. The deep learning model reliably segments leakage in pediatric uveitis FA, showing multicenter generalizability. Quantitative angiographic parameters correlate with clinical metrics, suggesting potential utility for informing disease management strategies. OCT-Based Evaluation of Vitreous Haze in the First-Line Antimetabolites as Steroid-Sparing Treatment Uveitis Trial.
To evaluate OCT vitreous signal intensity (OCT-VI) as a biomarker of vitreous haze (VH) in a longitudinal, prospective, and standardized manner in the First-Line Antimetabolites as Steroid-Sparing Treatment (FAST) Uveitis Trial. Secondary analysis of a block-randomized, observer-masked, multicenter clinical trial. Two hundred sixty-seven eyes from 147 patients with uveitis enrolled in the FAST Trial were evaluated in this analysis at baseline (BL), month 6 (M6), and month 12 (M12). For each patient, Heidelberg Spectralis OCT volume scans (Heidelberg Engineering) were imported into the Doheny Reading Center 3D-OCTOR software and underwent a semi-automated segmentation. For each OCT volume, 5 B-scans were used to calculate the OCT-VI as a ratio of the vitreous space to the entire image. OCT vitreous signal intensity at BL, M6, and M12 visits and the association with the National Eye Institute VH scale, anterior chamber (AC) cell grade, posterior synechiae (PS), lens status, visual acuity (VA), and central macular thickness (CMT). At BL, the median relative vitreous intensity was 0.43 (interquartile range, [0.33-0.54]) and reduced to 0.39 (0.27-0.53) at M6 and 0.39 (0.26-0.54) at M12. A mixed-effects linear regression model showed that OCT-VI and VH clinical grades were statistically significantly correlated (P < 0.001) when controlled for age, sex, time points, and eye. A statistically significant decrease of OCT-VI was observed over time with treatment (P < 0.001). A 2-step grade decrease in VH was associated with a -0.07 (standard deviation 0.19) change in OCT-VI. A mixed-effects linear regression model demonstrated a statistically significant association between OCT-VI and VA (b = 0.170, P = 0.020). A higher OCT-VI was associated with increasing grades of AC cell (P < 0.001) and presence of cataract (P < 0.001). There was no significant association (P > 0.05) of OCT-VI with PS or CMT. In the FAST Uveitis Trial, OCT-VI was significantly correlated with the VH clinical grades and demonstrated significant longitudinal changes. Microglia-independent rAAV-induced inflammation causes persistent ocular immune dysregulation rescued by S1P receptor modulation.
Inflammation elicited by rAAV vectors continues to present a critical challenge for long-term efficacy and safety of gene therapy in the eye. Preclinical models of gene therapy-associated uveitis (GTAU) show that despite resolution of early acute inflammatory response, persistent subclinical inflammation remains. Here, we employ the GTAU model in Cx3cr1CreER:R26-tdTomato+/- mice to reveal that intravitreal (IVT) rAAV2 administration elicits sustained microglial dysregulation and retention of CD3+ T cells extending to 50 days post-injection (dpi). Deploying pharmacological and genetic approaches, we define the absolute requirement for microglia and T cells to mediate rAAV2-induced inflammation. Targeted depletion confirmed microglia-independent mechanisms initiate GTAU, whilst elimination of lymphocytes prevented both inflammation and microglial activation. Systematic evaluation of therapeutic strategies reveals identified inhibition of T cell recruitment via sphingosine-1-phosphate receptor modulation, but not B cell depletion, as an effective steroid-sparing strategy to prevent both acute and long-term subclinical inflammation. Collectively, our findings challenge the paradigm of microglia-driven ocular inflammation and support the utility of targeted T cell immunomodulation strategies to control GTAU and maintain long-term ocular homeostasis.
Focus on Lactate and Lactylation Modification: The Potential Role in Ophthalmic Disease Treatment.
Lysine lactylation represents a novel post-translational modification (PTM) involved in cellular functions including glycolysis and macrophage polarisation. It differs in form and mechanism from other PTMs such as acetylation, methylation, phosphorylation, ubiquitination, and SUMOylation. As a recently discovered modification, lactylation has been implicated in the progression of multiple diseases. Recent studies further indicate lactylation's association with multiple ocular pathologies. This review systematically summarises and discusses lactylation's involvement in prevalent eye diseases, including myopia, retinopathy, ocular melanoma, uveitis, and macular degeneration. We further collate emerging data suggesting lactylation signalling pathways may represent potential therapeutic targets for ocular pathologies. This review aims to provide a comprehensive overview for holistic intervention strategies and multidimensional assessment across various ocular conditions, while offering valuable insights for future research and development from a lactylation perspective.
Visual Disturbance and Elevated Glutamic Acid Decarboxylase Antibody Levels as Manifestations of Central Nervous System Lymphoma.
Various neurologic manifestations are observed in the course of hematologic malignancies. Clinical neurologic symptoms and manifestations of malignancies may result from the malignancy itself, inflammatory responses, or paraneoplastic syndromes. Therefore, differential diagnosis on the basis of clinical presentation is challenging. This is the first study to report a patient with central nervous system lymphoma and elevated glutamic acid decarboxylase antibody levels who presented with uveitis masquerade syndrome (UMS), followed by progressive consciousness change, mimicking autoimmune encephalitis.
