The Association Between Uveitis and Optic Neuritis in Central Nervous System Demyelinating Disorders: A Multi-Institutional Cohort Study.
Uveitis and optic neuritis related to demyelinating disease share similar immune mechanisms and can have deleterious visual consequences. Data on uveitis incidence in patients with central nervous system (CNS) demyelinating diseases remain limited. Our objective was to assess the 5-year incidence of uveitis among patients with CNS demyelinating diseases with and without optic neuritis. Retrospective cohort study using a health records aggregate. We identified patients aged ≥18 years using diagnostic codes to arrange into 3 study groups: a composite CNS demyelinating disease cohort, a multiple sclerosis (MS) cohort, and a neuromyelitis optica/myelin oligodendrocyte glycoprotein antibody-associated disease cohort, each stratified by presence or absence of optic neuritis. We matched each study group based on demographics, clinical comorbidities, medications, and encounter visits. After matching, the 5-year cumulative incidence of any uveitis among patients with optic neuritis in the CNS demyelinating disease group was 1.09% (95% confidence intervals [CI] 0.950-1.25) compared with 0.610% (95% CI 0.501-0.743) in those without optic neuritis (log-rank P < 0.0001), with a hazard ratio (HR) of 1.99 (95% CI 1.57-2.52). The 5-year cumulative incidence of any uveitis among patients with optic neuritis in patients with MS was 0.986% (95% CI 0.846-1.15) compared to 0.543% (95% CI 0.437-0.676) in those without optic neuritis (log-rank P < 0.0001), with a HR of 1.95 (95% CI 1.51-2.53). Although patients with neuromyelitis optica/myelin oligodendrocyte glycoprotein antibody-associated disease showed similar trends, there was no statistical significance. Intermediate uveitis demonstrated the strongest associations with optic neuritis across study groups. Optic neuritis is a clinically meaningful risk factor for uveitis in patients with CNS demyelinating disease, underscoring the need for careful ophthalmic surveillance in this population.
Long-Term Follow Up of Presumed Trematode Induced Granulomatous Intermediate Uveitis: Predictors for Failure of Medical Treatment.
To identify predicators of medical treatment failure in patients with presumed trematode induced granulomatous intermediate uveitis (PTIGIU). A retrospective review of the medical records of children diagnosed with PTIGIU over a 4-year period was conducted. Main outcome measures included the management options and treatment outcomes. 98 eyes of 90 patients clinically diagnosed with PTIGIU were initially reviewed with a minimum follow up of 2 years. Mean age was 11 ± 2.38 years and 96.9% were males. All patients reported a history of freshwater exposure with a mean exposure to presentation time of 3 ± 2.822 weeks. UBM detected CB granuloma and confirmed the diagnosis in 53 eyes, 27 of which had an associated AC granuloma. Management options included surgical intervention in the form of parsplana vitrectomy ± lensectomy, or conservative management with systemic, topical or periocular steroids. Surgical interventions were done in 30 eyes, 20 of which had initially received conservative management that subsequently failed and required surgery. Univariate logistic regression revealed that CDVA, degree of vitreous/AC inflammation and lens status at initial presentation were significant predictors for surgical intervention (p < 0.001, 0.008, 0.005 and <0.001 respectively). CDVA, vitritis and degree of AC inflammation were also associated with failure of initial conservative management (p = 0.008, 0.005 and 0.009 respectively). Various management options exist for patients with PTIGIU, but patients with lower presenting vision and higher degrees of vitreous inflammation respond better to surgical intervention than medical therapy alone.
Relapsing Intermediate Uveitis following Resolved Syphilitic Retinitis in an HIV-Positive Patient: A Case Report.
Ocular syphilis has atypical presentation in immunocompromised individuals. We report a case of intermediate uveitis as a delayed immune-mediated relapse following the successful treatment of syphilitic retinitis in an HIV-positive individual. A 30-year-old HIV-positive man presented with decreased vision in his left eye. Examination revealed left eye granulomatous anterior uveitis with vitritis, retinal vasculitis, and optic disc edema. The patient was diagnosed with syphilitic uveitis, reactive for rapid plasma reagin (RPR), and initiated on intravenous penicillin by the internist. Topical corticosteroids were initiated, and oral corticosteroids were added after 1 week. Over 2 months, the lesions resolved clinically with decreasing RPR titers. After 2 months of complete remission and tapering of therapy, the patient presented with renewed disc edema, macular edema, vitreous cells, and snowballs suggestive of intermediate uveitis. The RPR titers showed a decreasing pattern. The patient responded well to oral corticosteroids, indicating a likely immune-mediated relapse rather than reinfection. This case highlights the need for continued surveillance, even after clinical resolution.
Pattern of Uveitis in a Secondary Eye Care Center at Panipat, Haryana.
Uveitis pattern varies according to regional, demographic, and racial factors. Therefore, an epidemiological study of the distribution of uveitis in various regions in India is likely to provide valuable information. There is no study available on the pattern of uveitis from Haryana. To report the pattern of uveitis in a secondary eye care center in Panipat, Haryana. A retrospective study was conducted on 65,126 new patients who attended the Ophthalmology outpatient department of a secondary eye care hospital in Panipat from April 2015 to October 2023. The data of all the subjects is taken from electronic medical records analyzed. Minimum age of the patients was 8 years, the maximum was 95 years and the mean age was 42.28 years. Out of 65,126 cases, 377 were found to have uveitis. The cumulative incidence proportion of uveitis among new patients attending our institute during the study period was 0.58%. The majority of patients were of anterior uveitis 158 (41.8%) and the minimum belonged to intermediate uveitis 39 (10.3%). It was also seen that the maximum cases were unilateral. Tailored laboratory investigations were done in all the cases. The most common cause was found to be idiopathic. Anterior uveitis is the most common entity in secondary care and the most common etiology found to be idiopathic.
An Atypical Presentation of Tubulointerstitial Nephritis With Uveitis (TINU) in an Older Patient Requiring Emergency Dialysis: A Case Report.
Tubulointerstitial nephritis with uveitis (TINU) is an immune-mediated systemic disorder most commonly described in adolescents and young adults, and its recognition in older patients remains challenging. We report the case of a 58-year-old woman who presented with systemic symptoms and rapidly progressive acute kidney injury. Renal function deteriorated significantly, necessitating emergency hemodialysis. Renal biopsy demonstrated acute tubulointerstitial nephritis with diffuse interstitial inflammation and minimal fibrosis. During hospitalization, ocular symptoms were identified, and a comprehensive ophthalmologic examination confirmed intermediate uveitis, establishing the diagnosis of TINU syndrome. Treatment with standard-dose corticosteroid therapy resulted in rapid clinical improvement, recovery of urine output, and discontinuation of dialysis within a short period. Renal function and ocular inflammation improved in parallel. Corticosteroids were gradually tapered without relapse. At long-term outpatient follow-up on September 15, 2025, the patient remained clinically stable, with serum creatinine levels ranging from 60 to 70 μmol/L and no recurrence of uveitis. This case highlights an atypical presentation of TINU in an older patient complicated by severe acute kidney injury requiring dialysis and underscores the importance of considering TINU in patients with unexplained acute kidney injury and ocular manifestations, regardless of age. Early recognition and appropriate immunosuppressive therapy are essential for favorable renal and ocular outcomes.
