A Novel Conservative Approach for Managing the Phthisical Silicone Oil-Filled Eye: Conjunctival Flap Coverage and Prosthetic Fitting.
To evaluate the clinical efficacy and safety of shallow lamellar keratectomy combined with total conjunctival flap coverage and subsequent customized prosthetic shell fitting for the management of atrophic, silicone oil-filled eyes following severe open globe injury. This retrospective case series included 24 patients (24 eyes) with phthisical, silicone oil-dependent eyes after open globe injury repair between October 2010 and May 2020. All patients underwent shallow lamellar keratectomy to remove diseased corneal tissue, followed by total conjunctival flap coverage. A customized prosthetic shell was fitted approximately one month postoperatively after confirmed flap healing. Outcome measures included corneal symptom relief assessed by the Numeric Rating Scale (NRS), palpebral fissure height, exophthalmometry, cosmetic satisfaction rated on a 5-point scale, and postoperative complications. The mean postoperative follow-up duration was 4.75 ± 2.51 years. All patients achieved complete resolution of pain and foreign body sensation (postoperative NRS=0 for all; preoperative median: 2.00 [IQR: 1.75-3.00] vs postoperative median: 0.00 [IQR: 0.00-0.00], P=0.001). Ocular protrusion increased significantly from a preoperative median of 7.00 mm (IQR: 6.00-8.00 mm) to 12.00 mm (IQR: 10.00-13.00 mm) postoperatively (P=0.001). Lid height increased from a preoperative median of 3.00 mm (IQR: 2.75-4.00 mm) to 10.00 mm (IQR: 8.00-11.25 mm) postoperatively (P=0.001). Patient satisfaction with ocular appearance improved markedly from a preoperative median of 1.00 (IQR: 1.00-1.25) to 5.00 (IQR: 4.75-5.00) postoperatively (P=0.001). Two eyes developed limbal epithelial implantation cysts, which were successfully managed without recurrence. Cranial CT revealed no silicone oil migration or sympathetic ophthalmia. This globe-preserving technique effectively alleviates corneal irritation and restores ocular symmetry in atrophic, silicone oil-filled eyes, offering a safe, cost-effective, and satisfactory conservative alternative to enucleation or evisceration. Further prospective studies with longer follow-up are warranted.
Sex-Related Effects on the Clinical Presentation of Uveitis.
The primary aim of this study was to determine sex-related patterns of uveitic entities. Secondary aims included sex-related patterns of uveitic complications. Retrospective clinical cohort study. 5703 participants presenting with uveitis from January 2009 to December 2023 were included. uveitis entities and complications. 2870 (50.3%) participants were female and 2831 (49.6%) were male. Males presented at a significantly younger age than females (42.2 vs 45.0). Significant sex-based differences were observed in the prevalence of specific uveitic conditions. Females were more likely to develop multifocal choroiditis (female: male ratio 1.818:1), Vogt-Koyanagi-Harada syndrome (female: male ratio 2.136:1), birdshot chorioretinopathy (female: male ratio 2.800:1, punctate inner choroidopathy (female: male ratio 6.571:1), drug-induced uveitis (female: male ratio 2.833:1), Crohn's disease (female: male ratio 3.714:1) and multiple evanescent white dot syndrome (female: male ratio 5.000:1). In contrast, males were more likely to develop acute posterior multifocal placoid pigment epitheliopathy (female: male ratio 0.364:1), sympathetic ophthalmia (female: male ratio 0.353:1) and syphilis (female: male ratio 0.404:1). Cataract was significantly more common in females, and this remained significant after adjusting for age at presentation, anatomical location and bilaterality. Cystoid macular edema and epiretinal membrane were more frequent in females, whereas retinal detachment occurred more often in males. Bilateral vision loss occurred in 238 patients (4.2%) and was significantly more common in females (4.9% vs 3.5%). Sex appears to be a key factor influencing the underlying etiology and clinical course of uveitis. These findings underscore the importance of considering sex as a biological variable in the diagnosis, monitoring, and management of uveitis.
Long-term surgical outcomes of primary silicone orbital implant after orbital evisceration in infectious versus non-infectious cases.
IntroductionEven with recent improvements, the optimal timing of orbital implant placement after evisceration surgery - particularly in infected cases - remains unclear. This study aimed to compare the long-term outcomes of primary silicone orbital implants after evisceration for infectious and non-infectious causes, focusing mainly on implant-related complications such as exposure or extrusion.MethodsA total of 32 eyes that underwent evisceration were analyzed. Patients were divided into two groups: infectious causes (Group A, n = 10) and non-infectious causes (Group B, n = 22). Postoperative follow-up duration, implant size, histopathological results, and complications - including implant exposure or extrusion - were evaluated.ResultsDemographic characteristics were similar between the groups. The mean follow-up duration was 34.0 ± 17.7 months in group A and 28.18 ± 18.1 months in group B (p = 0.403). The mean implant size was 17.8 ± 2.6 mm in group A and 18.3 ± 1.8 mm in group B (p = 0.693). Mild symblepharon occurred in 2 patients (20%) in group A, while upper lid ptosis was noted in 2 patients (9%) in group B. No cases of implant exposure, extrusion, or sympathetic ophthalmia were observed in either group.ConclusionOrbital evisceration results in comparable long-term outcomes for both infectious and non-infectious indications, with no observed cases of implant exposure or extrusion. These findings support the safe use of primary silicone implants in both scenarios when appropriate surgical technique is applied and adequate postoperative follow-up is provided.
An Endemic Case of West Nile Virus Chorioretinitis in Ohio.
West Nile virus (WNV) is a mosquito-borne flavivirus known to cause infectious uveitis. This case represents one of the few endemic cases in Ohio without a prior travel history with unique ocular manifestations. Case report. A 67-year-old female with past medical history of rheumatoid arthritis and past ocular history of right eye enucleation secondary to childhood penetrating trauma presented with left eye photopsias, floaters, photophobia, and headache. Exam revealed anterior chamber cell, Koeppe nodules, keratic precipitates. Funduscopic exam demonstrated vitreous cell and multifocal chorioretinal lesions. Initial testing was negative, and presumed sympathetic ophthalmia was diagnosed. Treatment with topical and systemic steroids was initiated. Patient recall and exam evolution prompted testing for WNV IgM, which was positive. Systemic and topical corticosteroids were tapered as the patient improved. This case highlights the expanding geographic reach of WNV, the potential for infection despite an unremarkable travel or insect bite history, the novel association of WNV with iris nodules and Kyrieleis plaques, and controversial efficacy of systemic corticosteroids in the management of WNV-associated chorioretinitis.
Incidence and Risk Factors for Sympathetic Ophthalmia Following Open Globe Injuries: A Population-Based Analysis.
To assess the incidence and risk factors for sympathetic ophthalmia (SO) following open globe injuries (OGIs) and to evaluate the protective effect of early eye removal surgeries (enucleation and evisceration). Retrospective clinical cohort study. Patients diagnosed with OGIs between September 2004 and September 2024. This study utilized de-identified data from the TriNetX network. Incidence rates of SO were calculated at 6 months, 1 year, 5 years, and 10 years postinjury. Risk factors for SO, including demographics and the impact of enucleation or evisceration performed within 30 days of injury, were analyzed using Cox proportional hazard models. Incidence rates of SO at 6 months, 1 year, 5 years, and 10 years postinjury. Of 63,763 patients with OGIs (mean age 42.0 ± 23.7 years; 70.9% male), the overall incidence of SO was 0.13% (1 in 768) at 6 months, rising to 0.18% (1 in 555) at 10 years. SO was diagnosed in 105 patients, with 74.3% presenting within 6 months of injury. Enucleation was performed in 1.82% of patients with OGIs at 6 months, increasing to 2.15% by 10 years. Although early eye removal (within 30 days) was associated with a lower hazard (hazard ratio (HR) 0.655), this finding was not statistically significant (95% CI: 0.236-1.817; P = .416). Demographic factors associated with increased SO risk included Native Hawaiian or other Pacific Islander ethnicity (HR: 6.775, 95% CI: 1.786-25.694, P = .005) and Hispanic ethnicity (HR: 2.862, 95% CI: 1.077-7.608, P = .035). SO following OGIs is rare but remains a long-term risk, with most cases presenting within the first year after injury. Early eye removal surgery (within 30 days) was not significantly associated with a lower risk of SO. Additionally, individuals of Native Hawaiian or Other Pacific Islander and Hispanic ethnicities are at increased risk for SO, warranting targeted follow-up in these populations.
