Variation in Management of Children With Coccidioidomycosis.
Coccidioidomycosis (CM) is not as well described in children as it is in adults. The Kaiser Permanente Southern California (KPSC) system provides care within an endemic region for CM, affording the opportunity to describe the diagnosis, management, and outcomes of CM in a large pediatric cohort. This study is a retrospective manual medical record review of patients aged 17 years or younger with a new diagnosis of CM between January 1, 2015, and December 31, 2021. We extracted demographic characteristics, symptoms, diagnostic testing, and details of patient management. CM was identified in 209 patients. The incidence was 3.3 cases per 100 000 children but varied within the area covered by KPSC. Common symptoms were fever, cough, and rash (>59% for each). Disseminated CM was present in 10 patients (4.8%). Of the 199 patients without disseminated infections, 57 were observed without antifungal treatment; 1 (1.2%) eventually required treatment. For the remaining 142 patients treated with antifungals, the median duration of treatment was 150 days (ranging from <3 months to >2 years). After stopping treatment, 3 (2.1%) had recurrence. Recurrence was identified primarily based on symptoms rather than serologic testing, and all responded well to treatment. Wide clinician variability in the management of pediatric coccidioidomycosis was observed. Recurrence after treatment or failure of observation without antifungals was rare. Results from serologic testing outside of the initial diagnosis in asymptomatic patients typically did not change clinical management, while symptomatic follow-up was important for management.
Exhausted T cell phenotypes in disseminated coccidioidomycosis.
Coccidioidomycosis presents clinically as a spectrum ranging from self-limiting Uncomplicated Valley Fever (UVF) in most cases to life-threatening Disseminated Coccidioidomycosis (DCM) in rare individuals. A few patterns of immunologic deficits allowing for dissemination have been identified, though the specific defects in most individuals with DCM remain undefined. We hypothesized that chronic antigen exposure in DCM engenders a state of T cell exhaustion. From a cohort of over 300 subjects with confirmed diagnoses of coccidioidomycosis, circulating T cell phenotypes were characterized via flow cytometry and Coccidioides-specific T cell responses were measured using the Activation-Induced Marker (AIM) assay. Male sex was significantly associated with disseminated disease (odds ratio 2.5; 95% CI: 1.5 - 4.0). 52% of subjects showed Coccidioides-specific T cell responses in our AIM assay. We noted a significant difference in subjects sampled in the first year of diagnosis, where only 8% of DCM subjects had T cell responses during this time, as compared to 44% of UVF subjects (p = 0.04). Among DCM patients with detectable AIM responses, CD4+ T cells demonstrated an exhausted phenotype with elevated PD-1 expression compared to UVF subjects. In vitro PD-1 blockade augmented IFNγ production in most tested DCM subjects. These findings suggest that dissemination may occur in some individuals during a period of impaired antigen-specific T-cell activity. Importantly, these responses can be augmented in vitro by PD-1 blocking antibodies, supporting further study of immune checkpoint therapy as an adjunct to antifungal treatment in disseminated coccidioidomycosis.
Coccidioidomycosis in cirrhosis: A high-risk syndrome with severe outcomes.
Patients with cirrhosis and end-stage liver disease (ESLD) are highly susceptible to severe infections due to cirrhosis-associated immune dysfunction (CAID). The clinical features and outcomes of coccidioidomycosis in this population remain poorly described. We conducted a retrospective cohort study of adults with cirrhosis and/or ESLD diagnosed with coccidioidomycosis between 2010 and 2025 at a tertiary referral center in California's San Joaquin Valley. Demographics, clinical features, diagnostics, management, and outcomes were abstracted from electronic health records. Forty-six patients met inclusion criteria. The mean age was 52 years, 74% were male, and 67% identified as Hispanic. Alcohol-related liver disease was the most common etiology, and 67% had decompensated cirrhosis. Respiratory symptoms predominated, with cough, dyspnea, and fever most frequent; 96% had abnormal chest imaging, most commonly diffuse infiltrates. Disseminated or complicated pulmonary disease occurred in 35%. Median diagnostic delay was 28 days. Antifungal therapy was given to 89% (primarily fluconazole); hepatotoxicity occurred in 26%. Hospitalization was required in 63%, ICU care in 24%, and relapse occurred in 24%. All-cause mortality was 33%, with a median time-to-death of 369 days. Coccidioidomycosis in cirrhosis represents a high-risk clinical syndrome marked by delayed diagnosis, high rates of hospitalization, treatment complications, and significant mortality. Early recognition and optimized anti-fungal management are urgently needed, and prospective studies should define best practices for this vulnerable population.
Blastomycosis, Histoplasmosis, and Coccidioidomycosis in Outpatient Community-Acquired Pneumonia.
Laboratory testing is necessary to distinguish blastomycosis, coccidioidomycosis, or histoplasmosis from other causes of community-acquired pneumonia (CAP). Robust data about testing for and diagnosis of these fungal diseases among patients with CAP throughout the US are lacking. To examine proportions and characteristics of (1) adult outpatients with CAP who underwent diagnostic testing for blastomycosis, coccidioidomycosis, or histoplasmosis; and (2) tested patients who received diagnoses of these diseases. This retrospective cohort study used 2017 to 2023 commercial health insurance claims data from the Merative MarketScan Commercial/Medicare Database. Adult outpatients with diagnosis codes for unspecified CAP were included. The primary outcome was the proportion of patients who underwent fungal diagnostic testing. The secondary outcomes were the proportions of tested patients who received diagnoses of blastomycosis, coccidioidomycosis, or histoplasmosis. Multivariable logistic regression models were used to estimate adjusted odds ratios (aORs) for characteristics independently associated with receiving a diagnosis of coccidioidomycosis or histoplasmosis. Among 573 994 patients (318 152 female [55%]; median [IQR] age, 54 [41-63] years) with unspecified CAP, 25 822 (5%) underwent fungal diagnostic testing, which occurred a median (IQR) of 3 (1-6) health care visits after the initial CAP diagnosis. Among tested patients, 755 (3%) received a blastomycosis, coccidioidomycosis, or histoplasmosis diagnosis code. Rash (aOR, 3.24; 95% CI, 2.27-4.63), lymphadenopathy (aOR, 1.74; 95% CI, 1.15-2.63), myalgia (aOR, 1.66; 95% CI, 1.11-2.49), chest pain (aOR, 1.65; 95% CI, 1.35-2.02), and receipt of antibiotics from multiple classes (aOR, 1.40; 95% CI, 1.17-1.67) were independently associated with increased odds of receiving a coccidioidomycosis diagnosis. Autoimmune inflammatory disease (aOR, 3.00; 95% CI, 1.72-5.21), chest pain (aOR, 1.84; 95% CI, 1.20-2.82), and abnormal weight loss (aOR, 5.15; 95% CI, 2.71-9.79) were associated with increased odds of receiving a histoplasmosis diagnosis. In this cohort study of patients with unspecified CAP, testing rates for blastomycosis, coccidioidomycosis, and histoplasmosis were low in many locations. Increased awareness of these fungal infections may help increase timely testing for fungal diseases among patients with CAP, decrease health care utilization and inappropriate antibiotic use, and improve patient outcomes.
