Peripheral T-Cell Lymphoma Mimicking Chronic Lymphocytic Inflammation with Pontine Perivascular Enhancement Responsive to Steroids: A Case Report with Confirmatory Brain Biopsy and Longitudinally Extensive Transverse Myelitis.
Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) is a steroid-responsive relapsing-remitting disorder of the central nervous system (CNS). However, CLIPPERS-like presentations have been increasingly recognized as potential early manifestations of underlying lymphoma, most commonly B-cell types. T-cell lymphomas in this context remain exceedingly rare. We report the first case of peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS), mimicking CLIPPERS with confirmatory brain biopsy. A 33-year-old man initially presented with pontocerebellar dysfunction and punctate gadolinium-enhancing hindbrain lesions, fulfilling diagnostic criteria for probable CLIPPERS. He achieved transient remission with corticosteroids but later developed relapsing neurological and pulmonary symptoms, radiological progression with supratentorial and spinal involvement including longitudinally extensive transverse myelitis (LETM), and steroid resistance. Despite nondiagnostic lung biopsy and the absence of malignant cells on cerebrospinal fluid (CSF) cytology, CSF circulating tumor DNA (ctDNA) analysis revealed pathogenic mutations in FAS and TET2. Stereotactic biopsy of a right frontal lesion ultimately confirmed PTCL-NOS. The patient's disease progressed despite chemotherapy, leading to death 6 months after diagnosis. This case highlights that CNS infiltration by PTCL-NOS can closely mimic CLIPPERS both clinically and radiologically. LETM may represent an additional warning feature in such presentations. Vigilance for clinical and radiologic red flags, the adjunctive use of CSF ctDNA analysis, and early reconsideration of diagnosis with timely biopsy should be emphasized to improve outcomes in patients with CLIPPERS-like syndromes.
CLIPPERS mimicking multifocal brain metastases: a diagnostic challenge.
Surgical management of intentional ingestion of two nail clippers in an incarcerated adult: a case report.
Foreign body (FB) ingestion is a frequent concern in emergency settings. Intentional ingestion of FBs in adults is more common among incarcerated individuals. Complications such as impaction, perforation, hemorrhage, and obstruction may arise, necessitating prompt evaluation. We report a case of a 26-year-old incarcerated male who presented to the emergency department after ingesting two nail clippers simultaneously. He complained of epigastric pain. Plain abdominal radiography revealed two objects in the stomach. Given the sharp nature of the FBs, his symptoms, and the risk of complications, a contrast-enhanced CT scan was performed, showing no additional findings. Endoscopic retrieval was unsuccessful, and he subsequently underwent open gastrostomy, which successfully removed the objects. Postoperative recovery was uneventful. Most gastrointestinal FBs pass spontaneously without intervention. However, outcomes differ significantly in incarcerated populations. Clinical presentation ranges from asymptomatic cases to acute abdomen. Management is challenging due to the variable size, shape, and sharpness of ingested objects. Abdominal radiography remains the first-line diagnostic tool. Treatment may include observation, endoscopic removal, or surgical intervention, depending on the clinical context. Although many FBs pass spontaneously or are retrieved endoscopically, some require surgery. Gastrostomy is widely accepted for the surgical removal of ingested FBs. Incarcerated individuals represent a unique clinical population in which intentional FB ingestion reflects complex behavioral patterns, posing therapeutic challenges and a high risk of recurrence. Delayed diagnosis may lead to serious complications. Psychiatric evaluation and preventive measures are essential to reduce recurrence in incarcerated patients.
Two Cases of CLIPPERS-like Syndrome Sharing a Hypomorphic UNC13D Variant.
Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) syndrome is a central nervous system (CNS) inflammatory disease characterized by contrast-enhanced MRI findings of salt-and-pepper-like lesions predominantly affecting the brainstem and cerebellum. We report two patients with CLIPPERS-like brain MRI findings who carried the same missense UNC13D variant in one allele along with deleterious variants in the opposite allele. Patient 1, a 23-year-old female, presented at 15 years of age with neurological symptoms and an MRI showing spontaneously resolving, contrast-enhancing lesions in the cerebellum. At age 16, the patient experienced an episode with systemic manifestations followed by recurrent CNS lesions that responded to steroid therapy. At age 22, the patient developed punctate to nodular contrast-enhancing lesions in the brainstem, cerebellum, and cerebrum, findings consistent with CLIPPERS. Patient 2, an 18-year-old female, presented at age 11 with ataxia and dysarthria, and an MRI showing multiple contrast-enhancing lesions in the cerebellum and brainstem, consistent with CLIPPERS MRI findings. Cerebellar biopsy revealed perivascular CD4+ T-lymphocyte infiltration, and the patient responded to steroid therapy, leading to an initial diagnosis of CLIPPERS. These patients were suspected of having inborn errors of immunity and were identified to have compound heterozygous UNC13D variants along with downregulated Munc13-4 protein. Both patients underwent allogeneic hematopoietic cell transplantation, with patient 1 remaining neurologically stable for two years post-transplantation, while patient 2 experienced a post-transplant relapse requiring steroid therapy. These cases highlight that biallelic variants in the UNC13D gene may cause CNS-predominant inflammation that mimics CLIPPERS. A case report of CLIPPERS involving brainstem, cerebellum, white matter of cerebral hemisphere, spinal cord and corpus callosum.
CLIPPERS (Chronic Lymphocytic Inflammation with Pontine Perivascular Enhancement Responsive to Steroids) is a rare inflammatory disorder of the central nervous system. We describe a case with an atypical disease course and unusual imaging features, broadening the radiological spectrum of this condition. An adult male presented with acute gait instability. Neurological examination revealed upper motor neuron signs, including hyperreflexia and patellar/ankle clonus. Brain and spinal MRI demonstrated characteristic punctate, gadolinium-enhancing lesions in the pons, cerebellum, and spinal cord. Symptoms improved markedly with high-dose corticosteroid therapy. However, over a 5-year follow-up, the patient experienced several clinical relapses, each associated with steroid taper or discontinuation. Subsequent MRI during relapses revealed new, atypical lesions within the corpus callosum in addition to the classic findings. Long-term disease control was ultimately achieved with mycophenolate mofetil, an immunosuppressive agent, which maintained clinical and radiological remission. This case highlights that probable CLIPPERS can involve atypical structures such as the corpus callosum, which should not preclude the diagnosis. The relapsing-remitting course underscores the importance of long-term monitoring and the critical role of immunosuppressive agents in sustaining remission. Early initiation of steroid-sparing immunosuppressants should be considered for patients with a relapsing disease course.
