Viral mechanisms, tropism, and clinical relevance regarding the ophthalmic manifestations of SARS-CoV-2 infection.
To explore the mechanisms underlying ocular infection by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), we conducted a comprehensive review of current literature, focusing on viral entry pathways, receptor expression in ocular tissues, and associated clinical manifestations. This review encompasses studies published within the last five years with a focus on original research and systematic reviews that provide molecular, histological, or clinical evidence. The findings show that SARS-CoV-2 can infect ocular tissues through multiple receptors beyond angiotensin-converting enzyme 2 (ACE2), including transmembrane serine protease 2 (TMPRSS2), CD147, alanyl aminopeptidase N (ANPEP), dipeptidyl peptidase 4 (DPP4), angiotensin II receptor type 2 (AGTR2), and polymeric immunoglobulin receptor (PIGR), which are expressed in retinal, conjunctival, corneal, limbal, and photoreceptor cells. The virus may also reach ocular structures via neurovascular invasion. Clinically, patients with coronavirus disease 2019 (COVID-19) may present with a broad spectrum of ophthalmic manifestations, including conjunctivitis, hyperreflective lesions in the inner retinal layers, flame-shaped hemorrhages, cotton-wool spots, retinal pallor, hard exudates, and various forms of maculopathy, such as paracentral acute middle maculopathy and acute macular neuroretinopathy (AMN). These signs reflect both direct viral damage and secondary effects of systemic inflammation and microvascular injury. Understanding the molecular and clinical spectrum of ocular involvement is essential for early diagnosis, appropriate ophthalmologic care, and the prevention of long-term visual sequelae in patients affected by COVID-19.
Choroidal vascular changes in eyes with acute macular neuroretinopathy and paracentral acute middle maculopathy: new insights.
To assess choroidal vasculature changes in acute macular neuroretinopathy (AMN) and paracentral acute middle maculopathy (PAMM) during the acute and resolution phases. Retrospective, cross-sectional study. Twenty-eight eyes from 28 patients were analyzed: 8 with AMN and 20 with PAMM. Also, 30 healthy age-matched controls were included. Clinical records and spectral-domain optical coherence tomography (SD-OCT) scans from patients with AMN and PAMM were assessed. Choroidal assessment was performed using an automated algorithm, binarized into stromal choroidal areas (SCA) and luminal choroidal areas (LCA). Choroidal vascularity index (CVI) was calculated as the ratio of LCA to total choroidal area (TCA). Statistical analysis was performed using non-parametric tests. The median age was 54 years (IQR = 32-68.5), and 60.7% were female. AMN eyes showed significantly increased subfoveal choroidal thickness (SFCT), TCA, SCA, and LCA compared to PAMM and controls (p = 0.044, p = 0.007, p = 0.002, p = 0.014 respectively). No differences in CVI were observed between the groups (p = 0.535). These findings remained consistent when comparing AMN with isolated PAMM and PAMM associated with retinal vascular diseases (p < 0.05). SFCT, TCA, SCA, and LCA significantly decreased in AMN eyes (p = 0.045, p = 0.041, p = 0.040, p = 0.038 respectively) after resolution, while no changes were observed in PAMM (p > 0.05). Also, resolved AMN exhibited higher SFCT, TCA, SCA and LCA compared to controls and resolved PAMM eyes (p < 0.05). Increased choroidal thickness is a characteristic feature of AMN, with reduction observed after resolution. On the other hand, no changes were observed in PAMM, suggesting distinct pathophysiological mechanisms between the two conditions.
Acute Macular Neuroretinopathy in Association with Orbital Inflammation and Optic Perineuritis.
A 17-year-old girl experienced acute onset of decreased vision with pain on movement in her left eye. Her best-corrected visual acuity was 20/20 in the right eye and 20/100 in the left eye. Pupillary examination revealed a positive relative afferent pupillary defect. Fundus examination showed optic disc hyperemia and retinal folds. A central scotoma was detected on a perimetry test. Fluorescein angiography indicated mild disc leakage in the late phase. Orbital magnetic resonance imaging disclosed enhancement in the soft tissue of the left retrobulbar area and at the sheath of the left optic nerve. Optical coherence tomography (OCT) revealed subretinal fluid at the fovea, disruption of the ellipsoid zone (EZ), a hyper-reflective band at the outer plexiform layer (OPL), and hyper-reflective foci (HRF) in the inner retinal layer. Pulse therapy was administered, followed by oral steroid tapering. One week later, OCT showed almost complete resolution of subretinal fluid, no HRF, partial improvement in the thickening of the OPL, but persistent EZ disruption. One month later, her vision in the left eye improved to 0.6. Here we present a rare case of AMN associated with orbital inflammation and optic perineuritis, mimicking acute optic neuritis. Although the status of myelin oligodendrocyte glycoprotein (MOG) antibodies in our patient remains uncertain, her ophthalmologic manifestations strongly suggest myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD). This study highlights the importance of OCT examination, especially macular scanning, for establishing a proper diagnosis.
Acute Macular Outer Retinopathy and Acute Vision Loss Associated With Electrolyte Derangements and Rapid Repletion.
Purpose: To describe a unique case of acute macular outer retinopathy presenting immediately after rapid correction of severe hypokalemia and hypomagnesemia, and to explore the potential association between electrolyte repletion and outer retinal injury. Methods: A single clinical case was retrospectively reviewed. Results: A 29-year-old woman presented with acute macular outer retinopathy and vision loss immediately following rapid intravenous electrolyte replacement. Optical coherence tomography revealed disruption of the outer retinal layers, and microperimetry demonstrated a corresponding decrease in retinal sensitivity. The patient was monitored over 5 months without intervention and achieved visual stability with partial improvement of outer retinal structure on imaging. Conclusions: This case highlights a potential relationship between rapid variations in systemic electrolyte concentrations and the development of acute macular outer retinopathy. The findings emphasize the importance of caution during rapid electrolyte repletion and suggest that multimodal retinal imaging may be valuable in monitoring visual symptoms that arise during or after electrolyte correction.
