A Síndrome do encarceramento ou síndrome de locked-in (LiS) é uma doença complexa que se apresenta com quadriplegia, paralisia bulbar e perda sensorial de todo o corpo devido a lesões no tronco cerebral, mais frequentemente na pons anterior. A cognição, o movimento ocular vertical, o pestanejar e a audição estão classicamente preservados nos doentes que sofrem desta doença. O diagnóstico da síndrome de locked-in é muitas vezes difícil devido à sua semelhança com condições como o mutismo acinético e o coma. O diagnóstico pode demorar semanas e os familiares que se encontram à cabeceira da cama são normalmente os primeiros a aperceberem-se da diminuição das funções motoras do doente.

mTORC1 inhibition by sirolimus as adjunctive treatment in experimental pneumococcal meningitis.
Sirolimus, an inhibitor of mammalian target of rapamycin complex (mTORC) 1, a regulatory protein involved in seizures and a modulator of inflammation, represents a potential new treatment strategy for pneumococcal meningitis. This study investigates the effects of mTORC1 inhibition using sirolimus in a mouse model of experimental pneumococcal meningitis. In a prospective, investigator-blinded and randomized trial, 96 mice were infected intracisternally with Streptococcus pneumoniae serotype 2. Mice were randomized for treatment with sirolimus and all mice received antibiotic treatment. In a clinical severity experiment, 48 mice were infected and scored every 4 h for disease severity until 72 h. In a time-point experiment, 48 mice were infected and terminated at 6 or 24 h after infection for evaluation. In the clinical severity experiment, sirolimus treatment did not improve survival, although seizures tended to occur less in treated mice [3 of 24 (13%) versus 5 of 24 (21%), P = 0.7]. In the time point experiment, clinical severity scores were increased in sirolimus-treated mice [maximum difference at 24 h after infection with a median of 10 (interquartile range 10-13) versus 7.5 (interquartile range 5-9) at 24 h after infection, P < 0.01]. While bacterial loads were similar across groups, sirolimus treatment increased inflammation. In the brain of treated mice, interleukin 6 was increased (median 8900 pg/ml versus 4804 pg/ml, P = 0.04), while interleukin 12 was elevated in both the brain (median 591 pg/ml versus 405 pg/ml, P = 0.03) and plasma (median 216 pg/ml versus 7 pg/ml, P < 0.01). Our findings illustrate the importance of mTORC1 signalling during pneumococcal meningitis in regulating inflammation. However, our results indicate that sirolimus is unlikely to be an effective treatment for this condition.
Thoughts on Fluoroquinolones in Resistant Pneumococcal Meningitis.
Asplenia refers to the absence of normal splenic function and may be anatomic or functional secondary to various disease states. The spleen, a major extramedullary lymphoid organ in the left upper quadrant, is typically nonpalpable and averages 10.6 by 5.2 cm, with size varying by sex and race (eg, larger in men and in White individuals than in women or Black individuals). Palpability or radiographic enlargement should prompt evaluation for inherited or acquired causes of splenomegaly. Patients receiving splenic irradiation, particularly 40 Gy or greater, are also at increased risk for developing hypo or asplenia. Functionally, the spleen filters senescent red blood cells, mounts immune responses to infectious pathogens (particularly encapsulated organisms), and supports extramedullary hematopoiesis. The spleen's 2 distinct regions, white pulp and red pulp, mediate these roles: the white pulp generates antigen-specific antibodies, while the red pulp, composed of sinusoids and the cords of Bilroth, filters blood and removes aged cells and circulating microorganisms. As the body's largest blood-filtering organ, the spleen plays a central role in the broader lymphatic system, which includes the lymph nodes, thymus, tonsils, and appendix. Asplenia may result from damage to the white pulp, red pulp, or both, and can arise from disease-related injury, infection, or direct toxic insults. Understanding its etiologies and complications is essential as asplenia significantly increases susceptibility to encapsulated bacteria. Infections with Neisseria meningitides or Streptococcal pneumoniae, and other pathogens can rapidly become life-threatening, and asplenic patients have a reported 200-fold higher risk of death from septicemia than individuals with normal splenic function. Numerous case reports describe fatal outcomes, including pneumococcal meningitis, following splenectomy for refractory immune thrombocytopenia. Mortality in invasive infections remains high at approximately 69% in meningitis, 64% in septicemia, and 7% in purpura fulminans. Invasive group A streptococcal infections, including endocarditis with cardiac rupture, have also been reported. Emerging data from the COVID-19 pandemic indicate that aspenic patients experience higher rates of hospitalization, though not necessarily increased mortality. These findings highlight the importance of vigilant monitoring and timely vaccination in this high-risk population.

Concurrent Detection of Streptococcus pneumoniae and Herpes Simplex Virus Type 1 (HSV-1) in Cerebrospinal Fluid: Clinical Interpretation Beyond Molecular Positivity.
Herpes simplex virus type 1 (HSV-1) encephalitis and acute bacterial meningitis are severe central nervous system infections associated with significant morbidity. While typically distinct, the increasing use of multiplex polymerase chain reaction (PCR) panels has led to occasional co-detection, creating diagnostic uncertainty. We describe a 44-year-old previously healthy male diagnosed with culture-confirmed pneumococcal meningitis in whom multiplex cerebrospinal fluid PCR also detected HSV-1. The clinical course was complicated by a generalized seizure on hospital day 3 and the development of ventriculitis on day 14, despite negative repeat microbiological assays. Following targeted antibacterial and antiviral therapy, the patient achieved complete neurological recovery. This case highlights the complexity of interpreting molecular results in the setting of severe blood-brain barrier disruption and emphasizes a cautious, chronologically driven approach to management.

Streptococcus pneumoniae (pneumococcus) is a leading cause of bacterial meningitis worldwide and is associated with cerebrovascular complications and long-term neurological sequelae. These outcomes are largely driven by an excessive yet ineffective neuroinflammatory response. Although pneumococci express multiple virulence factors that enable immune evasion, how these factors modulate phagocytic responses in the central nervous system remains unclear. Here, we identify neuraminidase A (NanA) as a critical regulator of phagocytic activation in microglia and macrophages. Using murine and human microglia and RAW 264.7 macrophages, we show that NanA deficiency increases bacterial adhesion, indicating enhanced immune recognition, but paradoxically promotes intracellular bacterial survival. Despite elevated expression of the phagocytic marker CD68, microglia infected with NanA-deficient pneumococci fail to efficiently eliminate intracellular bacteria, as evidenced by unchanged levels of the lysosomal enzyme cathepsin 3 compared with NanA-expressing strains. Treatment with recombinant NanA restored phagocytic activation and significantly enhanced clearance of NanA-deficient bacteria. This effect was dependent on NanA sialidase activity, as enzymatically inactive NanA failed to restore immune sensing. Notably, this immunomodulatory function was specific to the secreted form of NanA produced by the invasive TIGR4 strain and was not observed with membrane-anchored NanA from D39-derived mutants. Together, these findings reveal an unexpected role for NanA in promoting effective innate immune activation and bacterial clearance, highlighting its potential as a therapeutic target for pneumococcal meningitis and invasive pneumococcal disease.
