Em vertebrados, sabe-se que o NOI coordena sinais da medula espinhal para o cerebelo, a fim de regular a coordenação motora e o aprendizado. Foi demonstrado que essas conexões são intimamente associadas, pois a degeneração do cerebelo ou do NOI resulta na degeneração do outro.

Post-traumatic Hypertrophic Olivary Degeneration with Palatal Myoclonus-A Case Report.
This is a case report of a 7-year-old boy with a medical history of a motor vehicle accident (MVA) 2 years previously. Two years post-MVA, he developed quivering and twitching of the right side of his lip, palate, and neck. Multiple antiseizure medications were attempted with no effect. However, carbamazepine was started with marked improvement. The most recent brain magnetic resonance imaging showed findings suggestive of hypertrophic olivary degeneration (HOD). Electroencephalography captured the abnormal facial twitching with audible palatal clicking, but was not associated with electrographic changes. This case demonstrates the anatomic importance of the Guillain-Mollaret triangle, as traumatic disruption of this pathway resulted in palatal myoclonus. HOD occurring due to trauma is an incredibly rare etiology, with only 1 other case reported in the literature in a 27-year-old man, with no previous reports of posttraumatic HOD occurring in an individual under 18 discovered during our literature review.
Hypertrophic olivary degeneration mimicking tumor recurrence after brainstem radiosurgery: A diagnostic pitfall in neuro-oncology.
Hypertrophic olivary degeneration (HOD) is a rare trans-synaptic degeneration of the inferior olivary nucleus resulting from disruption of the dentato-rubro-olivary pathway, also known as the Guillain- Mollaret triangle. Unlike most degenerative processes, it produces olivary hypertrophy rather than atrophy. In oncology patients with prior posterior fossa or brainstem involvement, this condition can simulate tumor recurrence or radiation necrosis, leading to unnecessary interventions if unrecognized. A 38-year-old woman with a history of human epidermal growth factor receptor 2-positive invasive ductal carcinoma of the breast and previously treated brainstem metastasis presented with progressive right facial paresthesias and gait instability. Neurological examination revealed dysmetria, impaired rapid alternating movements, and gait ataxia without motor or sensory deficits. Magnetic resonance imaging (MRI) demonstrated a nodular enhancing pontine lesion due to the treated metastatic lesion and a new enlargement involving the rostral aspect of the left central tegmental tract with associated hypertrophic change of the inferior olivary nucleus, consistent with unilateral HOD. Magnetic resonance spectroscopy and perfusion imaging excluded neoplastic recurrence. A multidisciplinary neuro-oncology board concluded that the findings were secondary to radiation-induced tract injury. The patient continued maintenance chemotherapy and received symptomatic management and rehabilitative therapy, with serial MRI showing stability. HOD should be considered in patients with prior brainstem surgery or radiotherapy who develop delayed cerebellar signs and characteristic MRI findings. Recognizing its benign and self-limited nature is crucial to prevent misdiagnosis as tumor recurrence and avoid unnecessary oncologic or surgical interventions. Integration of advanced MRI techniques and multidisciplinary evaluation allows precise diagnosis and tailored management, ensuring appropriate treatment and long-term follow-up.
Inferior olive lesions in autoimmune glial fibrillary acidic protein astrocytopathy: a report of two cases.
Autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy is a newly identified inflammatory nervous system disorder characterized by Immunoglobulin G autoantibodies against GFAP. While several MRI features are considered typical for autoimmune GFAP astrocytopathy, reports of lesions affecting the inferior olivary nucleus (ION) are rare. In this context, we present two cases of autoimmune GFAP astrocytopathy with lesions in the ION. Case 1 involved a 64-year-old man with meningoencephalomyelitis, peripheral neuropathy, and autonomic dysfunction. His brain MRI revealed bilateral ION lesions and changes along the dentato–rubro–olivary pathway, likely indicating hypertrophic olivary degeneration (HOD). Spinal cord MRI showed bilateral T2 hyperintensities in the anterior horns and a longitudinally extensive spinal cord lesion. In Case 2, a 60-year-old man presented with meningoradiculitis. Although he lacked encephalitic symptoms, his brain MRI showed a left ION lesion with radial perivascular enhancement in the periventricular white matter. The olivary lesion was not linked to other abnormalities along the dentato–rubro–olivary pathway, and contrast enhancement appeared in the same region before the T2 lesion was visible. These findings imply that the inferior olivary involvement in Case 2 was a primary inflammatory lesion. Palatal tremor was not observed in either case. In autoimmune GFAP astrocytopathy, lesions in the ION can appear either as a primary inflammatory response or as HOD resulting from disruption of the dentato–rubro–olivary pathway. When ION lesions are detected during an acute phase that suggests inflammation, autoimmune GFAP astrocytopathy should be included in the differential diagnosis.
Hypertrophic olivary degeneration in a patient with lung cancer.
A 78-year-old woman developed a coarse tremor in her left hand that had persisted for 2 months. 3 years before, she had undergone surgery for lung adenocarcinoma, followed by chemotherapy. On examination, there was a postural and action tremor (3-4 Hz) in the left upper limb, identified as a Holmes tremor, with oculopalatal myoclonus. MR scan of the brain showed bilateral hypertrophic olivary degeneration with hyperintensity of the middle cerebellar peduncles. CT scan of the head showed a calcified lesion in the pontine tegmentum, suggesting disruption of the Guillain-Mollaret triangle, which comprises the dentatorubral, rubro-olivary and olivocerebellar pathways. The underlying cause was a pontine metastasis, evidenced by calcification on CT brain scan. This case highlights the importance of considering underlying malignancy in people with hypertrophic olivary degeneration or related symptoms and emphasises the role of CT brain scanning in identifying metastatic calcification when MR scan findings are inconclusive.
Bilateral Hypertrophic Olivary Degeneration Following Unilateral Mesencephalic Hemorrhage.
After a unilateral midbrain hemorrhage, clinicians should recognize that bilateral hypertrophic olivary degeneration may occur, presenting as palatal myoclonus, dysarthria, or tinnitus. This highlights the necessity for timely diagnosis and effective symptom management.
