Cor Triatriatum Dexter: The Largest Comprehensive Review in the Field on 124 Worldwide Cases (1968-Now).
Cor triatriatum dexter (CTD) is a rare congenital heart defect where a membrane divides the right atrium into two chambers, resulting from the incomplete regression of the right valve of the sinus venosus. Due to its rarity, only individual case reports and a limited number of case series have been published to date. This study constitutes the most extensive comprehensive review conducted in this area. Eight factors were evaluated: age at diagnosis, sex, clinical presentation, electrocardiographic findings, imaging (ultrasound, CT, or MRI), associated cardiac anomalies, and patient outcomes. The electronic databases PubMed and Scopus were searched from their inception until 30 October 2025. Only case reports and case series were considered for inclusion. Studies involving foetuses, autopsies, and animals were excluded. The collected data were primarily presented as percentages. One hundred fourteen studies were found encompassing 124 patients. The mean age at diagnosis was 33.3 ± 9.4 years The most common clinical presentations were dyspnoea (44.3%) and cyanosis (29.5%). The most commonly encountered ECG changes were supraventricular tachycardia/atrial flutter/atrial fibrillation (33.3%) and right bundle branch block (22.6%). On chest X-ray, cardiomegaly was noted in 46.5%. CTD was suspected or diagnosed by echocardiography in 95.2% of cases. The diagnosis was confirmed by CT and/or MRI in 34.1% of cases. A concomitant congenital heart defect was found in 67.7%, especially in the form of all kinds of atrial septal defect (38.1%) and of right valvular and right ventricular involvement (20.1%). An outcome was reported in 97/124. Surgical correction was the treatment of choice in 51.6%. Since 1991, a percutaneous approach has been employed in selected cases (5.1%). Conservative management was the treatment of choice in 43.3%. The mortality rate was 8.2%. The principal limitation of this systematic review lies in its reliance solely on case reports and small case series, reflecting the absence of large-scale studies on CTD. Nonetheless, it constitutes the most comprehensive analysis available to date.
Presumed atypical cor triatriatum dexter in a geriatric dog: A case report.
Membranous structures within the right atrium (RA), such as the Chiari network, eustachian valve, Thebesian valve, and cor triatriatum dexter (CTD), can present with overlapping imaging features and complicated diagnoses. A 17-year-old male Maltese presented with a cardiac murmur, cough, anorexia, and exercise intolerance. Echocardiography revealed a mobile membrane in the RA, dividing it into two chambers. Computed tomography (CT) confirmed this finding, demonstrating differential attenuation of contrast between the chambers. The vascular connections and anatomical relationships of the structure differed from those of previously reported CTD types. Although other sinus venosus valve remnants were considered, the findings supported a provisional diagnosis of atypical CTD. This case highlights the utility of multimodal imaging for the characterisation of right atrial membranous structures. In human medicine, a detailed anatomical comparison of sinus venosus valve remnants aids in the differential diagnosis. Applying a similar approach in veterinary medicine, especially in geriatric patients, may improve diagnostic accuracy. Comprehensive imaging evaluations, including echocardiography and CT, are recommended to assess RA membranes that may be misinterpreted as variants of the CTD or other embryological remnants.
Cor Triatriatum Dexter: Embryology, Presentation and Management.
Cor triatriatum dexter (CTD) is a rare congenital cardiac anomaly resulting from persistence of the right valve of the sinus venosus. Clinical presentation varies from neonatal hypoxemia to incidental diagnosis in adulthood. Optimal management strategies remain poorly defined due to limited published literature. We report a single-center series of four patients diagnosed with CTD. Clinical presentation, echocardiographic findings, surgical management, and postoperative outcomes were retrospectively reviewed. All four patients presented in the neonatal period with hypoxemia and failed critical congenital heart disease (CCHD) pulse oximetry screening, ultimately resulting in echocardiographic diagnosis of a redundant membrane within the right atrium consistent with CTD. All four patients underwent surgical repair at an average of 21 days-of-life (5-41 DOL) and 4 kg (3.4-4.5 kg). Mean cardiopulmonary bypass time was 33 minutes (14-44 min). Mean postoperative length of stay was 6 days (3-9 days). Follow-up was available in 3 patients, all of whom were doing well with no cardiac symptoms and on no cardiac medications up to 7 years postoperatively. CTD repair can be performed with a relatively short cardiopulmonary bypass time, limited postoperative hospital stay, and no cardiac concerns at medium term follow-up. We advocate for an individualized approach when deciding surgical intervention vs. watchful waiting but generally err on the side of early surgical repair.

Congenital heart diseases (CHDs) are structural anomalies present at birth, resulting from aberrant embryonic development. In veterinary medicine, CHDs are an important reason for disease and death in both dogs and cats. Their prevalence varies across species and breeds, with certain defects being more common in specific breeds due to genetic predisposition. In this article, we will focus specifically on cor triatriatum dexter (CTD), an uncommon congenital defect that involves the division of the right atrium into two chambers by a fibromuscular membrane. This malformation can have serious consequences, depending on the severity of the obstruction created by the membrane, and the presence of the often-associated congenital anomalies. It can range from mild, clinically silent anomalies to severe defects that result in heart failure, cyanosis, or sudden death. The present review aims to (i) explore the pathophysiology of CTD in dogs; (ii) outline the clinical scenario underlying the increased interest in CHDs by overviewing the evolution of appropriate diagnostic techniques and the treatment options available; and (iii) provide a discussion from a medical-legal and ethical point of view.
