A síndrome de Loeys-Dietz (SLD) é uma doença genética autossômica dominante que afeta o tecido conjuntivo. Apresenta características semelhantes à síndrome de Marfan e à síndrome de Ehlers-Danlos. A doença é caracterizada por aneurismas na aorta, frequentemente em crianças, e a aorta também pode sofrer dissecção súbita nas camadas enfraquecidas da parede da aorta. Aneurismas e dissecções também podem ocorrer em artérias além da aorta.

[A case of cryptogenic stroke with platypnea-orthodeoxia syndrome and paroxysmal atrial fibrillation].
The patient was a 72-year-old man who presented to our hospital with gait disturbance. He had sustained a spinal fracture one year earlier. Neurological examination revealed left-sided hemiparesis, dysarthria, and a National Institutes of Health Stroke Scale (NIHSS) score of 3. Brain MRI demonstrated cortical infarctions in both cerebral hemispheres. Transesophageal echocardiography revealed a grade 3 patent foramen ovale (PFO) accompanied by an atrial septal aneurysm. He exhibited hypoxemia in the upright position, which improved when supine, leading to a diagnosis of platypnea-orthodeoxia syndrome (POS). He underwent percutaneous PFO closure and was subsequently discharged home. Fifteen weeks after surgery, left-sided paralysis was observed, and paroxysmal atrial fibrillation was detected, leading to a diagnosis of cardioembolic stroke. A PFO, which had been asymptomatic, contributed to the development of POS due to its impact on thoracic compression fractures and atherosclerosis. In the care of elderly patients, the coexistence of multiple conditions is not uncommon, necessitating careful interpretation of test results.
Percutaneous Patent Foramen Ovale Closure and Aneurysm Elimination by Direct Suture.
Patent foramen ovale (PFO) with significant atrial septal aneurysm (ASA) may pose technical challenges. In selected cases, direct-suture closure may represent an effective alternative to close the PFO and reduce aneurysmal redundancy. A 54-year-old woman with recurrent transient ischemic episodes and hyperintensity changes on magnetic resonance imaging was referred for percutaneous PFO closure. Transesophageal echocardiography demonstrated right-to-left shunt and markedly aneurysmal interatrial septum with constant rightward displacement. Under echocardiographic guidance, the septum was displaced leftward using a transseptal sheath, allowing wire passage. The PFO was subsequently closed with direct sutures, eliminating the ASA by creating a loop on the right atrial side. Direct-suture closure can be considered even in the presence of large ASA, as it not only allows effective PFO closure but also reduction of aneurysmal redundancy. Direct-suture percutaneous PFO closure can successfully and safely be performed in patients with ASA.
Correlation Between Patent Foramen Ovale Morphological Characteristics and Right-to-Left Shunt Severity in Stroke Patients with High RoPE Scores.
To evaluate the association between Risk of Paradoxical Embolism (RoPE) score, patent foramen ovale (PFO) morphological characteristics, and right-to-left shunt severity in ischemic stroke patients with high RoPE scores. In this prospective observational study, 162 ischemic stroke patients with confirmed PFO and RoPE scores ≥7 were enrolled. All patients underwent contrast-enhanced transcranial Doppler, transthoracic echocardiography, and transesophageal echocardiography. PFO morphology (size, tunnel length, atrial septal aneurysm, Eustachian valve, Chiari network) and shunt severity were assessed. Associations between RoPE score, its components, and PFO characteristics were analyzed. Patients with higher RoPE scores (9-10) showed a greater prevalence of complex PFO than those with scores of 7-8 (71.2% vs. 45.4%, p<0.001). RoPE score independently predicted complex PFO morphology (adjusted OR 2.48, 95% CI 1.68-3.67). Younger age and absence of hypertension were the strongest contributing factors. RoPE score correlated positively with shunt severity (ρ=0.58, p<0.001), with severe shunting more frequent in patients with higher scores. Higher RoPE scores are strongly associated with complex PFO anatomy and severe right-to-left shunting. Integrating detailed PFO assessment with clinical risk stratification may improve individualized management of PFO-associated stroke.
Platypnea-orthodeoxia syndrome treated by percutaneous patent foramen ovale closure: a case report and review of the literature.
Platypnea-orthodeoxia syndrome is a rare but striking clinical syndrome, usually characterized by dyspnea and hypoxemia in the upright position, which is relieved by recumbency. However, the exact prevalence remains unknown. Patent foramen ovale is most commonly associated with this syndrome. Although patent foramen ovale is a common condition, the incidence of platypnea-orthodeoxia syndrome in patients with patent foramen ovale is relatively low. Here, we report a case of a 68-year-old Chinese man with platypnea-orthodeoxia syndrome due to patent foramen ovale without pulmonary hypertension who was successfully treated with transcatheter closure. A 68-year-old Chinese man presented to the emergency department with acute-onset dyspnea and hypoxemia without significant precipitating factors. Tachycardia and tachypnea were observed after the initial assessment. Transthoracic echocardiography revealed an enlarged aortic root (42 mm), mild aortic valve regurgitation, and impaired left ventricular diastolic function. No significant additional findings were observed. The patient was subsequently admitted to the Respiratory Medicine Department for further investigation. Comprehensive clinical and laboratory reevaluations revealed postural hypoxemia and dyspnea. Arterial blood gases in the upright and supine positions were as follows: partial pressure of arterial oxygen = 33/69 mmHg, partial pressure of arterial carbon dioxide = 17/22 mmHg, pH = 7.54/7.48, HCO3- = 14.5/16.4 mmol/L, and arterial oxygen saturation = 73/95% (1 minute after stopping oxygen supplementation), suggesting a provisional diagnosis of platypnea-orthodeoxia syndrome. Further evaluation using transesophageal echocardiography identified patent foramen ovale. Agitated saline transthoracic echocardiography confirmed patent foramen ovale with an atrial septal aneurysm and a right-to-left intracardiac shunt within 3-6 cardiac cycles, establishing a diagnosis of platypnea-orthodeoxia syndrome. Percutaneous patent foramen ovale closure was successfully performed. After the procedure, no residual shunting was observed, and platypnea-orthodeoxia syndrome symptoms significantly improved. Currently, the patient is in good general condition, and no breathlessness is observed. A high index of suspicion for platypnea-orthodeoxia syndrome is recommended for elderly patients with unexplained hypoxia and shortness of breath. Identification of the underlying mechanisms of platypnea-orthodeoxia syndrome will help determine future therapeutic options. An enlarged aortic root may be the most likely cause of right-to-left shunting in patients with patent-foramen-ovale-induced platypnea-orthodeoxia syndrome. More attention should be paid to aortic root size.
The Impact of Intravenous Agitated Saline Contrast Echocardiography on Subclinical Neuronal Injury Determined By Neuron-Specific Enolase.
Patent foramen ovale (PFO) is a congenital defect of the interatrial septum with 25%-30% prevalence. Diagnostic echocardiography involves the injection of agitated saline, and microbubble passage from the right to the left atrium is monitored. The reliability of this method has been questioned in case series, with a low incidence (0.062%) of generally transient, mild clinical cerebrovascular events. This study aimed to examine whether transthoracic echocardiography, which was performed on patients with PFO and atrial septal aneurysm (ASA) without neurological history, and healthy controls can cause silent neuronal injury by evaluating neuronspecific enolase (NSE) with saline contrast echocardiography (SCE). Fifty-two patients who underwent SCE for color flow through the interatrial septum or prominent interatrial septal aneurysm and were diagnosed with PFO were included. Fifty-one control patients without PFO or ASA were included. Serum samples were collected from the patients before and 12 hours after the SCE test, and NSE levels were measured by Enzyme Linked Immunosorbent Assay (ELISA). Median baseline NSE levels were 6.4 (2.8-9.6) ng/mL in the PFO/ASA group and 5.3 (3.6-9.1) ng/mL in controls. At 12 hours, median NSE levels remained similar (6.6 (3.8-11.5) vs. 5.8 (4.6-9.6) ng/mL; P = .90). The median change in NSE (ΔNSE) did not differ between groups [1.19 (-1.01 to 2.23) vs. 0.73 (-1.16 to 2.25) ng/mL; P = .58]. There was no relationship between age, presence of interatrial septal aneurysm, mitral annulus calcification, number of bubbles passed in the SCE test, and baseline and 12-h change in NSE levels (P = .926). Neuronspecific enolase level changes were found to be correlated between groups (P < .001). This study is the first to evaluate whether SCE causes silent neuronal injury. It is suggested that when evaluated with NSE, the SCE method is safe and does not cause silent neuronal injury in patients without underlying neuronal susceptibility.
