A malignant tumor of plasma cells usually arising in the bone marrow and characterized by diffuse involvement of the skeletal system, hyperglobulinemia, Bence-Jones proteinuria and anemia. Complications of multiple myeloma are bone pain, hypercalcemia, renal failure and spinal cord compression. The aberrant antibodies that are produced lead to impaired humoral immunity and patients have a high prevalence of infection. Amyloidosis may develop in some patients. Multiple myeloma is part of a spectrum of diseases ranging from monoclonal gammopathy of unknown significance (MGUS) to plasma cell leukemia.

Clinical experience with metagenomic next-generation sequencing (mNGS) for the detection of Tropheryma whipplei in respiratory specimens: A multicenter retrospective observational study.
Tropheryma whipplei (T. whipplei) is the causative bacterium of Whipple's disease (WD), a chronic and systemic infectious condition that predominantly affects the gastrointestinal tract. Sporadic cases of T. whipplei pneumonia have been documented recently. This multicenter retrospective observational study was conducted on patients with T. whipplei positive respiratory specimens admitted to Peking University People's Hospital and China-Japan Friendship Hospital, from Apr 2021 to Jul 2024. Metagenomic next-Generation sequencing (mNGS) was performed using the patient's bronchoalveolar lavage fluid (BALF), and the quantitative polymerase chain reaction (qPCR) of T. whipplei was also adopted. The clinical data of patients were systematically evaluated. Among 91 patients (aged 25-82, mean 57; 48% male), common symptoms included cough (60%), expectoration (48%), dyspnea (42%), and fever (30%). Notably, 22% were asymptomatic. Besides, 20 patients (22%) had a pre-existing condition of interstitial lung disease. Among all 91 patients, 14 were diagnosed with pneumonia, while the remaining 77 had bacterial colonization. Pneumonia cases showed higher T. whipplei mNGS reads than colonization (P = 0.0298). Samples testing positive for T. whipplei by qPCR exhibited significantly higher mNGS sequence reads compared to qPCR-negative samples (P < 0.0001). All pneumonia patients received antibiotics therapy tailored to their condition. One died from respiratory failure, while the remaining 13 recovered. The application of mNGS on respiratory specimens stands as an exceptional diagnostic modality, proficient in identifying rare microbial infections, exemplified by those induced by T. whipplei. Future research should launch prospective trials to optimize regimens, assess outcomes, and track long-term survival precisely.
Deciphering whipple's disease complexity.
Whipple's disease is a sporadic infectious condition, with an incidence rate of approximately 1 per million individuals. The causative agent is the gram-positive bacterium Tropheryma whipplei. The disease manifests with a wide range of clinical symptoms, including non-specific presentations such as diarrhea, arthralgia, and fever, as well as the more pathognomonic lipodystrophy. This diversity in presentation poses a significant diagnostic challenge even for experienced clinicians. Our review aims to provide an updated overview encompassing the latest insights into Whipple's disease, focusing on epidemiology, pathophysiology, genetic predisposition, clinical manifestations, diagnosis, immune reconstitution inflammatory syndrome, and treatment. Herein, we have additionally explored many of the confounding factors in the diagnosis and management of Whipple's disease, including the variable presentations among patients colonized by Trophyrema whipplei as well as the limitations of current treatment options, and underscore the need for further research and guidelines related to this complex disease process.
Whipple's Disease: Panniculitis Among Other Dermatoses. A Case Report and Literature Review.
Whipple's disease (WD) is a rare infectious disease caused by Tropheryma whipplei, which primarily affects the gastrointestinal tract but may also involve the joints, central nervous system, and skin. Although cutaneous manifestations are often overlooked, they can provide valuable diagnostic clues. We report the case of a 54-year-old male with long-standing seronegative migratory polyarthritis refractory to conventional treatment, who subsequently developed multiple subcutaneous inflammatory nodules and systemic symptoms. Histopathological examination of these lesions revealed a non-vasculitic, septal-predominant panniculitis with foamy macrophages containing periodic acid-Schiff (PAS)-positive diastase-resistant intracytoplasmic inclusions, raising a strong suspicion of Whipple's disease. Subsequent duodenal biopsies confirmed the diagnosis by electron microscopy and polymerase chain reaction (PCR) for T. whipplei. Panniculitis in WD represents a highly specific but rarely reported finding, characterized by thickened subcutaneous septa with partial lobular involvement, foamy macrophage infiltration, and absence of vasculitis. A literature review identified 15 cases of histologically confirmed WD-associated panniculitis, suggesting that this manifestation may be under-recognized in clinical practice. This study underscores the importance of cutaneous pathology in WD and expands current knowledge on its dermatological manifestations by providing clinicopathologic, ultrastructural, and literature-based data.
Tropheryma whipplei Infection Presenting Initially with Chest Pain.
Tropheryma whipplei is a rare pathogenic bacterium, a Gram-positive actinomycete widely distributed in the natural environment. It mainly infects individuals with immunodeficiency, but is rarely observed in individuals with normal immune function. Most patients present with non-specific symptoms, and since it is not feasible to culture the bacteria using conventional methods, diagnosing this infection is very difficult. In this case, next-generation sequencing (NGS) detection via bronchoscopy confirmed the final diagnosis of Tropheryma whipplei infection. Bronchoscopy and next-generation sequencing were used. Through bronchoscopy, NGS of bronchoalveolar lavage fluid indicated the presence of Tropheryma whipplei. Therefore, anti-infective treatment was administered. For cases of long-standing chest pain of unknown origin with abnormal findings on chest imaging, it is crucial to stabilize the patient's condition while identifying the underlying cause. Prompt implementation of relevant diagnostic procedures, such as bronchoscopy, is essential for establishing a diagnosis.
Atypical Migrating Nummular Retinal Lesions in an Immunosuppressed Patient With Whipple Disease.
Whipple disease is a rare systemic infection of Tropheryma whipplei with nonspecific clinical features. Ocular involvement is uncommon but has been reported in both anterior and posterior segments of the eye. We present the case of a 70-year-old man with a history of cutaneous T-cell lymphoma, who developed unilateral ocular inflammation with hypopyon, vitritis, and migrating nummular retinal lesions. Despite intravitreal antimicrobial therapy, inflammation persisted. Four months later, he developed systemic symptoms and findings on duodenal biopsy consistent with Whipple disease. This case highlights an atypical ocular presentation suspected to be associated with Whipple disease and the diagnostic challenges in immunocompromised patients lacking classical gastrointestinal symptoms.
