Fulminant Rhabdomyolysis and Acute Kidney Injury Associated With Pontiac Fever: A Case Report.
Legionella pneumophila typically manifests in two distinct clinical forms: Legionnaires' disease, characterized by severe pneumonia, and Pontiac fever (PF), a non-pneumonic, influenza-like illness that is usually self-limiting within 2-5 days. While PF is characterized by a high attack rate among exposed individuals in outbreak settings, its true incidence remains unknown and is likely underdiagnosed. Consequently, in routine surveillance data, Legionnaires' disease accounts for the vast majority of reported Legionella infections, with PF representing only a negligible fraction. We report a rare case of a 24-year-old man who presented with high fever, myalgia, and gastrointestinal distress, subsequently developing fulminant rhabdomyolysis. Laboratory findings were remarkable for a peak creatine kinase (CK) level of 737,860 U/L and Kidney Disease: Improving Global Outcomes (KDIGO) stage III acute kidney injury (AKI) requiring renal replacement therapy. Despite the complete absence of pulmonary involvement or hypoxemia, serological testing performed during outpatient follow-up retrospectively confirmed Legionella infection. This case emphasizes that PF, despite its reputation as a mild illness, can rarely precipitate life-threatening systemic muscle destruction and multi-organ dysfunction.
Frequency of rhabdomyolysis in Legionella infection and associated outcomes: a retrospective cross-sectional study.
Legionella is a common cause of community-acquired pneumonia and can affect multiple organ systems. An association with rhabdomyolysis and acute kidney injury (AKI) have been noted for several decades, but population-based studies are limited and could aid in better understanding the prevalence, associated risk factors, and clinical significance of this complication. We performed a retrospective manual review of the electronic health record on 323 patients with legionellosis identified on the basis of urinary antigen or culture in a large, urban hospital system. Demographic and clinical data were extracted by four physician reviewers. Rhabdomyolysis was assessed on the basis of creatine kinase (CK) values ≥1,000 IU/L or suggestive urinalysis findings. AKI was classified according to Kidney Disease: Improving Global Outcomes (KDIGO) guidelines. Patients were excluded for missing data or potential alternative causes of rhabdomyolysis. Data were analyzed using appropriate parametric and non-parametric statistical tests and multivariate logistic regression analysis. A total of 210 patients were included in the analysis. Fifty patients (23.8%) had evidence of rhabdomyolysis, 20% of whom had severe rhabdomyolysis (CK ≥15,000 IU/L). Rhabdomyolysis was associated with male sex, black race, and age <65. Common comorbidities were not associated with rhabdomyolysis. Rhabdomyolysis was associated with AKI, including for mild cases (CK 1000-5000 IU/L), with longer hospital length of stay, and with higher likelihood of intensive care unit admission, but not with overall mortality, which was 5.1% in the patient cohort. Rhabdomyolysis is a common feature of legionellosis, is associated with AKI, and may be a marker of more severe disease.
Diagnostic methods and therapeutics strategies of Legionella infection in postbone marrow transplantation.
Organ transplantation represents a critical therapeutic intervention for patients with end-stage organ failure or hematological malignancies, often serving as a last-resort treatment. Among these, bone marrow transplantation (BMT) is vital but complex, as it induces profound and long-lasting immunosuppression. Patients undergoing BMT are highly vulnerable to opportunistic infections due to concurrent chemotherapy, radiation, and immunosuppressive therapies. Legionella infections emerge as a significant threat, accounting for considerable morbidity and mortality in hospitalized immunocompromised individuals. These infections often progress rapidly to severe pneumonia, with high mortality rates compared to those infecting immunocompetent people. Early and accurate diagnosis remains challenging due to nonspecific clinical presentations and limitations of conventional microbiological methods. Consequently, timely detection using advanced diagnostic tools and therapeutic intervention is necessary. This comprehensive review critically observes the epidemiology, risk factors, diagnostic methods, clinical manifestations, and treatments of Legionella in BMT recipients. It emphasizes the need for institutional prevention protocols to alleviate the exposure risks to reduce the burden of Legionella-related complications in high-risk BMT recipients.
A rare cause of dysarthria: Legionnaires' disease.
A case admitted with dysarthria and syncope and hyperintense lesion in the splenium of the corpus callosum, diagnosed as Legionnaires' disease by detecting Legionella antigen in the urine, and recovered only with antibiotic treatment is presented because it is rare in the literature. When a 64-year-old female patient was admitted to the emergency department with complaints of sudden loss of consciousness and fainting at home, she had speech impairment, and her pulse oxygen saturation in room air was 88%. In the neurological examination, there was no pathological finding except dysarthria in the patient with no motor deficit. The patient had no respiratory complaints and had a recent travel history. In laboratory examinations, hyponatremia was detected with increased C-reactive protein and liver enzymes. Consolidation was observed in the lower right zone on chest computed tomography. Ampicillin + sulbactam (4x1 gr, intravenous) and clarithromycin (2 × 500 mg orally) were initiated. On cranial magnetic resonance, a hyperintense lesion was observed in the splenium of the corpus callosum in the oval T2 sequence. Legionella pneumophila serogroup 1 antigen in urine was reported as positive. On the fourth day of her hospitalization, the patient, whose CRP and liver enzyme values regressed, her hyponatremia improved, her pulse oxygen saturation increased to 92% in room air, and her speech became comprehensible, was discharged after oral antibiotic treatment was arranged. On the 12th day, the speech ability of the patient completely returned to normal. Legionella infection should be suspected in the presence of pneumonia and corpus callosum splenium lesion.
A Case of Legionella Pneumonia Suspected as Typical Pneumonia After Lung Cancer Surgery, Diagnosed by a Repeat Urinary Legionella Antigen Test.
Legionella pneumophila is a leading cause of severe atypical pneumonia and is associated with high mortality when initial treatment is inadequate. The urinary Legionella antigen test serves as a rapid, first-line diagnostic tool; however, early results may be negative, and antigen shedding can be intermittent. In these situations, timely retesting is essential, especially in postoperative or high-risk patients. We report the case of a 74-year-old man, a former smoker with diabetes and moderate aortic stenosis, who underwent thoracoscopic left lower lobectomy and was discharged on postoperative day (POD) six. He developed a fever on POD 27, accompanied by right upper lobe pneumonia. Outpatient urinary antigen tests for pneumococcus and Legionella were negative, and oral garenoxacin was initiated. However, hypoxemia and inflammation worsened, resulting in urgent admission on POD 30. Broad-spectrum β-lactams (sulbactam/ampicillin, followed by piperacillin/tazobactam) were ineffective. Chest CT revealed enlarging infiltrates and a parapneumonic effusion requiring drainage, despite negative pleural fluid cultures. Given the persistent deterioration of the patient's condition, levofloxacin was added, and the urinary Legionella antigen test (R70829, MIZUHO MEDY Co., Ltd., Tosu, Japan) was repeated on hospital day five (POD 35), which returned positive, confirming Legionella pneumonia. Subsequent targeted therapy with azithromycin (days 6-8 and 13-15), alongside levofloxacin, resulted in clinical improvement. Despite complications during the clinical course, including acute kidney injury necessitating two hemodialysis sessions and a creatine kinase peak of 1,563 U/L, the chest drain was removed on day 21, oxygen was discontinued by day 31, and the patient was transferred to long-term care on day 54. The sequence of renal failure preceding creatine kinase elevation suggests direct renal involvement by L. pneumophila rather than primary rhabdomyolysis. Vigilant retesting enabled timely pathogen-directed therapy and a favorable outcome. This case report highlights that a single negative urinary antigen test does not rule out Legionella infection. We accordingly recommend early repeat antigen testing, ideally within one week, for postoperative pneumonia unresponsive to β-lactams, together with prompt macrolide or fluoroquinolone therapy.
