Inactivated Japanese encephalitis virus vaccination imprints fusion loop-biased antibody responses that are attenuated by repeated live-attenuated dengue vaccination.
Immune imprinting, also known as immune history, is a core aspect of adaptive immunity that influences antibody responses to future antigen exposures. Nevertheless, the impact of sequential flavivirus vaccinations on epitope targeting and antibody activity in humans remains incompletely understood. This question is particularly important in regions where the inactivated Japanese encephalitis virus (JEV) vaccines and the live-attenuated dengue virus (DENV) vaccines are used, as both have been associated with an increased risk of symptomatic dengue infection and severe illness. We studied the impact of prior inactivated JEV IXIARO vaccination and simultaneous vaccination on humoral immunity following live-attenuated dengue CYD-TDV vaccination. Long-term analysis showed that JEV IXIARO priming guides the dengue vaccine-induced antibody response toward conserved fusion loop epitopes (FLEs) of the DENV envelope protein, as indicated by 4G2 FLE-bias. This imprinting was characterized by higher levels of 4G2 FLE-like antibodies, rapid recall responses after dengue vaccination, and broad but low-potency neutralization across dengue serotypes and Zika virus. Notably, 4G2 FLE-focused responses correlated with higher FcγRIIa-mediated antibody-dependent enhancement relative to neutralization potency, suggesting functional effects beyond neutralization. To better understand epitope dominance within the native envelope, we used a structurally defined fusion loop epitope mutant (FLE-mut) envelope dimer assay. Disrupting fusion loop accessibility significantly decreased antibody binding, confirming that FLE-specific antibodies are a major component of the response after sequential vaccination. Importantly, a complete series of live-attenuated dengue vaccine reduced 4G2 FLE bias, encouraged the recruitment of non-fusion-loop epitopes, and lessened FcγRIIa-biased antibody activity. Overall, these results show that vaccination platform, timing, and regimen are critical determinants of epitope dominance and antibody quality following flavivirus vaccination.
Clinico-etiological profile and prognostic factors of acute febrile encephalopathy in children: a prospective study from Indian subcontinent.
Acute febrile encephalopathy (AFE) is a common neurological emergency in all age groups. In tropical regions, long-standing infections such as Japanese encephalitis and bacterial meningitis continue to be major causes of AFE, while changing presentations of familiar infections and newly emerged diseases like COVID-19 are also becoming increasingly notable. To describe various clinical presentations, etiology and mortality of AFE in pediatric age group. This prospective observational study was undertaken in a tertiary care institute of Northern India from April 2022 to April 2023. Children aged 1 month to 18 years presenting to the pediatric emergency department with fever of <2 weeks duration and altered sensorium lasting >24 hours were eligible for inclusion. A total of 100 patients (49% males) with AFE were enrolled. The mean (SD) duration of encephalopathy was 1.39 (0.69) days. The most frequent etiologies were viral encephalitis (17%), dengue encephalopathy (12%), and hepatic encephalopathy (9%). The peak incidence occurred between August and November. The overall case fatality rate was 33%. Under-5 children with severe acute malnutrition (SAM) had the highest fatality rate (56%, 9/16). Among survivors, 16% (11/67) had poor functional outcomes. Higher odds of mortality were observed in patients with a Glasgow coma scale score <8, hypoglycemia at admission, requirement for mechanical ventilation or inotropic support, and presence of multiorgan dysfunction. AFE demonstrates a predictable seasonal pattern and is associated with high mortality and morbidity, particularly in under-5 children with SAM, who represent the most vulnerable group.
A time-varying geospatial model of habitat suitability for Japanese encephalitis virus vectors and vertebrate hosts in Australia.
In the austral summer of 2021-2022, Australia experienced an unprecedented Japanese encephalitis virus (JEV) outbreak, with detections over 3000 km south of previous occurrences. Given the limited knowledge of JEV transmission ecology in Australia, we developed geospatial models of transmission risk to support the public health response. We created time-varying habitat suitability models for suspected mosquito vectors and ardeid hosts using month-scaled occurrence and covariate data from 2000-2023. Ardeid host presence-absence data were obtained from eBird and BirdLife Australia, with habitat suitability estimated using gradient-boosted regression tree models. A national dataset of Culex occurrences was compiled from mosquito surveillance records, literature, and biodiversity databases. Penalised logistic regression was used to model mosquito vector habitat suitability. Vector and host habitat predictions for the outbreak peak in February 2022 were rescaled using JEV infection locations in the public domain to create a combined habitat suitability surface. Our models aligned with detected JEV infections at the continental scale, highlighting transmission suitability across tropical northern Australia and major inland drainage basins in the East. Unlike existing models, we predicted lower suitability along the eastern seaboard, suggesting a delimiting effect of the Great Dividing Range. Our approach provides the most comprehensive and temporally dynamic models for JEV hosts and vectors in Australia, with a significantly larger vector dataset than previous studies. The novel method of rescaling host and vector outputs into a combined surface offers new insights into JEV transmission risk. Favourable conditions were repeated in 2023 with few detected infections, emphasising that JEV ecology in Australia remains poorly understood. This study's results can support improvements in arbovirus surveillance systems, promoting earlier detection of circulating viruses. Increased focus on vector ecology and distributions is crucial for better understanding JEV transmission in Australia.
Isolation and characterization of Japanese encephalitis virus genotype I from pig provides evidence for the presence of the virus in nasal secretion and co-circulation of JEV genotypes in Assam, India.
Japanese encephalitis (JE) is a leading cause of viral encephalitis, among children, in many Asian countries despite the availability of effective vaccines. There are five JE virus genotypes (GI through GV), with GIII being the most prevalent. However, in the past three decades GI has emerged as the dominant genotype across several Asian countries, while the reappearance of GV is a concern due to the reduced cross-neutralization offered by existing GIII-based vaccines. Although both GI and GIII have been reported to co-circulate in India, all previous JEV isolations from pigs have been of the GIII. The objective of the study was to elucidate the JEV genotype diversity among pigs in Assam through molecular and virological investigation. We collected blood, serum and nasal swab samples from apparently healthy pigs as a part of routine disease surveillance in pigs of Kamrup (Rural) district, Assam, India. The samples were processed using standard molecular biology (qRT-PCR, Gene Sequencing, Phylogenetic analysis) and virological techniques (Virus isolation, immunofluorescence, plaque assay) for JE virus detection, isolation and characterization. In this study, we report the first isolation and characterization of a JEV GI from a nasal swab of a naturally infected pig from Assam, India and the isolate was designated as JEV/Pig/Assam/NIVEDI-1/2025 (GI). The identity of the JEV isolate was confirmed by RT-qPCR, phylogeny based on 5'UTR-prM region, full-length envelope protein gene, and immunofluorescence assay. The isolate reached a peak titer of 106.5 TCID50/mL at 72 h post-infection in Porcine stable kidney cells and produced smaller plaques (1.88 ± 0.56 mm) than the reference GIII strain (2.68 ± 0.48 mm) (p < 0.01). The findings underscore that JEV GI is circulating in Assam and there is need for strengthened JEV surveillance in swine to monitor genotype shifts, understand viral evolution, and generate field isolates critical for vaccine evaluation and preparedness against emerging JEV genotypes. The study also demonstrates the feasibility of using nasal swabs for virus detection and isolation thereby providing evidence for the presence of JEV in nasal secretion of naturally infected pigs.
Spatial transcriptomics uncovers vasculature-centered cellular interactions driving Japanese encephalitis progression in a mouse model.
The cellular interactions that drive the neuropathology of most neuroinvasive viruses remain elusive. We used Japanese encephalitis virus (JEV) to infect female BALB/c mice and applied Stereo-seq to simultaneously capture host and viral transcriptomes in situ, thereby constructing a comprehensive spatiotemporal atlas of Japanese encephalitis (JE) pathogenesis. Our analysis pinpoints Ly6c2+ monocytes as the primary virus carrier, the most abundant infiltrating immune cell type, and a major source of IFN-γ in the infected mouse brain. Following infection, Ackr1+ endothelial cell activation is linked to blood-brain barrier disruption and immune cell chemotaxis, particularly Ly6c2+ monocytes. The crosstalk between these two cell types appears to orchestrate the pronounced inflammation and cell death, including pyroptosis and necroptosis, which radiate from the vasculature to different brain regions. Disrupting the activation and interactions of these two cell types may help mitigate JE progression. This study also provides a technical framework for investigating the pathogenesis of other neurotropic viruses.
