Bilateral ovarian fibromas with duplex collecting system and ectopic ureter in an 11-year-old girl: a case report with genetic analysis.
Bilateral ovarian fibromas in children are exceedingly rare. Their coexistence with congenital anomalies of the kidney and urinary tract (CAKUT) has not been reported. An 11-year-old girl presented with abdominal pain and lifelong urinary dribbling. Imaging revealed bilateral ovarian masses and a left duplex collecting system with a dilated upper-pole ureter inserting ectopically into the posterior urethra. Serum tumor markers including CA125 were within normal limits. No ascites or peritoneal deposits were identified. Laparoscopic bilateral tumor excision preserved normal ovarian tissue; histopathology confirmed ovarian fibromas with low proliferative index. Three months later, laparoscopic excision of the ectopic distal ureteral segment and ipsilateral ureteroureterostomy completely resolved urinary symptoms. Trio whole-exome sequencing identified heterozygous variants in SUFU (p.Asp182Asn) and FN1 (p.Asn649Ile), both inherited from unaffected parents. At 12-month follow-up, the patient had spontaneous menarche with preserved ovarian function. This unprecedented dual genitourinary presentation expands the clinical spectrum of pediatric ovarian fibromas. Combined SUFU and FN1 variants suggest possible shared mesodermal developmental susceptibility. Fertility-preserving surgery combined with staged urologic reconstruction achieved full functional recovery.
An exceptional case of sudden death due to pseudo-Meigs syndrome.
Pseudo-Meigs syndrome is a rare clinical entity characterized by the triad of ascites, pleural effusion, and a benign pelvic tumor other than an ovarian fibroma. Although typically considered benign and reversible following tumor removal, the syndrome can result in severe life-threatening compromise if unrecognized or untreated. Unlike the classical Meigs syndrome, which has occasionally been associated with fatal outcomes, no deaths related to pseudo-Meigs syndrome due to benign tumors have been previously reported. This report presents the sudden death of a young adult woman who complained of acute shortness of breath and died shortly thereafter. Postmortem examination revealed more than 2.5 L of serous fluid within the thoracic and abdominal cavities, severe pulmonary collapse, and a markedly enlarged uterus containing a large leiomyoma. Histologic evaluation confirmed the tumor as a uterine leiomyoma. The findings observed at autopsy account for the proposed pathophysiologic mechanisms of death, in which impaired gas exchange and increased intrathoracic pressure likely compromised cardiac output, leading to death. These findings support a diagnosis of pseudo-Meigs syndrome with fatal cardiopulmonary consequences. This case represents the first fatality attributed to pseudo-Meigs syndrome associated with a benign tumor. Awareness of this potential outcome is essential for both clinicians and forensic pathologists when evaluating individuals with large benign pelvic tumors and unexplained cavity effusions.
First reported case of triple benign ovarian collision tumor of the left ovary mimicking malignancy on ultrasound and CT imaging: sclerosing stromal tumor, serous cystadenoma, and fibroma.
Sclerosing stromal tumor (SST) is an uncommon benign ovarian stromal neoplasm, while serous cystadenoma is one of the most frequent benign epithelial ovarian tumors. Ovarian fibroma is another benign stromal tumor that may radiologically mimic malignancy. The coexistence of these three tumors within a single ovary is exceedingly rare. We report the case of a 45-year-old woman who presented with lower abdominal pain and urinary symptoms, in whom a large left adnexal mass was detected by pelvic ultrasonography and further evaluated by contrast-enhanced computed tomography. Imaging revealed a large complex solid-cystic mass with irregular margins, raising suspicion for ovarian malignancy. Surgical excision was performed, and histopathological examination confirmed the concurrent presence of sclerosing stromal tumor, serous cystadenoma, and fibroma within the same ovary, consistent with a collision tumor. This case emphasizes the importance of careful histopathological evaluation to avoid misdiagnosis and unnecessary aggressive treatment.
Angiosarcoma developing in an ovarian fibroma: A case report.
Angiosarcomas are malignant vascular tumors primarily located in the dermis or soft tissues predominantly in the head and neck region. Ovarian localization is extremely rare, with only a few published cases. A 63-year-old patient was admitted to the hospital due to severe pain in the hypogastric and abdominal regions. The condition was interpreted as an acute abdomen and referred to surgery, and an ovarian tumor that measured 23cm×11cm×5cm and weighed 7500g was resected. Macroscopically, the cut surface showed two well-defined areas: one consisting of a matte white tissue with a fascicular appearance and firm consistency and the other hemorrhagic with necrotic areas. Histological examination revealed two patterns: one characterized by a proliferation of spindle-shaped cells without atypia, and the other by neoplastic proliferation with marked atypia, forming vascular channels or fascicles. The vascular component was positive for CD31, CD34, and ERG. The findings were interpreted as an angiosarcoma associated with a fibroma, and the patient died in the immediate postoperative period. The low incidence and ominous prognosis of primary or metastatic ovarian angiosarcomas have been confirmed. Regarding the histogenesis, a strong likelihood exists of a relationship with the ovary's rich vascular network. In both well-differentiated and poorly differentiated cases, it is crucial to consider differential diagnoses and perform immunohistochemical techniques, which can assist in achieving an accurate diagnosis. The association between fibroma and angiosarcoma is poorly understood and the histogenesis remains unclear.
Beyond malignancy: Unravelling the Enigma of Meigs syndrome in ovarian fibroma.
Meigs syndrome is characterised by the triad of ovarian fibroma associated with ascites and pleural effusion, the latter of which resolves after removal of the fibroma. This uncommon but important clinical entity mimics malignancy in postmenopausal females. We present two such cases.
