Pleuropulmonary blastoma
A rare pediatric intrathoracic neoplasm. The tumor arises from the lung, pleura, or both, and appears to be purely mesenchymal in phenotype. It lacks malignant epithelial elements, a feature that distinguishes it from the classic adult-type pulmonary blastoma. It arises during fetal lung development and is often part of an inherited cancer syndrome. The tumor contain both epithelial and mesenchymal cells. Early in tumorigenesis, cysts form in lung airspaces, and these cysts are lined with benign-appearing epithelium. Mesenchymal cells susceptible to malignant transformation reside within the cyst walls and form a dense layer beneath the epithelial lining. In a subset of patients, overgrowth of the mesenchymal cells produces a sarcoma, a transition that is associated with a poorer prognosis. Some patients have multilocular cystic nephroma, a benign kidney tumor.

A disorder characterized by eyelid dysplasia, small palpebral fissures, drooping eyelids and a skin fold curving in the mediolateral direction, inferior to the inner canthus. In type I BPSE (BPES1) eyelid abnormalities are associated with female infertility. Affected females show an ovarian deficit due to primary amenorrhea or to premature ovarian failure (POF). In type II BPSE (BPES2) affected individuals show only the eyelid defects.

Adult ovarian granulosa cell tumor with high-grade hepatoid transformation: Case report and review of literature.
Adult Granulosa Cell Tumors (AGCTs) are uncommon ovarian neoplasms typically characterized by low-grade malignant behavior. However, AGCTs can occasionally undergo high-grade transformation, which may substantially impact clinical outcomes. In this study, we report the first documented instance of AGCT with high-grade hepatoid transformation (AGCT-HGHT) and perform a review of previously reported cases of AGCT with high-grade/sarcomatoid transformation (AGCT-HST). This case expands the spectrum of AGCT-HST and ovarian tumors with hepatoid morphology and emphasizes the need for further research into the molecular mechanisms and clinical significance of these genetic alterations as potential prognostic markers and therapeutic targets in AGCTs.
LPS-induced TLR4 activation enhances NF-κB signalling in human ovarian granulosa-like tumor cell line and induced primordial Follicle activation in mouse ex vivo ovary assay.
The immune system is increasingly recognized as a regulator of ovarian function, yet its role in early folliculogenesis remains unclear. Ovulation exhibits inflammatory-like processes involving Toll-like receptors, which is expressed in early follicles and may influence their activation. This study aims to demonstrate how the TLR4 inhibitor C34 reduces primordial follicle activation in an ex vivo ovary assay. We further examine how this effect relates to LPS stimulation and its interaction with the well-known primordial follicle activation pathway, the AKT signaling pathway. Using Western blotting, we observe increased phosphorylation of AKT. In KGN cells, LPC-induced TLR4 activation triggers NF-κB signalling and ELISA reveals production of the monocyte chemoattractant protein 1 (MCP-1). MCP-1 enhances AKT phosphorylation and cell proliferation, suggesting a link between immune signalling and follicle activation. These findings indicate that LPS-induced activation of TLR4 may interact with PI3K-AKT signalling to regulate primordial follicle activation and ovarian reserve maintenance.
Clinico-pathological characteristics of adult granulosa cell tumors of the ovary and the role of staging surgery: A national database study.
The role of staging surgery in women with Adult Ovarian Granulosa Cell Tumors (AGCT) remains uncertain. This study aimed to assess if omitting secondary staging surgery is safe amongst women who were incompletely staged at primary surgery using recurrence rate as the primary outcome. Furthermore, we sought to identify risk factors for recurrence which may be informative for selection of patients for conservative treatment. Women diagnosed with AGCT between 2005 and 2019 were identified using data from nationwide Danish Databases (DGCD and NPR). Patients undergoing primary staging surgery, those undergoing incomplete staging surgery with secondary staging and those undergoing incomplete staging not followed by secondary staging. Adjusted binary logistic regression and Cox analyses were used to assess recurrence risk and recurrence free survival. We identified 223 cases with AGCT in Denmark. The recurrence rates were 17%, and 4.8% in patients undergoing complete and incomplete primary staging surgery, respectively (p < 0.015). FIGO Stage (1A vs. ≥IC), tumor size, and high mitotic activity were identified as risk factors of recurrence. The risk of recurrence among patients who were conservatively treated after incomplete primary staging surgery was very low (4.8%), which could be explained by a low-risk profile (high proportion of stage IA, small tumors, and low/moderate mitotic activity) among these patients. In a real-world setting, omitting secondary staging surgery seems safe in primary incompletely staged patients with a low-risk recurrence profile. We confirmed that FIGO stage, tumor size, and mitotic activity are all significantly associated with the risk of recurrence in incompletely staged patients.
[Sertoli-leydig cell tumor of the ovary with rare DICER1 mutation locus: report of a case].
本文报道1例伴DICER1少见位点突变的卵巢中分化支持-间质细胞瘤。患者女,16岁。因“月经紊乱伴周期缩短4个月”入院,血常规检查睾酮及甲胎蛋白均升高。组织学呈小叶状结构,间质显著水肿;部分区呈上皮样,胞质丰富,间质硬化;部分区呈梭形;可见嗜酸性Leydig细胞散在或成簇分布;此外,本例特征性表现为,可见多灶大小不等的滤泡样结构,多位于小叶内。分子检测:Sanger测序显示DICER1第24、25号外显子均为野生型;二代测序检测显示DICER1第26号外显子错义突变c.5125G>A(p.Asp1709Asn)。术后随访28个月,未出现复发或转移。卵巢支持-间质细胞瘤形态学谱系宽泛,可伴有滤泡样结构,形态与幼年型粒层细胞瘤有重叠,对于DICER1非热点区突变的病例,二代测序检测有助于明确诊断。.
Fertility Preservation in Female Cancer Patients: Comprehensive Clinical, Surgical, and Radiologic Guide for Radiologists.
Fertility preservation is a critical consideration for young women undergoing cancer treatment, particularly in cases of gynecologic malignancies. Various options are available and include fertility-sparing treatment (FST) procedures and fertility-preserving treatment (FPT) options such as oocyte or embryo cryopreservation, ovarian tissue cryopreservation, and surgical transposition of the ovaries or uterus. In cases of endometrial cancer, FST entails hysteroscopic resection or dilation and curettage, followed by high-dose progestin therapy for early-stage low-risk tumors. To be suitable for FST, at MRI the tumor must be confined to the endometrium, without myometrial or cervical stromal invasion and without evidence of metastasis. For cervical cancer, FST such as conization or trachelectomy, in combination with lymph node assessment, may be considered if the tumor is confined to the cervix and is 2 cm or smaller in diameter. MRI is pivotal for assessing the tumor extent, largest tumor size, depth of cervical stromal invasion, and proximity of the tumor to the internal os. For ovarian cancer, FST is feasible in select cases, including those of borderline tumors, stage IA low-grade epithelial cancers, and germ cell or sex cord stromal tumors. It often is associated with high survival rates that exceed 90% in cases of early-stage disease, although the risk of recurrence varies based on histologic factors. MRI is essential for characterizing and evaluating the extent of these tumors. A comprehensive approach with imaging, histopathologic analysis, and patient counseling combined is essential for developing personalized FST and suitable FPT protocols.
