Os anticorpos (Ac) são glicoproteínas do tipo gamaglobulina, a fracção de globulinas mais abundante no plasma sanguíneo. Podem encontrar-se em forma solúvel no sangue ou noutros fluídos corporais dos vertebrados, ou podem estar inseridos na membrana plasmática, onde actuam como receptores nos linfócitos B e são empregues pelo sistema imunitário para neutralizar patógenos tais como bactérias patogénicas e viroses. Em geral, considera-se que tanto anticorpo como imunoglobulina são termos equivalentes, sendo que o primeiro termo faz referência à função, enquanto que o segundo alude à estrutura.

Ectodermal dysplasia and immunodeficiency 2

A form of ectoderma dysplasia, a heterogeneous group of disorders due to abnormal development of two or more ectodermal structures. This form of ectodermal dysplasia is associated with decreased production of pro-inflammatory cytokines and certain interferons, rendering patients susceptible to infection. EDAID2 inheritance is autosomal dominant.

A malignant disease characterized by progressive enlargement of the lymph nodes, spleen and general lymphoid tissue, and the presence of large, usually multinucleate, cells (Reed-Sternberg cells). Reed-Sternberg cells compose only 1-2% of the total tumor cell mass. The remainder is composed of a variety of reactive, mixed inflammatory cells consisting of lymphocytes, plasma cells, neutrophils, eosinophils and histiocytes.

Virus-related cancers remain a notable subset of all cancers, and many are still difficult to control, despite the fact that most infectious diseases other than emerging infectious ones can now be controlled. Among these, Epstein-Barr virus (EBV)-associated cancers, which are less common in the West and more common in East Asia, have a particularly poor prognosis. Due to the low frequency of EBV-associated cancers in Europe and the United States, little progress has been made in research and development for new drugs specific to these cancers, making drugs from East Asia essential for improving prognosis. This article focuses on EBV-associated cancers of hematopoietic origin, and describes the current status of various novel therapies, as well as the anti-transferrin antibody for aggressive NK leukemia developed by the author.

[Malignant neoplasms and transplantation].

Malignant neoplasms constitute a major burden of morbidity and mortality in the general population. This necessitates intense screening of transplant candidates and even closer surveillance of immunosuppressed solid organ recipients. Active malignancy is an exclusion criterion to solid organ transplantation, with few exceptions, namely localized hepatic neoplasms. Accelerated tumor progression characterizes post-transplantation malignancies. Intensified surveillance is justified in elevated rates, e.g., of skin cancer and virus-associated neoplasms, especially Epstein-Barr virus-associated post-transplantation lymphoproliferative disease (PTLD). Renal cell cancer rates rise after kidney transplantation, predominantly affecting the native kidneys. Chemotherapeutic dose adjustments for renal and hepatic function pharmacokinetic interactions are frequent and require active monitoring. Immunotherapies pose new challenges by induction of allograft rejection. Data on management of immunosuppression are emerging. Individualized concepts need to take into account therapeutic options of both anti-cancer therapy and organ replacement. Malignome als insgesamt zweithäufigste Todesursache spielen auch bei Transplantationskandidaten und nach Transplantation eine große Rolle. Ein ausreichender Malignomausschluss vor der Transplantation verbessert die Sicherheit der Immunsuppression. In einzelnen Fällen kann die Transplantation der Leber allerdings auch Mittel der Tumortherapie sein. Nach einer Transplantation ist eine intensivierte Tumorvorsorge notwendig. Dabei sind nach der Transplantation nichtmelanomatöse Hauttumoren extrem häufig. Typischerweise und relativ zur Allgemeinbevölkerung steigt vor allem das Risiko virusbedingter Tumoren. Hier ist wegen der signifikanten Mortalität die lymphoproliferative Erkrankung nach Transplantation hervorzuheben, die mit dem Epstein-Barr-Virus assoziiert ist. Nach Nierentransplantation sind Nierenzellkarzinome, insbesondere der Eigennieren, überzufällig häufig. Chemotherapeutikadosierungen müssen an die Leber- und Nierenfunktion angepasst werden, gerade bei neueren Substanzen sind mögliche Arzneimittelinteraktionen mit hoher Aufmerksamkeit zu recherchieren. Die bei vielen Tumorentitäten erfolgreichen Immuntherapien sind nach Organtransplantation eine große Herausforderung, da sie regelhaft auch eine Immunreaktion gegen das Transplantat hervorrufen. Hier muss individuell entschieden werden, auch danach, ob das Transplantat lebenserhaltend ist oder gegebenenfalls durch Dialyse ersetzt werden kann.

Impact of Antibody Immune Response and Immune Cells on Osteoporosis and Fractures.

The immune system plays a critical role in the development and progression of osteoporosis and fractures. However, the causal relationships between antibody immune responses, immune cells, and these bone conditions remain unclear. This study aimed to explore these relationships using Mendelian randomization (MR) analysis. We collected complete blood count data from patients with fractures and healthy individuals and analyzed their differences. Then, we conducted a 2-sample, 2-step MR analysis to investigate the causal effects of antibody immune responses on osteoporosis and fractures, using inverse-variance weighted (IVW) as the primary method. We also explored whether immune cells mediate the pathway between antibodies and osteoporosis or fractures. Finally, we analyzed the functions and expression levels of key genes involved. Overall, the fracture group exhibited increased white blood cell count, absolute neutrophil count, absolute monocyte count, platelet count, and their respective proportions, while absolute lymphocyte count, absolute eosinophil count, absolute basophil count, red blood cell count, and their proportions were decreased. We identified 44 causal relationships between antibodies and osteoporosis or fractures, with 7 supported by multiple MR methods, and 5 showing odds ratios significantly deviating from 1 in the IVW analysis. Epstein-Barr virus-related antibodies had a notable impact on osteoporosis and fractures. The human leukocyte antigen (HLA) gene family, particularly HLA-DPB1, emerged as a significant risk factor. However, immune cells were not found to mediate these effects. This study elucidated the causal relationships between antibody immune responses, immune cells, and osteoporosis or fractures. The HLA gene family plays a crucial role in the interaction between antibodies and these bone conditions, with HLA-DPB1 identified as a key risk gene. Immune cells do not serve as mediators in this process. These findings provide valuable insights for future research.

Clinical features and risk factors for Epstein-Barr virus-associated encephalitis: a retrospective cohort study.

Epstein-Barr virus (EBV) has been gradually recognized as an important pathogen of encephalitis, but our knowledge of EBV-associated encephalitis is still limited. The aim of this study was to describe the clinical features and explore the risk factors for EBV-associated encephalitis in a large cohort of Chinese patients. All patients with confirmed encephalitis in our center from June 2020 to April 2021 were enrolled. Data were extracted from the electronic medical record system and analyzed by Student's t test, Mann-Whitney U test, chi-square test, and logistic regression analyses. This study included a total of 364 patients diagnosed with encephalitis, among which 86 cases (23.6%) had EBV detected in their cerebrospinal fluid (CSF), and 39 cases were diagnosed with EBV-associated encephalitis. The clinical characteristics of EBV-associated encephalitis differ from those caused by other herpes viruses. Compared to other herpes virus-related encephalitis, patients with EBV encephalitis exhibited significantly higher protein levels (1.310 [0.695, 1.840] vs. 0.710 [0.490, 1.700], p < 0.001), lower glucose levels (3.030 [2.585, 3.640] vs. 3.380 [2.990, 4.030], p < 0.001), and a higher incidence of meningeal involvement on MRI (11 (30.6%) vs. 3 (6.5%), p = 0.010). Additionally, univariate logistic regression analysis revealed that age (OR = 1.015, 95% CI: 1.002-1.029), HIV infection (OR = 4.285, 95% CI: 1.582-11.816), non-tumor immunosuppression (OR = 5.713, 95% CI: 1.588-22.883), and peripheral blood EBV infection (OR = 10.204, 95% CI: 4.231-26.346) were independent risk factors for CNS EBV infection. Compared to other herpes virus-associated encephalitis, EBV encephalitis is characterized by a higher degree of meningeal involvement, elevated CSF protein levels, lower glucose levels, and a reduced T-lymphocyte count in peripheral blood. These characteristics suggest that EBV encephalitis may have a distinct pathophysiology.
