A Phase 1 Study of Neoadjuvant Cabozantinib in Combination With Radiation Therapy for Sarcomas of the Extremities.
Cabozantinib demonstrates activity in multiple soft tissue sarcoma (STS) subtypes, but use with concurrent radiation therapy (RT) has been limited by concern for risk of fistula or perforation. This phase 1 trial evaluated the safety of concurrent cabozantinib and RT as neoadjuvant therapy in patients with extremity STS. Adults with newly diagnosed localized extremity STS planned for neoadjuvant RT and surgical resection were eligible. Participants received radiation with 5000 to 5040 cGy with conventional fractionation and cabozantinib 40 mg or 60 mg daily. Patients were observed for dose-limiting toxicity (DLT) up to 28 days after completion of concurrent cabozantinib/RT. The primary objective was to identify a recommended phase 2 dose (RP2D) of cabozantinib for combination with RT, and secondary objectives included estimating rates of treatment-related adverse event (TRAE), margin positivity, and objective response. Six patients were enrolled with histologic subtypes of undifferentiated pleomorphic sarcoma, myxofibrosarcoma, leiomyosarcoma, and myxoid liposarcoma. No DLT due to the combination of cabozantinib and radiation was observed, but 3/6 (50%) patients required dose-reduction due to TRAE of cabozantinib alone. No grade ≥3 toxicities were attributed to RT. The RP2D was cabozantinib 60 mg. Six (100%) patients demonstrated stable disease at 12 weeks, and 5 (83%) underwent R0 resection. Two (33%) patients experienced metastatic relapse, and 1 (17%) died without relapse; 3 (50%) patients survived without relapse by last contact. No local recurrences occurred. In this phase 1 trial, concurrent cabozantinib/RT was feasible and demonstrated an acceptable safety profile for patients with extremity STS.
Real-world characterization of patients with advanced or metastatic liposarcoma, including dedifferentiated liposarcoma, in Japan in the MASTER KEY project.
Efficacy of available treatments for liposarcoma (LPS), including dedifferentiated liposarcoma (DDLPS) is limited, and detailed real-world clinical data for patients with LPS treated in Japan are scarce. We used the MASTER KEY Registry Database to evaluate the clinical characteristics and outcomes of patients with advanced/metastatic LPS treated with systemic antineoplastic therapy in Japan. This non-interventional/observational cohort study included patients with LPS, including DDLPS, and well-differentiated LPS (WDLPS), and a history of systemic antineoplastic therapy, enrolled in the MASTER KEY Project Registry Database from 1 May 2017 to 31 December 2022. Outcomes were analysed by type of treatment. Primary outcomes: progression-free survival (PFS); overall survival (OS). Secondary outcomes: overall response rate; disease control rate. Sensitivity analyses (prospective registry subgroup) were performed to minimize immortal time bias. Since 2017, 100 patients with LPS were prospectively enrolled in the database; of these, 62 had DDLPS and 11 had WDLPS. In patients with LPS, median PFS was 11.1 months after first-line (1 L) pharmacotherapy and 6.9 months after 1 L doxorubicin. In patients with DDLPS, median PFS was 6.9 months after 1 L pharmacotherapy and 4.4 months after 1 L doxorubicin. Median OS after 1 L pharmacotherapy was 45.8 months in patients with LPS, and 40.7 months in those with DDLPS. Median OS in the sensitivity analysis was not reached for patients with LPS and was 13.9 months for the DDLPS subgroup. These data investigating DDLPS in Japanese patients highlight the poor outcomes and lack of effective treatment options in the real-world clinical practice setting.
Immunoreactivity for CD34, Desmin, Keratins, KIT, Alpha-Smooth Muscle Actin, S100, and Vimentin in Malignant Mesenchymal Neoplasms in Guinea Pigs: A Series of 62 Cases From a Single Institution.
There has been a lack of research regarding diagnostic immunohistochemistry in soft tissue pathology in rodents. We aimed to analyse the immunohistochemical expression of CD34, keratin AE1/AE3, keratin 8/18, desmin, KIT, SMA, S100, and vimentin in various malignant mesenchymal tumours in guinea pigs. A total of 62 tumours included 10 fibrosarcomas, 8 myxosarcomas, 8 undifferentiated pleomorphic sarcomas, 6 undifferentiated round cell sarcomas, 6 dedifferentiated liposarcomas (involving deep soft tissue, skin or subcutis), 12 uterine leiomyosarcomas, 4 hemangiosarcomas (involving skin and spleen), and 8 gastrointestinal stromal tumours (GIST). Desmin and SMA positivity was present in tumours of various lineages, including hemangiosarcoma. CD34 positivity was restricted to hemangiosarcomas, GISTs and one case of undifferentiated pleomorphic sarcoma, whereas KIT positivity was detected only in GISTs. Lesser amounts of keratins positivity were observed in fibrosarcoma, myxosarcoma, undifferentiated pleomorphic sarcoma, and dedifferentiated liposarcoma. S100 positivity was identified in myxosarcoma, undifferentiated pleomorphic sarcoma, and dediffentiated liposarcoma and vimentin positivity was detected in all tumour types. The strong and diffuse staining pattern of desmin and SMA can be very helpful in distinguishing leiomyosarcoma from its spindle cell mimics. A novel finding is that SMA positivity was identified in malignant endothelium. Immunolabelling for CD34 and KIT provides reliable markers for vascular neoplasms and GISTs. Vimentin does not allow to distinguish between different mesenchymal malignancies in guinea pigs. Occasional positivity for keratins and S100 points to potential pitfalls and emphasised the need for a panel of immunohistochemical stains in the investigation of mesenchymal tumours.
Sarcopenia as an independent prognostic marker in liposarcoma: A longitudinal analysis of body composition and survival.
Sarcopenia is increasingly recognized as an important prognostic factor in oncology; however, its clinical relevance in liposarcoma remains insufficiently defined. This study aimed to evaluate longitudinal changes in CT-derived body composition parameters in liposarcoma patients, to assess the influence of tumor grade, recurrence, and treatment modalities on these parameters and to determine the association of baseline sarcopenia and progressive muscle loss with overall survival and functional status. In a retrospective, single center study between 2010 and 2024, 64 patients were analyzed. All patients underwent surgical tumor resection of a histologically confirmed liposarcoma. Included were patients with two consecutive CT scans. The following morphometric parameters were measured on CT axial images at the height of lumbar vertebral 3: Skeletal muscle index (SMI), paraspinal muscle index (PSMI), psoas muscle index (PMI), skeletal muscle density (SMD), and visceral adipose tissue (VAT). Standardized Hounsfield unit thresholds were used for the assessment. Additionally, the influence of tumor grade, recurrence, and treatment modalities on body composition was assessed. A Kaplan Meier survival analysis was performed using data from the residents´ registration office. Survival was further analyzed by Cox regression using uni- and multivariate models. Metric data was compared using student´s t-test. Significant reductions in SMI, PSMI, PMI, and VAT were observed over the disease course, particularly among patients with high-grade tumors, chemotherapy, or local tumor recurrence. Baseline sarcopenia and a progressive SMI loss were independently associated with reduced overall survival. In multivariate analysis, baseline sarcopenia (HR: 2.331, p = 0.007) and a ≥ 15% SMI decline (HR: 2.601, p = 0.006) remained significant predictors of mortality. Both markers did not correlate with changes in Eastern Cooperative Oncology Group (ECOG) performance status. CT-morphometric parameters deteriorate substantially during the disease course of liposarcoma patients and serve as independent predictors of survival. These findings support the integration of CT-based body composition analysis into routine oncologic assessment and highlight its potential role in identifying high-risk patients for early supportive intervention.
Molecular and Immune Mechanisms Governing Cancer Metastasis, Including Dormancy, Microenvironmental Niches, and Tumor-Specific Programs.
Metastasis is still the leading cause of cancer-related death. It happens when disseminated tumor cells (DTCs) successfully navigate a series of steps and adapt to the unique conditions of distant organs. In this review, key molecular and immune mechanisms that shape metastatic spread, long-term survival, and eventual outgrowth are examined, with a focus on how tumor-intrinsic programs interact with extracellular matrix (ECM) remodeling, angiogenesis, and immune regulation. Gene networks that sustain tumor-cell plasticity and invasion are described, including EMT-linked transcription factors such as SNAIL and TWIST, as well as broader transcriptional regulators like SP1. Also, how epigenetic mechanisms, such as EZH2 activity, DNA methylation, chromatin remodeling, and noncoding RNAs, lock in pro-metastatic states and support adaptation under therapeutic pressure. Finally, proteases and matrix-modifying enzymes that physically and biochemically reshape tissues, including MMPs, uPA, cathepsins, LOX/LOXL2, and heparinase, are discussed for their roles in releasing stored growth signals and building permissive niches that enable seeding and colonization. In parallel, immune-evasion strategies that protect circulating and newly seeded tumor cells are discussed, including platelet-mediated shielding, suppressive myeloid populations, checkpoint signaling, and stromal barriers that exclude effector lymphocytes. A major focus is metastatic dormancy, cellular, angiogenic, and immune-mediated, framed as a reversible survival state regulated by stress signaling, adhesion cues, metabolic rewiring, and niche constraints, and as a key determinant of late relapse. Tumor-specific metastatic programs across mesenchymal malignancies (osteosarcoma, chondrosarcoma, and liposarcoma) and selected high-burden cancers (melanoma, hepatocellular carcinoma, glioblastoma, and breast cancer) are highlighted, emphasizing shared principles and divergent organotropisms. Emerging therapeutic strategies that target both the "seed" and the "soil" are also discussed, including immunotherapy combinations, stromal/ECM normalization, chemokine-axis inhibition, epigenetic reprogramming, and liquid-biopsy-enabled minimal residual disease monitoring, to prevent reactivation and improve durable control of metastatic disease.
