Neuroblastoma-associated opsoclonus-myoclonus-ataxia syndrome: an important yet overlooked diagnosis in pediatric ataxia.
Opsoclonus-myoclonus-ataxia syndrome (OMAS) is a rare, immune-mediated neurological disorder, often associated with neuroblastoma (NB) in children. This study aimed to describe the clinical features, diagnostic challenges, treatment, and outcomes of pediatric patients with NB-associated OMAS. We retrospectively reviewed medical records of seven children diagnosed with NB-associated OMAS between November 1, 2015 and January 31, 2025. Data on demographics, clinical presentation, tumor characteristics, treatment protocols, relapses, and outcomes were collected. The Mitchell-Pike OMS Rating Scale was used to assess severity. The cohort included four girls and three boys, aged 18-36 months (median 31). All had a history of infection and/or vaccination preceding symptom onset. Ataxia was the most common initial symptom (6/7, 85.7%), followed by behavioral disturbances and sleep disorders (5/7, 71.4% each). At presentation, opsoclonus was present in two patients, which was initially misdiagnosed as nystagmus. All but one patient had opsoclonus during the disease course. Neuroblastomas were located in abdominal (n=4), thoracic (n=1), and sacral (n=2) regions; all tumors measured.
HLA and T-Cell Receptor Investigations in Idiopathic and Paraneoplastic Opsoclonus-Myoclonus in Children.
Opsoclonus-myoclonus-ataxia syndrome (OMAS) is a rare pediatric autoimmune neurologic disorder, with 40%-50% of cases occurring as a paraneoplastic syndrome in the setting of neuroblastoma or other neuroblastic tumors. The strong clinical similarities between idiopathic and paraneoplastic OMAS have led to the hypothesis that neuroblastoma may initially be present in idiopathic cases but spontaneously regress before OMAS onset. Both B- and T-cell mechanisms appear to contribute to OMAS autoimmunity, although their precise roles remain unknown. The aim of this study was to investigate similarities and differences between idiopathic and paraneoplastic OMAS by analyzing HLA profiles and T-cell receptor (TCR) repertoires in a Spanish pediatric cohort. Children with OMAS were enrolled in a nationwide prospective study (2013-2024). Neural autoantibody testing was performed in all patients. HLA genotyping was performed using next-generation sequencing (GenDx/Illumina) and compared with a reference Spanish population (n = 4,335). The Benjamini-Hochberg false discovery rate method was used to control for multiple comparisons across HLA antigens. TCR repertoires were analyzed in peripheral blood samples of 12 patients using the Adaptive Biotechnologies immunoSEQ assay. The bioinformatics tool Grouping of Lymphocyte Interactions by Paratope Hotspots (GLIPH2) was used to identify shared motifs within CDR3β-TCR sequences. Among 25 children with OMAS (median age 22 months [interquartile range (IQR): 15-38]), 12 (48%) had neuroblastoma and 3 (12%) had neuronal antibodies (one with anti-Hu, one with anti-myelin oligodendrocyte glycoprotein, one against unknown neuronal surface antigens). Some HLA class II antigens (DRB1*08, DRB1*10, and DQB1*04) were consistently overrepresented in patients with OMAS compared with Spanish controls (all p-adjusted <0.05). Distinct HLA antigens were associated with idiopathic OMAS (HLA-DRB1*08, HLA-DQB1*04), while paraneoplastic OMAS was linked to HLA-DRB1*10 (all p-adjusted <0.05), suggesting subgroup-specific genetic risks. TCR analysis revealed that patients with paraneoplastic OMAS showed greater TCR repertoire similarity, whereas shared sequences were rare in idiopathic OMAS. Despite similar clinical presentations, idiopathic and paraneoplastic OMASs appear to have distinct immunopathogenic mechanisms. In paraneoplastic OMAS, enrichment of specific HLA antigens and convergent TCR profiles supports antigen-driven T cell-mediated autoimmunity.
Opsoclonus-myoclonus syndrome with anti-SOX1 antibodies.
High Expression of CCL2 Correlated With Dendritic Cell Recruitment in Neuroblastomas Associated With Opsoclonus-Myoclonus Syndrome.
Neuroblastoma (NB) accounts for 10% to 15% of cancer-related deaths among children. In rare cases, NB is associated with opsoclonus-myoclonus syndrome (OMS), a neurological paraneoplastic syndrome. These low-risk (LR) NBs with OMS have a favorable prognosis and an immune-enriched microenvironment. The recruitment of dendritic cells (DCs) and T lymphocytes into tumors is known to be mediated by CCL2 and CXCL12, respectively. We investigated whether intratumor CCL2 and CXCL12 promote DC recruitment within the microenvironment of OMS-associated LR-NBs. We conducted a multicentric retrospective case-control study on 44 pediatric patients with NBs associated or not with OMS. The non-OMS population was subdivided into an LR-NB group versus a high-risk (HR)-NB group, with or without MYCN amplification. Immunohistochemistry was performed using anti-CCL2 and anti-CXCL12 antibodies. DC subpopulations were stained using OPAL spectral multiplex immunofluorescence. Immunostaining scores were established by semiquantitative optical analysis. Gene signatures of DC, as well as CCL2 and CXCL12 mRNA expressions, were also analyzed on a public RNA-Seq data set of NB. We observed a significantly higher protein expression of CCL2 in OMS-associated LR-NBs compared with MYCN-amplified HR-NBs, whereas CXCL12 was not preferentially expressed by any of the NB groups. Furthermore, DC infiltrate was more abundant in OMS-associated LR-NBs compared with any other NB risk groups and positively correlated with CCL2 expression. In silico analysis of public RNA-Seq data demonstrated a higher CCL2 expression in LR-NB with or without OMS compared with HR groups, and that the DC transcriptomic signature was higher in OMS-associated LR-NBs compared with HR-NB groups. We also observed a positive correlation between the DC gene signature and CCL2 or CXCL12 mRNA expression, to a lesser extent. Our results suggest that a DC-prone microenvironment might explain the favorable oncologic outcome of OMS-associated LR-NBs. CCL2 could be a therapeutic target to mobilize DCs in HR-NBs or in tumors characterized by a paucity of tumor-infiltrating DCs.
Prolonged Diagnostic Latency and Long-term Surveillance in Anti-Ri Antibody-positive Opsoclonus-myoclonus Syndrome with Carcinoma of Unknown Primary.
Opsoclonus-myoclonus syndrome (OMS) is a rare paraneoplastic neurological disorder that is usually associated with breast cancer or small-cell lung carcinoma. We report the case of a 71-year-old woman with anti-Ri antibody-positive OMS, in whom no malignancy was initially identified. Thirty months later, carcinoma of an unknown primary tumor with supraclavicular and liver metastases was diagnosed. Neurological symptoms remained stable with immunosuppressive therapy, but the overall outcome was poor owing to progressive malignancy. This case underscores the fact that neurological stability does not necessarily equate to a favorable prognosis, and highlights the need for long-term surveillance in cases involving paraneoplastic syndromes.
